EXHIBIT E – PROTOTYPICAL PROJECT

NOTICE OF PROPOSED PROJECT, SOLICITATION

Date: October 13, 2015

PROJECT TITLE AND NO.: REPAIR HANGAR AND SHOPS, BLDG 685

PROJECT DESCRIPTION: The project consists of remodeling interior areas of the building, replacing exterior windows, refurbishing hangar door and adding fire suppression and detection system. Contractor shall provide all plant, labor, material, transportation, tools, equipment, appliances, supplies, services, permits, insurance and supervision necessary to Repair Hangar and Shops, Bldg. 685 IAW the Statement of Work, Specification and drawings. All work shall be in strict accordance with the Plans/drawings and Specifications dated (Repair Hangar & Shops, Bldg. 685, March 5, 2010 – Revised)

Note General Requirements are found in solicitation and resulting contract indicated as Division 1 in Section 00800

MAGNITUDE OF PROJECT: $1,000,000 and $5,000,000

PERFORMANCE PERIOD: 300 Days, after receipt of Notice to Proceed

SITE VISIT: A site visit will be conducted in conjunction with the MATOC pre-proposal meeting.

PERFORMANCE & PAYMENT BONDS: In accordance with FAR 28.102-1 and Contract Clause 52.228-15, Contractors are reminded that any amount awarded over $30,000 shall require Payment Bonds, and awards exceeding $150,000 shall require both Payment and Performance Bonds. All bonds are due no later than 10 days after award.

QUESTIONS AND ANSWERS: All questions and answers must be in writing. Email questions to colleen.m.moore.civ@mail.mil AND colleen.m.dalquist.civ@mail.mil DO NOT contact engineers or AE as this will only delay receipt of answers. Questions must be received NLT three workdays prior to closing. Answers will be provided with questions, on a non-attribution basis to all eligible MATOC contractors

PROJECT SAFETY: In accordance with FAR 52.236-13, Accident Prevention, construction contractors are required to use the U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1. To view this manual click here http://www.usace.army.mil/CESO/Pages/EM385-1-1.aspx

WAGE DETERMINATION: Currently, General Decision Number MN150036 Modification No. 05 dated July 17, 2015 for Hennepin County is applicable for pricing of this proto-typical task order. Rates can be found at http://www.wdol.gov/wdol/scafiles/davisbacon/MN.html

PROPOSAL DUE DATE: All offers are due with proposal for MATOC basic contract. Failure to provide a price will make offer noncompliant, eliminated from the competition and Offeror ineligible for a basic contract award. Basis for award is in the RFP.

The Following Provisions/Clauses if marked [X] are applicable to this task order

☒ FAR 52.246-21 Warranty of Construction, is applicable to this task order.

☐ Performance of Services during Crisis Declared by the National Command Authority. (September 2002) This special contract clause IS NOT APPLICABLE TO THIS TASK ORDER.

☐ FAR 52.217-5 Evaluation of Options applies to this solicitation

☐ FAR 52.211-12 Liquidated Damages. If applicable, the following amounts will be applicable to this TASK ORDER [Not Applicable – Proto-typical Project not being awarded]

☐ FAR 52.236-14 Availability and Use of Utility Services The Contracting Officer has determined that Government-operated utilities are adequate and will be furnished to the contractor without charge where existing outlets are available. The contractor is responsible for installing temporary service outlets, as necessary, at its expense IAW the clause entitled “Availability and Use of Utility Services” (FAR Clause 52.236-14). IF TASK ORDER IS OVER $25,000 FAR 52.204-10 Reporting Executive Compensation and First Tier Subcontract Awards (Aug 2012) is applicable to this order. The following are the codes needed to accomplish the reporting:

☐ DFARS 252.236-7006 Cost Limitation (JAN 1997)

(a) Certain items in this solicitation are subject to statutory cost limitations. The limitations are stated in the Schedule.

(b) An offer which does not state separate prices for the items identified in the Schedule as subject to a cost limitation may be considered nonresponsive.

(c) Prices stated in offers for items subject to cost limitations shall include an appropriate apportionment of all costs, direct and indirect, overhead, and profit.

(d) Offers may be rejected which—(1) Are materially unbalanced for the purpose of bringing items within cost limitations; or(2) Exceed the cost limitations, unless the limitations have been waived by the Government prior to award.

☐ FAR 52.217-7 Option for Increased Quantity- Separately Priced Line Item applies to award when option(s) not taken at time of award. The Government reserves the right to award option individual line items at time of basic line item award or after award, but not later than [N/A – Proto-typical project not being awarded] days after initial award. Performance period upon award of options will not be extended.

☒ AT Level I Training. This provision/contract text is for contractor employees with an area of performance within an Army National Guard controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access to Army/Air National Guard installations, facilities and controlled access shall complete AT Level I awareness training. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the Contracting Officer’s Representative (COR).
