REQUEST FOR ACCESS TO LIMITED DISTRIBUTION MATERIALS
Please fill out all information as completely and accurately as possible. The Government will use the information provided to assess eligibility for receipt of limited distribution materials. In some cases, the Government may find sufficient evidence of potential benefit to the Government to grant access to limited distribution materials even when the eligibility determination is negative.
The Government uses all available information to make its determinations, including information from this request and information retrieved from open sources and Government access restricted databases.
All distribution exceptions incur risk to the Soldier, the U.S. Government, or U.S Government interests.
Because of this, the Government is not obligated to grant any such exceptions.

2) If you answered yes to question 1, please provide the following information for at least one contract in the following priority order: DoD, Department of Homeland Security, any other U.S. Government Agency.

3) If you are not currently under contract with a U.S. Government Agency, but your company has been under contract with a U.S. Government Agency in the past, please provide the following information for that past contract in the following priority order: DoD, Department of Homeland Security, any other U.S. Government Agency.
