1. How many vacancies are there on the current contract?
Answer: 3 Nurse Case Managers, 1 for the 81st RD which is in SC, 1 for the 63rd RD which is in CA and 1 for the 88th RD which is in WI.
2. I am wondering if we are supposed to be filling in the boxes on the front page of the form related to pricing including discount terms, unit price and total amount. Our pricing for Schedule B is included with the Pricing volume so wasn’t sure if I was supposed to include total amount on the SF 1449.
Answer: Yes, please fill out the front page in accordance with the instructions in 52.212-1 and the addendum.
NOTE: Soliciation complies with FAR 15.101© and DFARS 215.101-2-70.

Paragraph (a) is hereby supplemented with the following:
a. The Award will be made using best value Lowest Priced Technically Acceptable (LPTA) procedures. The government will initially list proposals from lowest to highest price. The government will evaluate the Technical Acceptability of the five (5) lowest priced proposals first. If one of those proposals are rated Technically Acceptable, the government will not evaluate any other proposals. If all of those proposals are determined technically unacceptable, the government will evaluate the next five (5) lowest price proposals and continue in that manner until a proposal is rated technically acceptable. If a proposal is rated Technically Unacceptable , the government will not evaluate any other factor. USING THIS PROCEDURE, THE GOVERNMENT MIGHT NOT
EVALUATE ALL PROPOSALS.
Offers will be evaluated to determine compliance with all requirements of the solicitation, including any attachments and exhibits. The evaluation criteria will be applied to all offers in the same manner. Each proposal will be evaluated strictly in accordance with its content and the Government will not assume that performance will include areas not specified in the offeror's proposal.
Factor 1 - General - The Government shall review the following:
Solicitation and any Amendments for signatures;
Representation and Certifications;
Factor II - TECHNICAL EVALUATION.
The proposal technical factors and subfactors will be evaluated to determine the adequacy of the offeror’s proposed compensation to individual health care providers HCPs for the local area. The primary determinant will be the base salary and fringe benefits paid to the individual HCP. Offerors must propose a base salary and fringe benefits which is at least commensurate with the salaries of similar health care providers in the local area in order to be technically acceptable. This salary in almost all instances exceeds the prevailing wage rates in the Department of Labor’s wage determination for a locality. The Government has developed an independent government estimate (IGE) based upon its assessment of base salaries paid in the local area as well as salaries paid on current contracts. A low, or no Compensation Plan, may be viewed as evidence of failure to comprehend the complexity of the contract requirements or appropriate compensation levels for geographic area where performance will take place. Plan must fully address how the offeror intends to perform the requirement Offerors which propose low base salaries and fringe benefits, particularly below rates paid in their local area, will have the burden of establishing that their plans will result in the recruitment and retention of quality personnel and the uninterrupted performance of services throughout the life of the contract.
Technical factors as follows:
Subfactor 1: Recruitment Plan - Submit a plan how they will recruit qualified personnel in time to work on day one of the performance period. There are currently incumbents for the Lead Psychological Health Director, Directors of Psychological Health and Nurse Case Managers.
Subfactor 2: Retention Plan - Submit a plan how they will retain qualified personnel to provide continuity of care throughout the performance period.
Subfactor 3: Training and Immunization Plan - Offeror will submit its plan demonstrating how the offeror will track training and immunizations.
Subfactor 4: Compensation Plan – Proposal will include a compensation plan for each provider type for the base and four option periods.
Subfactor 5: Travel Plan - Explain how your company will handle the travel expenses and reimburse the employee. For example does your company issue credit cards for travel. Or does the employee have to pay for travel up front and then be reimbursed.
Subfactor 6: Transition Plan: Submit their plan how to adequately provide services on day one of the period of performance to meet Womack's Army Mission. Currently there are incumbents for the Lead Director of Psychological Health, and the Directors of Psychological Health.
Subfactor 7: Management Plan & Quality Control Plan: The subfactor will describe how the company plans to manage the program in accordance with the PWS and control the quality of the program.
I f any technical subfactor received an unacceptable rating, the combined technical will be unacceptable.
The recruitment and retention plan will evaluate the various incentives the company will offer the HCPs.
The Technical Evaluation Subfactors shall not exceed 50 pages; additional pages will not be reviewed or evaluated.
Technical merit will be evaluated as either Acceptable or Unacceptable.
Definitions are as follows:
Technical Acceptable/Unacceptable Ratings:
Acceptable Proposal clearly meets the minimum requirements of the solicitation.
Unacceptable Proposal does not clearly meet the minimum requirements of the solicitation.
Note: To receive consideration for award, a rating of acceptable must be achieved for all technical factors.
The technical subfactors are of equal importance. To be rated technically acceptable, offerors must receive a rating of acceptable for each factor. A rating of unacceptable in either subfactor will result in an unacceptable rating for the technical factor. For those proposals rated technically unacceptable the government will not evaluate any other evaluation factors.
Factor III: PAST PERFORMANCE. Offeror is required to submit information, on a single page, identifying federal, state and local Government and private contracts performed within the past three years, which are similar in scope and complexity to the effort described in this solicitation. If none, offeror should so state and provide information on references. Note - past performance will be accepted from a sub-contractor / teaming partner if the prime contractor does not have relevant past performance.
Information provided by the Contractor shall state: The type of service provided with information to verify references: Names; telephone numbers; Points of Contact; amount of contract; and email addresses.
The information which will be used for evaluation will be obtained from those presented in the Offeror’s past performance documentation, the Contractor Performance Assessment Reporting Systems (CPARS) http://www.cpars.csd.disa.mil/cparsmain.htm, and any other sources. The Government may reject a proposal if it is found that any information that has a negative impact on the offeror’s past performance record has been deleted, misrepresented, or withheld. Past performance surveys should be based upon performance of like services to those in this solicitation. Past performance will be rated on an “acceptable” or “unacceptable” basis using the ratings below:
Past Performance Evaluation Ratings
Acceptable Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown. (See note below.)
Unacceptable Based on the offeror’s performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort Note: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a) (2) (iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.”
Factor IV: PRICE. Price will be evaluated separately from the technical and past performance factors and will be evaluated for fairness and reasonability in terms of:
A. That the prices are consistent with and reflect the special pricing for staffing requirement.
B. All vendors’ prices must be deemed to be fair and reasonable to qualify for award and within the guidelines of the special pricing.
C. In Accordance With FAR 15.404-1, price fair and reasonableness will be used to evaluate an offeror’s proposal, but price will not be scored. Pricing will be evaluated to determine if fair . The Government is interested in proposals that meet the requirements with acceptable risk, at a fair and reasonable price.
D. Given that FAR 52.217-8 provides that the Government may exercise the Option to Extend Services at the rates specified in the contract (i.e., the rates in effect when the Extension of Services option is exercised), for purposes of evaluating the Option to Extend Services, the Contracting Officer will consider the prices submitted for the base year and each option year, since those are the binding prices should the Option to Extend be exercised.
ADDENDUM TO 52.212-1
Para (b) Submission of Offers. The following supplements this paragraph with respect to the information and documents required for submission in response to this solicitation.
(1) ELECTRONIC OFFERS: Offerors may submit electronic offers in response to this solicitation. All offerors will submit proposals for the following evaluation factors no later than the closing date and time noted on the SF 1449.
Proposals must be submitted by 11 AM EST on the closing date. Proposals will not accepted after the closing date and time.
Part I - General: Offerors are required to submit a completed Standard Form (SF) 1449 (Solicitation/ Contract/ Order for Commercial items) (including acknowledge of Commercial items). All final monetary extensions shall be no more than two decimal places. Extensions beyond two decimal places will not be used in price calculation.
Failure to follow the below Contract Proposal preparation instructions may cause your proposal to be deemed unacceptable by the government. The General Volume shall be organized as follows and contain the identified information.
- SF 1449 - The SF1449 shall be submitted fully completed. The offeror is cautioned that the SF 1449 must contain a signature in block 30A of the form. The Contractor shall acknowledge any amendments to the RFP in accordance with the instructions on the SF 1449 and with FAR Clause 52.212-1, Instructions to Offerors - Commercial Items. The Offeror shall provide the name, title and telephone number of the company/division point contractually. Also, identify those individuals authorized with the Government.

Items shall be completed with all blocks in each certification representation completed accurately.
- Executive Summary - Provide an executive summary that introduces the contractor team and proposal submission, to include a list of team members and subcontracts and the task areas they are designated to provide support. The summary shall also include a list of the proposal package contents and identify the proposal's point of contract (POC) to include their name, email address, and phone number.

Award will be made to the offeror provided its technical proposal conforms to the requirements of the solicitation; it has an acceptable record of past performance; price is determined to be fair and reasonable by the contracting officer; and the contracting officer determines the offeror is responsible.
The offer must arrive by the time specified in the solicitation to the Contract Specialist, Ms. Naomi Williams and to the Contracting Officer, Ms. Cheryl Ricker at the following email addresses: naomi.r.williams8.civ@mail.mil & cheryl.a.ricker2.civ@mail.mil
(2) Offerors must request verification by email to Mrs. Naomi Williams to confirm that their proposal was received by the time specified date and time noted in the solicitation. The Government will not be responsible for any failure of transmission; or receipt of the offer, and or any failure of the offeror to verify receipt of the emailed offer. The government is not responsible for late proposals received.
(3) The Government intends to award a firm fixed-price non personal services contract under the authority of U.S.C.
2304 and 41 U.S.C. 253. Accordingly, offerors are instructed to insure that their offered rates are fair and reasonable. Offerors must propose a firm-fixed price for each Contract Line Item.
(4) Any questions with respect to the solicitation shall be submitted via email not later than 9 August 2021 by 2 PM EST. Questions will not be answered if they are received late. Please submit them to the Contracting Officer, Ms.
Cheryl A Ricker (cheryl.a.ricker2@civ.mail.mil) and the Contract Specialist, Ms. Naomi Williams (naomi.r.williams8.civ@mail.mil). Telephone calls will not be responded to. Responses will be posted on BetaSAM
(5) BASIS FOR AWARD. The Government will make a Lowest Price Tenchincally Acceptable (LPTA) award consistent with the evaluation criteria discussed in FAR 52.212-2 and the Addendum to FAR 52.212-2. Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror’s initial offer should contain the offeror’s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer;
and waive informalities and minor irregularities in offers received.
Factors to be evaluated are listed in paragraph 1 above. Award will be made to the responsible offeror whose proposal conforms to all solicitation requirements, such as terms and conditions, representations and certifications, technical requirements, and also provides the best value to the Government based on the results of the evaluation described in Addendum 52.212-2.
The Contracting Officer intends to award without discussions, but reserves the right to do so at his or her sole discretion. The Government intends to award a firm fixed price contract,
(6) The Government may reserve the right to reject a proposal if the offeror misrepresents or withholds past performance information or information regarding its certifications or business condition, or its capability to perform the requirements of this contract.
(7) SUBMISSION OF OFFERS. The solicitation shall be completed, signed and received at this office not later than the date/time specified in Block 8 of the solicitation (Standard Form [SF] 1449). The solicitation must be received no later than the designated time identified on page one (1) block 7(a). This date and time is considered received no later than the "Eastern Standard Time Zone". Please note that your offer must include all of the following information. The solicitation shall be submitted in four (4) volumes:
Volume I -General Schedule (Signed Solicitation (front page), and any solicitation amendments).
Volume II - Technical Volume III - Past Performance Volume IV - Price
Note: The contractor is required to provide one unsanitized copy, and one sanitized copy of the technical proposal. The sanitized copy shall be void of any identifiable references, such as CAGE Code, names (company or individual), etc. Offers submitted without a sanitized and un-sanitized version of the technical proposal will not be evaluated and will not be considered for award.

Volume II: TECHNICAL MERIT INFORMATION. The contractor shall provide written plans identifying how they will meet the technical subfactors identified above: Recruitment; Retention; Training and Immunization Plan;
Compensation Plan, Transitional Plan and Management & Quality Control Plan to provide continuity of services on day one of the contract. Describe how you plan to recruit and retain qualified personnel to fill these positions. The plan will include the offeror’s compensation plan for its health care providers; and shall specifically provide, by
CLIN, a breakout of the health care provider’s annual salary rate, hourly salary rate, fringe benefits and other benefits. The written Recruitment and Retention Plan will outline how the vendor will recruit and retain qualified healthcare providers. It shall also describe the contractor plans and procedures to track training and immunizations which are in place to ensure providers maintain required certifications. The Proposal will also define how and what the Quality Control plan, and how they plan to Transition employees and provide service on day one of the contract.
The management plan will explain how the contractor plans on managing the project.
PART III: PAST PERFORMANCE. Offeror is required to submit information, on a single page, identifying federal, state and local government and private contracts performed within the past three years, which are similar in scope, type of work, and complexity to the effort described in this solicitation. If none, offeror should so state and provide information on references.
PART IV: PRICE. The offeror shall submit the following: Base year and all option year prices, explanation or breakdown of unit prices (i.e., G&A, profit, overhead, fully burdened rates).
If the prices are the same for each contract period, provide one schedule and state that the schedule applies to all contract periods. The information provided in this schedule will be used to assist in the price analysis described in EVALUATION OF THE PROPOSED PRICES, in ADDENDUM TO FAR 52.212-2, EVALUATION – COMMERCIAL ITEMS. Offerors must complete and return the Schedule B and Attachment # X, Fully Burdened Billable Rates must be submitted by all offerors as part of the Price Proposal submission.
All offerors will submit the following:
1) Price Schedule- for Base and four option years. Round figures to reflect two digits only- for the pricing schedule (unit and extended prices).
2) Provide excel spread sheet showing pricing for base and outyears which shall reflect schedule B pricing.
Please provide and show the total cost for the base and all option years.
3) Schedule pricing shall incorporate and include the Quantity (number of hours); X Unit Price (hours) and total amount for that Line Item.
Para (g) Contract award. This is amended to provide that the Government may conduct discussions prior to contract award at its sole discretion.
Para (j) Unique entity identifier (Data Universal Numbering System (DUNS) / CAGE Code). The offeror shall obtain these said Codes and registration in the System Award Management (SAM) and the Online Representations and Certifications. If you are a US firm the codes are mandatory. Award cannot and shall not be made to an offeror without these codes.
Para (m) Proposal Preparation Costs. The Government will not pay any costs incurred in the submission of an offer, in making any necessary studies for the preparation thereof, or for any visit(s) the Contracting Officer may request for the purpose of clarification of the offer.
Para (n) The offeror is responsible for ensuring that its proposal is complete. The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of any stated claims.
The proposal shall not simply rephrase or restate the Government's requirements, but rather shall provide convincing rationale to address how the offeror intends to perform the requirement. Offerors shall assume that the government has no prior knowledge of their experience and will base its evaluation solely on the information presented in the offeror's proposal.
Para (o) Protests received at a level higher than the contracting officer will be referred to the contracting officer.
Protestor may request a review by the contracting officer or an independent review at a level higher than the contracting officer. Requests for appeals or independent review will be addressed to the contracting officer for referral to the review authority. Questions on this process will be referred to the cognizant contracting officer identified in the solicitation.

52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (JUN 2020)
(a) North American Industry Classification System (NAICS) code and small business size standard. The NAICS code(s) and small business size standard(s) for this acquisition appear elsewhere in the solicitation. However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees.
(b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show--
(1) The solicitation number;
(2) The time specified in the solicitation for receipt of offers;
(3) The name, address, and telephone number of the offeror;
(4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary;
(5) Terms of any express warranty;
(6) Price and any discount terms;
(7) "Remit to" address, if different than mailing address;
(8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically);
(9) Acknowledgment of Solicitation Amendments;
(10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and
(11) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.
(d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender's request and expense, unless they are destroyed during preaward testing.
(e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions, including alternative line items (provided that the alternative line items are consistent with subpart 4.10 of the Federal Acquisition Regulation), or alternative commercial items for satisfying the requirements of this solicitation.
Each offer submitted will be evaluated separately.
(f) Late submissions, modifications, revisions, and withdrawals of offers:
(1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due.
(2)(i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is “late” and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and--
(A) If it was transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers; or
(B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government's control prior to the time set for receipt of offers; or
(C) If this solicitation is a request for proposals, it was the only proposal received.
(ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted.
(3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel.
(4) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume.
(5) Offers may be withdrawn by written notice received at any time before the exact time set for receipt of offers.
Oral offers in response to oral solicitations may be withdrawn orally. If the solicitation authorizes facsimile offers, offers may be withdrawn via facsimile received at any time before the exact time set for receipt of offers, subject to the conditions specified in the solicitation concerning facsimile offers. An offer may be withdrawn in person by an offeror or its authorized representative if, before the exact time set for receipt of offers, the identity of the person requesting withdrawal is established and the person signs a receipt for the offer.
(g) Contract award (not applicable to Invitation for Bids). The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror's initial offer should contain the offeror's best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received.
(h) Multiple awards. The Government may accept any item or group of items of an offer, unless the offeror qualifies the offer by specific limitations. Unless otherwise provided in the Schedule, offers may not be submitted for quantities less than those specified. The Government reserves the right to make an award on any item for a quantity less than the quantity offered, at the unit prices offered, unless the offeror specifies otherwise in the offer.
(i) Availability of requirements documents cited in the solicitation. (1)(i) The GSA Index of Federal Specifications, Standards and Commercial Item Descriptions, FPMR Part 101-29, and copies of specifications, standards, and commercial item descriptions cited in this solicitation may be obtained for a fee by submitting a request to--GSA Federal Supply Service Specifications Section, Suite 8100, 470 East L'Enfant Plaza, SW, Washington, DC 20407, Telephone (202) 619-8925, Facsimile (202) 619-8978.

(j) Unique entity identifier. (Applies to all offers that exceed the micro-purchase threshold, and offers at or below the micro-purchase threshold if the solicitation requires the Contractor to be registered in the System for Award Management (SAM).) The Offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation "Unique Entity Identifier" followed by the unique entity identifier that identifies the Offeror's name and address. The Offeror also shall enter its Electronic Funds Transfer (EFT) indicator, if applicable. The EFT indicator is a four-character suffix to the unique entity identifier. The suffix is assigned at the discretion of the Offeror to establish additional SAM records for identifying alternative EFT accounts (see FAR subpart 32.11) for the same entity. If the Offeror does not have a unique entity identifier, it should contact the entity designated at www.sam.gov for unique entity identifier establishment directly to obtain one. The Offeror should indicate that it is an offeror for a Government contract when contacting the entity designated at www.sam.gov for establishing the unique entity identifier.

(l) Debriefing. If a post-award debriefing is given to requesting offerors, the Government shall disclose the following information, if applicable:
(1) The agency's evaluation of the significant weak or deficient factors in the debriefed offeror's offer.
(2) The overall evaluated cost or price and technical rating of the successful and the debriefed offeror and past performance information on the debriefed offeror.
(3) The overall ranking of all offerors, when any ranking was developed by the agency during source selection.
(4) A summary of the rationale for award;
(5) For acquisitions of commercial items, the make and model of the item to be delivered by the successful offeror.
(6) Reasonable responses to relevant questions posed by the debriefed offeror as to whether source-selection procedures set forth in the solicitation, applicable regulations, and other applicable authorities were followed by the agency.

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

Award will be made using best value Lowest Price Technically Acceptable (LPTA) procedures. Award will be made to the offeror provided its technical proposal conforms to the requirements of the solicitation; it has an acceptable record of past performance; its price is determined to fair and reasonable by the contracting officer; and the contracting officer determines the offeror is responsible.
(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
