About this file

This is a Certified Drug-Free Workplace Report Coversheet issued by the State of West Virginia Purchasing Division in accordance with West Virginia Code 21-1D-7b. The form serves as a mandatory annual reporting requirement for all contractors working on state contracts, with submission required upon project completion or annually, whichever occurs first. Contractors must provide detailed documentation of their drug-free workplace compliance efforts, including evidence of employee education and training programs that meet West Virginia Code 21-1D-5 requirements. The coversheet requires vendors to identify the HHS-certified laboratory conducting drug testing, report the average number of employees assigned to the public improvement project, and provide comprehensive drug test results categorized by pre-employment and new hires, reasonable suspicion, post-accident, and random testing, including both positive and negative test counts.

Submission procedures are based on contract value thresholds: contracts exceeding $25,000 must be submitted to the West Virginia Purchasing Division at 2019 Washington Street East, Charleston, WV 25305, while contracts of $25,000 or less are submitted directly to the public authority issuing the contract. The vendor must complete all required sections of the coversheet, including contract identification number and purpose, requesting agency, vendor contact information, and verification checkboxes confirming that all required report content has been included in the attached documentation.
