About this file

This document is an amendment to a solicitation for facility operations and maintenance services. The amendment adds wage determinations for the Service Contract Act and Davis Bacon Act, modifies instructions for proposal submission, and provides responses to contractor questions to clarify requirements. Services required under the contract include preventative and demand maintenance, and minor construction, for US Army facilities in Doral and Homestead, Florida. The amendment sets the response date for proposals as of May 13, 2019, and provides additional details on cost proposal formatting and past performance submission requirements. It also addresses questions on staffing, equipment lists, and maintenance responsibilities to refine offerors' understanding of the performance work statement.

SUMMARY OF CHANGES

SECTION C - DESCRIPTIONS AND SPECIFICATIONS

The following have been modified:

GOVERNMENT RESPONSES

Note 1: There is a requirement for the contractor to lease a facility/warehouse to maintain overstock materials, supplies, and equipment. The pricing for this lease shall be added to CLINs 0002, 1001, 2001, 3001, 4001 (General Work Management & Support).

Contractor Question 1: Key Point 4 indicates the contractor needs to provide proof of an approved accounting system. As an 8(a) small business, the Cost Accounting Standard 8-103.2 CAS Exemptions include “Contracts and subcontracts with small businesses” are exempt. Approved cost accounting systems typically go hand-in-hand with CAS. Given this is an 8(a) set-aside, would the government remove this requirement? (Solicitation Page:

116/Reference: Section L: 3.c. (i)(4))

Government Response 1: In accordance with FAR Part 30 (see 48 CFR 9903.201-1(b) (FAR Appendix) for these and other exemptions) Cost Accounting Standards Administration this Key Point 4 will be deleted.

Contractor Question 2a: (ii) VOLUME II – Cost/Price states “submit prices for each SubCLIN. Does this mean you require a Summary Roll-Up and Direct Labor Table for each SubCLIN? (Solicitation Page: 116/Reference:

Volume II (ii))

Government Response 2a: In reference to (ii) Volume II. No, the Government does not require a Summary Roll- Up and Direct Labor Table for each SubCLIN.

Contractor Question 2b: (ii) VOLUME II – Cost/Price states “submit prices for each SubCLIN. Under (4) b) 3rd para - you state for each CLIN provide a Summary Roll-Up and Direct Labor Table. Therefore, is it correct you want the Roll-Up and Table for both the CLIN and SubCLIN level? (Solicitation Page: 116/Reference: Volume II (ii))

Government Response 2b: No. As stated in the paragraph prior to the heading ‘SUMMARY ROLL UP”, “For each of the cost CLINs, the offeror shall submit an escalated cost summary roll-up and a direct labor table as shown below in Microsoft Excel format.”

Contractor Question 2c: (ii) VOLUME II – Cost/Price states “submit prices for each SubCLIN. Under Notes: (1) -you require cost elements by WBS. Do you want the cost elements (i.e. Direct Labor Table and Summary Roll-Up) by WBS, CLIN, and SubCLIN? (Solicitation Page: 116 117-119/Reference: Volume II (ii))

Government Response 2c: No. As stated in the paragraph prior to the heading ‘SUMMARY ROLL UP”, “For each of the cost CLINs, the offeror shall submit an escalated cost summary roll-up and a direct labor table as shown below in Microsoft Excel format.” Direct Labor Table requests information be provided by WBS. See pages 117- 119 for examples.

Contractor Question 2d: (ii) VOLUME II – Cost/Price states “submit prices for each SubCLIN. Do you want the same Cost Substantiation information for the Option Years as well? (Solicitation Page: 116/Reference: Volume II (ii))

Government Response 2d: Yes. Section B shall be filled out in its entirety.

Contractor Question 3: States if an employee is cross-utilized we can use the example term “see paragraph 4.0”.

Are you saying to put this phrase into the Direct Labor Table or where? (Solicitation Page 119/Reference: Notes (1))

Government Response 3: No. Use the example provided in Note (1) “For example, if an employee performs duties under WBS/PWS paragraphs 4.0, 4.5, and 5.5, and you only listed a cost/price under paragraph 4.0, insert “see paragraph 4.0” in the pricing section for paragraphs 4.5 and 5.5. Cross-references should be utilized to preclude unnecessary duplication of data between sections.”

Contractor Question 4: States CLINs 0005 and 0006 are for informational purposes and are not to be filled in.

Does that apply to the SubCLINs 0005AA-0005AD and 0006AA-0006AB as well? (Solicitation Schedule B)

Government Response 4: The CLINs are information, but the SubCLINs are priced and must be filled in.

Contractor Question 5: In Section L 3. Cost/Price Factor (e) - you state in determining probable cost in the out-years the wage determination rates are escalated 3%. Are we to price the 3% Wage Determination escalation in our pricing in the out-years? (Solicitation Page 128/Reference: Evaluation (e))

Government Response 5: Yes, the offeror shall price a 3% escalation in pricing in the out-years.

Contractor Question 6: Are you requiring any other cost detail beyond the Summary Roll-Up, Direct Labor Table, and the Cost Rationale? (Solicitation Page 116-120/Reference: Cost/Price)

Government Response 6: No. The Government is not requiring any further cost detail other than what has been requested in Volume II.

Contractor Question 7: In the Cost Rationale section you list rates required, can we add to that list? (Solicitation Page 119/Reference: Notes (5))

Government Response 7: No further rates are to be added in the cost rationale section. Failure to follow the above cost proposal preparation instructions may cause your proposal to be deemed unacceptable by the Government.

Contractor Question 8: Can we assume the Direct Labor Table and Summary Roll-Up are to be provided in Excel?

Which version of Excel can you accept? (Solicitation Page 118-119/Reference: Volume II Cost/Price)

Government Response 8: Yes, the Direct Labor Table and Summary Roll-Up are to be provided in Excel. There was no version specified in the solicitation, as such any version will be acceptable.

Contractor Question 9: Are we to assume CLIN 0004 in Schedule B is for 11 months also. (Solicitation Reference:

Schedule B)

Government Response 9: CLIN 0004 is an informational CLIN.

Contractor Question 10: Should this be Volume IV versus Volume V? (Solicitation Page 112 122/Reference:

Section L: (v))

Government Response 10: The SOLICITATIONS, OFFER AND AWARD DOCUMENTS AND CERTIFICATIONS/REPRESENTATIONS Volume will be renamed IV versus V.

Contractor Question 11: How many past performance references are permitted? (Solicitation Page 120/Reference:

Section L: (iii) Volume III)

Government Response 11: The Government has not put a limit on the number of past performance references.

Contractor Question 12: Indicates Attachment 1 is a Sample Consent Letter. Solicitation shows Attachment 1 is the PRS. Would the government provide a sample Consent Letter? (Solicitation Page 121/Reference: Section L:

Factor II 4.)

Government Response 12: Attachment 1 has been added to the amendment.

Contractor Question 13: Indicates Attachment 2 is related to Specific Relevant Contracts Format. Attachment 2 is the Deliverables Schedule. Would the government provide this Attachment? (Solicitation Page 122/Reference:

Section L: 4. Format)

Government Response 13: Attachment 2 has been deleted.

Contractor Question 14: In addition to SCA and DB, is there a Collective Bargaining Agreement? (Solicitation Page 117/Reference: Volume II (b))

Government Response 14: No, there is not a Collective Bargaining Agreement.

Contractor Question 15: The Volume numbers on page 114 do not match the Volume numbers for the details.

Which Volume numbers should we use? (Solicitation Page 114/Reference: Section L)

Government Response 15: The volumes will be corrected to mirror those on page 114 of the solicitation.

Contractor Question 16: How should the contractor estimate overtime? Is there any historical data for overtime?

(Solicitation Page 118/Reference: Notes (1))

Government (Requiring Activity) Response 16: Most overtime requirements would be associated with Demand Maintenance or Project Orders and would not be part of your firm fixed price proposal. Generally, preventative maintenance work that involves a disruption of utilities (electrical power or air conditioning predominantly) would need to be performed afterhours or on weekends or holidays, and may involve overtime.

Contractor Question 17: How would the government like us to price the Phase-In? Should we create our own Excel file and submit with the pricing? (Solicitation: Schedule B)

Government Response 17: The contractor shall use Attachment A3-4 as a reference and place pricing for the Phase-In in CLIN 0001.

Contractor Question 18: May the Site Supervisor act as the Alternate Project Manager? (Solicitation: Performance Work Statement paragraph 1.6.10.)

Government (Requiring Activity) Response 18: No, as per the Performance Work Statement Paragraph 1.6.10 “The following personnel are considered key personnel by the government: project manager/alternate project manager, site supervisor. The contractor shall provide a project manager who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the contractor when the manager is absent shall be designated in writing to the KO. The project manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract.”

Contractor Question 19: We are able to identify Leased Facilities in the Attachment but cannot identify Real Property Facilities. We are trying to determine the difference between standard preventive maintenance (CLIN 0004) and Real Property preventive maintenance. Would the government send this information? (Solicitation:

Schedule B and Performance Work Statement Attachment 5 – Facility List)

Government (Requiring Activity) Response 19: The only leased facilities (listed as “Army Leased” on Attachment 5) to be maintained under this contract requirement are Buildings 736 and 741 (and their associated supporting facilities as listed in Attachment 5) on the Homestead/SOCSOUTH Site, all other facilities on the Homestead/SOCSOUTH Site and all facilities on the Doral/SOUTHCOM Site are Army Real Property (listed as “Army Owned” on Attachment 5). The scope of maintenance for the facilities is the same; the reason for the separate CLINs relates to different government funding programs and a requirement for the government to pay for sustainment costs from different lines of accounting.

Contractor Question 20: Hours of operation seem to vary. Solicitation 1.6.4 references 0730-1630 M-F.

Solicitation l. lists normal working hours of 0630-1730. Can this be clarified? (Solicitation Page 80/ Performance Work Statement paragraph 1.6.4.)

Government (Requiring Activity) Response 20: As per paragraph 1.6.4 in Performance Work Statement, contractor is responsible for conducting business between the hours of 7:30 AM to 4:30 PM, Monday through Friday, except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. Routine inspection, maintenance, repair and minor construction work shall be accomplished during normal duty hours from 7:30 AM. to 4:30 PM., Monday through Friday (excluding observed Federal Holidays), or as otherwise specified by the KO

Contractor Question 21: Can Attachment-8 be provided? (Solicitation Performance Work Statement paragraph 5.3.2.2.2.)

Government (Requiring Activity) Response 21: The contractor shall provide and maintain a daily log. The log sheets shall at a minimum include the information identified in Attachment 8. Attachment 8 is provided.

Contractor Question 22: Can Attachment-6 be provided? (Solicitation Performance Work Statement paragraph 5.4.)

Government (Requiring Activity) Response 22: The contractor shall perform preventive maintenance on facilities and equipment listed below. The contractor shall provide, as part of the bid price, preventive maintenance parts and materials up to a cost of $500.00 per PM cycle per individual item upon which PM is performed. Individual items are listed as separate lines in Attachment 6. Attachment 6 is provided.

Contractor Question 23: Are IJO & PWO the same? If yes, please clarify the discrepancy in the minimum and maximum labor/material thresholds. (Solicitation: Schedule B and Performance Work Statement paragraph 1.4.5.)

Government (Requiring Activity) Response 23: An IJO is an Individual Job Order and a PWO is a Project Work Order. Yes, they are one in the same and the minimum of $2,000 and may not exceed $250,000.

Contractor Question 24: Solicitation indicates submission of Section G – Contract Administration Data. It appears Section G does not require any type of signature. Should we just include the single page in our submission?

(Solicitation Page: 123/Reference: Section G)

Government Response 24: The requirement for submission of Section G – Contract Administration Data will be deleted from the solicitation.

Contractor Question 25: If submitting as a Joint Venture, should we include a copy of the Joint Venture in our submission? If yes, into which Volume?

Government Response 25: The Government will add the following statement in Section L, Volume I, Key Point 7 and Section M, Volume I (i), Section 1 – Sub-factor 1 (1): “The contractor provided evidence of a Joint Venture or partnership.”

Contractor Question 26: How is this work currently being performed; by whom; and, at what monthly cost to the government?

Government Response 26: Yes, this work is currently being performed by the incumbent, Integrated Systems Development. The Government cannot provide any further information.

Contractor Question 27: Will an award be made to a company without a Facility Clearance Level and sponsor after award?

Government Response 27: The Government will not make an award to a contractor without a Facility Clearance.

The Government will not sponsor a contractor.

Contractor Question 28: Will the Government consider Major Subcontractor’s Experience and Past Performance as relevant?

Government Response 28: The contractor, not the subcontractor’s experience and past performance will be relevant.

Contractor Question 29: Is restriping of crosswalks and parking lots included in this contract?

Government (Requiring Activity) Response 29: It is not part of the Firm Fixed Price (FFP) for normal preventative maintenance, however, it may be requested as an Individual Job Order (IJO) or Project Work Order (PWO) at an additional cost.

Contractor Question 30: What is the contractor’s obligation for maintaining and servicing advanced metering?

Government (Requiring Activity) Response 30: All utilities meters on site are maintained by our utilities providers; this requirement does not included any meter maintenance or repair.

Contractor Question 31: What is the contractor’s responsibility of repairs exceeding 32 hours and $2,000.00?

Government (Requiring Activity) Response 31: The $2,000 / 32-hour limitation does not apply to “maintenance” and “repair” work, it applies to “construction” (construction of a complete and usable new facility or alteration/improvement to an existing facility). Any “construction” work that exceeds $2,000 or 32-hours cannot be performed as a Demand Maintenance Order (DMO), and must be performed as an Individual Job / Project Order (IJO/PWO) in order to comply with the Davis-Bacon Act. The government will always identify work requested via DMO as “construction” or “maintenance and repair”. If the work is construction and it will exceed $2,000 or 32-hours, it must be completed as an IJO/PWO. “Maintenance and repair work” that is outside the scope of Preventative Maintenance will be performed as a DMO or an IJO/PWO depending on urgency of need as determined by the government. For example, if a critical utility component fails the repair will most likely be performed as a DMO under an emergency priority, however, repainting non-critical road and parking lot markings will most likely be performed as an IJO/PWO.

Contractor Question 32: Does the Government take responsibility for repairs that exceed 32 hours and $2,000.00?

Government (Requiring Activity) Response 32: Maintenance and repairs that are outside the scope of preventative maintenance, will be performed as either a DMO or an IJO/PWO depending on urgency of need as determined by the Government. The $2,000 / 32-hour limitation relates to “construction” that must be performed as an IJO/PWO in order to comply with the Davis-Bacon Act; refer to the Government Answer to Question 31 above.

Contractor Question 33: Will the O&M contractor be provided office space on site?

Government (Requiring Activity) Response 33: Yes, there is a limited amount of administrative and storage space provided in the Bldg 3 Central Air Conditioning Plant at the Doral/SOUTHCOM site (1,600 SqFt) and Bldg 741 at the Homestead/SOCSOUTH site (500 SqFt), however this is not sufficient for storage of equipment and material required to perform. The current contractor performing these requirements leases approximately 5,000 Sq Ft of space off-site in Doral (approximately 1,000 SqFt of administrative space and 4,000 SF of shop and material storage space).

Contractor Question 34: When was the last eddy current test performed on the 4‐1,000 ton chillers?

Government (Requiring Activity) Response 34: The last eddy current test performed on the 4-1,000 ton chillers was conducted February 2019 and is done once per year.

Contractor Question 35: When was the last De‐Energized Electrical Testing Performed?

Government (Requiring Activity) Response 35: We do not De-Energize the electrical systems. Annual IR Scans are performed instead. The last test was performed in July 2018.

Contractor Question 36: When was the last flow obstruction test performed on the fire suppression system?

Government (Requiring Activity) Response 36: The last flow obstruction test was conducted on December 18, 2018 and the next test is planned for June 2019.

Contractor Question 37: What is the contractor’s responsibility for AV equipment?

Government (Requiring Activity) Response 37: None, it is not part of this contract requirement, Audio-Visual, Information and Communications systems are maintained by a separate entity and their contractors on site; this requirement only maintains electrical and air conditioning equipment that are considered part of the buildings and support these systems.

Contractor Question 38: What is the contractor’s responsibility for CCTV and other electronic surveillance equipment?

Government (Requiring Activity) Response 38: None, maintenance of electronic security systems (access control devices, surveillance cameras and close circuit television systems, etc) it is not part of this contract requirement, these systems are maintained by a separate entity and their contractors on site; this requirement only maintains electrical and air conditioning equipment that are considered part of the buildings and support these systems.

Contractor Question 39: Since all restroom fixtures are motion actuated, would the government provide a battery count and type?

Government (Requiring Activity) Response 39: The current contractor replaces approximately 200 C-Type Batteries in Flushometers (Water Closets and Urinals) and 400 AA-Type batteries (Sinks) monthly.

Contractor Question 40: Will the government provide an Equipment Inventory List by building?

Government (Requiring Activity) Response 40: See Attachment 6.

Contractor Question 41: How many elevators are in this PWS; their location; Traction or Hydraulic?

Government (Requiring Activity) Response 41: All elevators are eight (8) Hydraulic Elevaotrs at the Doral/SOUTHCOM site, five (5) in Bldg 1, one (1) in Bldg 2 and two (2) in Bldg 3. There are four (4) Traction Elevators at the Homestead/SOCSOUTH site, all are located in Bldg 600. The elevators are included in Attachment 6.

Contractor Question 42: Will the government provide a list of the security cleared subcontractors currently serving the two centers?

Government Response 42: No. The Government will not provide a list of the security cleared subcontractors currently serving the two (2) locations.

Contractor Question 43: Does the O&M contractor have any responsibility for the roof anchoring system?

Government (Requiring Activity) Response 43: There are no requirements for preventative maintenance of roof anchoring systems, however if maintenance, repair or alteration of these systems were determined as necessary, the work would be requested by the Government as either a Demand Maintenance Order (DMO) or as an Individual Job / Project Order (IJO/PWO) depending on the type of work required and the urgency of need as determined by the Government.

Contractor Question 44: CLIN X005AB DMO – Materials; does the Cap. Of $808,800.00 include subcontractor costs for i.e.: Fire Alarm and Fire Suppression; Annual Infrared Imaging of Electrical Distribution; Physical Plant Water Systems, UPS and Battery Maintenance…………………………All subcontracted costs for PM and one‐time services?

Government (Requiring Activity) Response 44: The Cap. Of $808,800.00 is an estimated level of effort derived from historical data. The estimate does include subcontractor costs. These numbers can be adjusted by the government from year to year as facilities age and require more maintenance effort, or due to the impacts of inflation and/or changes in Service Contract Act and/or Davis Bacon Act wage rates.

Notes:

(1) The offeror shall provide cost elements by WBS associated with the appropriate PWS paragraph. Provide cost data to the fourth level of the WBS/PWS, except for general information and work classified and management, for which data provided to the third level is adequate. If multiple WBS/PWS paragraphs are being performed by one cross-utilized employee and the employee is listed under only one of the paragraphs, ensure that paragraphs for which cost is not included is cross referenced to the paragraph containing the cost/price. For example, if an employee performs duties under WBS/PWS paragraphs 4.0, 4.5, and 5.5, and you only listed a cost/price under paragraph 4.0, insert “see paragraph 4.0” in the pricing section for paragraphs 4.5 and

5.5. Cross-references should be utilized to preclude unnecessary duplication of data between sections.

(2) The offeror shall identify each position as exempt or non-exempt. If the position is non-exempt, the offeror shall clearly identify the specific level and title of the labor category.

(3) In the cost/price rationale section of the cost proposal, the offeror shall explain how productive hours were developed. For example, paid hours of 2087 minus 80 hours sick leave, 20 hours training, etc.

(4) In the cost/price rationale section of the cost/price proposal, the offeror shall explain how fringe benefits were developed. For example, if costs for non-exempt positions are based on rates identified in the applicable wage determination simply state this with the applicable rate. For exempt positions explain the method of cost buildup, including any specific rates and bases used. Provide rationale used in developing any applicable rates.

(5) In the cost rationale section of the cost/price proposal, the offeror shall provide the following rates used:

_______% Rate FICA

_______% Rate FUTA

_______% Rate SUTA

_______% Workers Compensation

_______% Personal Liability Insurance

Failure to follow the above cost proposal preparation instructions may cause your proposal to be deemed unacceptable by the Government.

The Government reserves the right to require the submission of Other Than Certified Cost or Pricing Data as is deemed necessary to arrive at a determination of fair and reasonable price.

(12) For proposal preparation purposes, the expected or approximate date for initiation of contract performance is 1 July 2019.

(iii) VOLUME III – Past Performance. The Past Performance evaluation will be accomplished by reviewing aspects of an Offeror's recent and relevant Past Performance, focusing on and targeting performance which is relevant to the effort as it directly relates to the services being procured under this solicitation and as defined in Section M (Basis for Award).

Past Performance information described herein is required on the offeror and all subcontractors, teaming partners, and/or joint venture partners proposed to perform 50% or more of the proposed effort based on the total proposed price. The offeror shall submit, along with the information required in this paragraph, a consent letter, executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing release of adverse past performance information to the offeror so the offeror can respond to such information. For each identified effort for a commercial customer, the offeror shall also submit a client authorization letter, authorizing release to the Government of requested information on the offeror's performance.

1. The offeror shall include documentation regarding their relevant past performance as it directly relates to the work being procured under this solicitation. The offeror SHALL NOT go back any farther than three (3) years for the submitted data. The past performance data shall document a successful history of past contract performance.

2. In conducting the Past Performance evaluation, the Government reserves the right to use both the information provided in the offeror’s Past Performance Volume and information obtained from other sources, such as the Past Performance Information Retrieval System (PPIRS) or similar systems, Defense Contract Management Agency (DCMA) and commercial sources. Offerors are reminded that both independent data and data provided by offerors in their proposals may be used by the Government to evaluate offeror past performance. However, the burden of providing thorough, complete, and current past performance information as requested in this paragraph remains with the offerors. Proposals that do not contain the information requested by this paragraph risk rejection or a less than acceptable performance rating by the Government. All past performance comments received will be taken into account and could affect the overall rating. The overall past performance evaluation is a subjective decision based on the whole of all data received. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.”

3. Offerors shall construct and present their proposal/responses in the format listed below. Offerors shall address each element by providing sufficient data/information to enable to Government to assign an acceptable rating to each element.

FACTOR II – PAST PERFORMANCE – The past performance evaluation is an assessment of the offeror’s probability of meeting the minimum past performance solicitation requirements. This assessment is based on the offeror’s record of relevant and recent past performance information that pertain to the products and/or services outlined in the solicitation requirements.

Past performance will be initially evaluated to determine whether the offeror’s present/past performance is recent, and relevant or not relevant to the effort to be acquired. The solicitation shall establish the criteria for recency and relevancy in relation to the requirement being procured. In establishing what is relevant for the acquisition, consideration should be given to what aspects of an offeror’s contract performance history would give the most confidence that the offeror will satisfy the requirements of the contract.

4. Submission Requirements. The offeror shall submit a Past Performance Volume containing the following:

Table of Contents

Summary Page describing the role of the offeror and each subcontractor, teaming partner, and /or joint venture partner that the offeror is required to provide Past Performance Specific Relevant Contract Reference Sheets for the past performance experience in accordance with Attachment 1.

Consent Letters executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing the release of past performance information so the offeror can respond to such information. A sample consent letter is attached as Attachment 1).

Client Authorization Letters for each identified effort for a commercial customer authorizing release to the Government of requested information on the offeror’s performance.

Organization Structure Change History— Organizational Structure Change History. Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant present or past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of your company. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider present and past performance information provided by other sources as well as that provided by the offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.

Specific Relevant Contracts Format - Sheets in accordance with Attachment 2 and are limited to 3 pages per reference. The offeror shall provide documentation outlining the offeror’s past performance with contracts, as a prime or major subcontractor, which is the same or similar in nature, size, and complexity to the services being procured under this Solicitation. The submittal shall include rationale supporting your assertion of relevance and how it was determined that the work performed previously was the same or similar in nature, size, and complexity to the work specified by this solicitation. Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort, and to what aspects of the proposed effort they relate.

Past Performance Questionnaire - See Attachment 3, Past Performance Questionnaire.

For the contracts identified on each Specific Relevant Contract Reference, the offeror shall forward a copy of the Past Performance Questionnaire to the points of contact responsible for monitoring performance under such contracts. The points of contact shall return the questionnaires directly to dyan.e.jackson.civ@mail.mil and catherine.p.richardson.civ@mail.mil not later than 13 May 2019. Any questionnaires not returned directly to the Contract Specialist/Contracting Officer will not be reviewed and evaluated. The information contained in the questionnaires will be used to evaluate the offeror’s past performance. New entities that have no past customers shall annotate on the Summary Page that they have had no previous clients and that the minimum number of questionnaires cannot be provided.
