About this file
This is a Questions and Answers (Q&A) document from a federal contracting solicitation for Centralized Long-term Care (CLC) survey services with the Department of Veterans Affairs (VA). The document addresses eight questions raised by potential offerors regarding proposal submission requirements, technical specifications, and pricing.
Key clarifications from the Q&A include: (1) the WOSB set-aside will remain unchanged to maintain the Woman Owned Small Business designation; (2) SMQT certification is not required for surveyors, with applicable Title 42 Public Health Code Federal Regulations Part 483 components governing qualifications; (3) Life Safety survey portions must occur concurrently with clinical survey activities; (4) the VA will provide operating bed counts and average daily census data via Task Orders; (5) the Quality Assurance and Surveillance Plan (QASP) is excluded from the 150-page technical volume limitation; (6) Survey Schedules are required as part of Volume I proposals; (7) travel and maintenance expenses must be estimated in all Contract Line Items requiring travel; and (8) Attachment G establishes the monthly survey schedule that offerors should use for planning purposes, with surveys occurring the same month annually. These clarifications address proposal formatting, staffing qualifications, survey logistics, and cost estimation methodologies relevant to offerors developing their proposals.

Section 10 of the Standard Form
We are aware of very few contractors (less than 5) that are qualified to complete the full scope of work in the RFP. Using the Woman Owned Small Business (WOSB) designation for this opportunity likely eliminates all the qualified bidders except for one. Would the government consider changing the set aside to Small Business instead of WOSB to increase competition?
Not at this time.
B.4 Performance Work Statement A.2 General Requirements 7
Proposed Modification To promote competition and ensure the VA continues to receive the highest-caliber expertise, we propose the following change to the technical requirements:
"Surveyors must be SMQT-certified OR possess equivalent experience (e.g., 10+ years of healthcare facility survey experience, MHA/clinical degree, and mastery of 38 CFR Part 51).
SMQT certification is not a requirement for this acquisition.. Applicable components of Title 42 Public Health Code Federal Regulations Part 483
B.4 Performance Work Statement C.4. Activity 3:
Surveys 19
Are the Life Safety portions of the surveys required to occur concurrently with the remainder of the survey team activities, or may the Life Safety surveyor conduct the assessment on separate dates from the clinical survey team? If separate scheduling is permitted, is there a required timeframe (e.g., number of days or weeks) within which the Life Safety survey must occur relative to the clinical survey component?
Yes, the Life Safety portion of the CLC Unannounced Survey is required to occur concurrently with the Clinical portion of the CLC Unannounced Survey.
B.4 Performance Work Statement C.4. Activity 3:
Surveys
Please provide the most recently available Average Daily Census (ADC) for each CLC and SCI/D Long Term Care Center.
The Program Office will provide the contractor the number of operating beds and the average daily census by means of TO(s).

Proposal Files
The page-limit table indicates that the Volume I subsections total 135 pages, while the QAP and QASP are identified as having “No Page Limitation.” Please confirm whether the Government intends the QAP and QASP to be excluded from the 135-page limitation (i.e., additive to the cap), or whether those sections count toward the overall 135-page total.
The QASP is a separate document and one of the first deliverables. It is not included in the 150 page limitation of the technical volume submission.

Proposal Files and 4. Technical Approach
100-101
Please clarify whether Survey Schedules are required within Volume I of the proposal submission. The Technical Volume summary on page 100 appears to omit Survey Schedules from Volume I requirements; however, Factor 1 on page 101 references Survey Schedules.
pg 100 table updated to reflect "1. Survey Schedule" and page limit.
E.5 Proposal Submission Instructions 6.
Price Volume Factor 2
How will surveyor travel and maintenance expenses (e.g., lodging, meals, airfare, rental vehicles) be accounted for in this Solicitation? Are offerors expected to include travel and maintenance expenses within CLIN 001 (CLC Annual Onsite Surveys) or should travel and maintenance costs be excluded from pricing proposals?
Travel should be estimated in all CLINs that require travel.

G
Should Attachment G (Attachment G Survey Due By Dates) be used by offerors to determine the proposed survey schedule, or is the attachment intended only as an illustrative example? Additionally, could the Government provide the latest survey dates by facility?
Yes, Attachment G shows which facilities are surveyed each month. No, latest survey dates are not required. Survey should take place this month every year.
