About this file

The document is a Construction Bid Submission Review Form issued by the State of West Virginia Purchasing Division, designed to provide guidance for potential contractors bidding on construction projects. The form outlines critical errors that can result in immediate bid disqualification, including failure to attend mandatory pre-bid meetings, not signing the bid, lacking a valid bid bond, missing mandatory solicitation requirements, late bid submission, and federal or state debarment status.

The review form details additional potential disqualification reasons before contract award, such as not acknowledging addenda, having outstanding state debt, workers' compensation or unemployment compensation delinquencies, lack of vendor registration, insufficient bonds or insurance, incomplete subcontractor listings, missing contractor's license number, and absence of a signed drug-free workplace affidavit. The document emphasizes that this checklist is informational and not comprehensive, strongly advising bidders to thoroughly review all bid documents to prevent potential disqualification and understand the complete bidding requirements.
