EXHIBIT D – TECHNICAL PROPOSAL DATA REQUIREMENTS

INSTRUCTIONS

The Technical proposal for this solicitation consists of information provided in response to the questions provided in this exhibit. Offerors must answer all questions in each sub-factor. Offerors are strongly cautioned to read sections 00800 and Division One of the solicitation documents as well as the evaluation information found in Section 00120 before completing Exhibit D as your answers must meet the minimum requirements found in these sections. Failure to address a sub-factor will make the sub-factor deficient and may result in an overall unacceptable rating.

The technical proposal is limited to 20 single pages/10 double pages exclusive of tabs, indexes, dividers, and attachments listed in this exhibit. Any additional attachments you provide, to include Safety or Quality Control Plans, will be included in the page count provided. It is highly advisable to state the question then give the answer. This will aid in the evaluation. Offerors who preceded their answer to the each question with a reiteration of the question as it appears in this exhibit may add one (1) additional page to their total page count (21 pages). Pages in excess of the limits WILL NOT be considered in the evaluation.

Each answer should be covered in sufficient detail to clearly address all required information. Answer questions in order presented. In the event you reference another document or part of the proposal you must be clear as to where the information can be found to answer the particular question(s). Such statements as see Company profile or see Safety Plan are not acceptable. You must indicate on what page of the proposal the answer can be found in the referenced document. Failure to do so may make your proposal non-compliant or the particular sub-factor considered deficient.

Offerors will be evaluated for all dollar levels in the range requested in “Areas of Consideration.” Considerable consideration will be given to the highest dollar level you are requesting. Failure to demonstrate experience will adversely affect your technical evaluation. If you have no experience at the higher dollar levels consider requesting a lower range with the option to advance to higher range amounts in the option years.

Request only locations that you will regularly and actively pursue task order offerings in the dollar range you have requested. Lack of experience or an acceptable plan to overcome the lack of experience in a location selected that is also indicated in the solicitation as a key or critical location may adversely affect the evaluation.

Failure to include all information requested may adversely affect the evaluation. A proposal that merely reiterates or promises to accomplish the requirements of the RFP may be considered unacceptable.

A Proposal that is not presented in an orderly format may be considered noncompliant with solicitation instructions not evaluated and removed from consideration for award. A proposal that is orderly and sufficiently documented will be easy for the Government to understand and will enable the Government to perform a thorough and fair evaluation.

TECHNICAL PROPOSAL QUESTIONS

1. CAPABILITY AND EXPERIENCE:

1A: In the format provided in Attachment 1 show experience on multiple disciplined projects. Additionally, discuss your experience and technical capability to perform and manage multi-disciplined projects for the dollar levels and locations you are requesting in “Areas of Consideration”. The discussion should address familiarity with construction in the locations selected, and familiarity with issues pertaining to work in this area and involvement of the local workforce. Discuss your ability to manage multiple projects at one time. Note: An Offeror, who has the capability to perform both as a General contractor and self-perform a major element of construction, for example, Electrical, or Mechanical or Roofing, may be afforded additional consideration in the evaluation

1B: This contract requires short notice site visits/proposal meetings, normally within 2 calendar days, but can be as short as 240 minutes. Consider reporting to the 133rd Airlift Wing, St. Paul, MN or 148th Fighter Wing, Duluth, MN if you chose that location. Give details on how will you ensure compliance with these requirements? Note that response to a task order notice must be made by the prime contractor (Contract holder) or employee of the prime contractor. Response cannot be made by a subcontractor representing the contractor.

1C: Discuss experience with constructing to United States Green Building Council (USGBC) LEED® standards. If no experience so state and indicate how you plan to overcome this weakness should a task order have these requirements? (NA to Offerors requesting $1M or less)

1D: Discuss your understanding of and plan to be in compliance with contract Clause 52.236-6, Superintendence by the Contractor. “At all times during performance of this contract and until the work is completed and accepted, the Contractor shall directly superintend the work or assign and have on the worksite a competent superintendent who is satisfactory to the Contracting Officer and has authority to act for the Contractor.” The use of a subcontractor foreman/superintendent is NOT a satisfactory method of accomplishing this requirement. Narrative must include a positive statement that this is understood and will be complied with.

2. ORGANIZATION:

2A: Provide as an attachment an Organizational Chart listing company personnel and their roles. (Titles) (Attachment 2) (Not required if a sole proprietorship and requesting consideration only to $500,000. Discuss organization in narrative.)

2B: Provide ( 1) names, (2) duties and (3) levels of responsibility of key personnel to include the Project Manager, Site Superintendent(s), Quality Control Manager and Safety Manager. Indicate to whom the Quality Control and Safety Managers report. (Lines of authority)

2C: Limitations On Subcontracting - Explain in detail how you will perform at least 15%, not including the cost of materials, with your own forces on resulting task orders. Provide a list of the in-house trades that you employ. Provide the number (not names) of these employees and if they are full-time, part- time etc.

2D: If applicable, describe the support and interface with your home office or corporate headquarters for such aspects as financial, management and technical support. If not applicable state 2D- Not applicable.

3. SPECIFIC PERSONNEL

3A: Discuss the education, training and experience in managing construction projects of the key personnel listed in 2B. (A resume or experience cut sheet limited to two (2) pages may be submitted if it provides this information. (Attachment # 3) If proposed personnel are currently not on your staff Letter(s) of Intent must be provided.

3B: Provide list of major subcontractors and their trades that you intend to use on these projects. (These also should have been listed in Exhibit A, PART 2 and there must be an Exhibit B in the proposal for the named subcontractors or they will not be considered.)

3C: Tell us if you have worked together in the past. If yes, on what kind of jobs?

4. QUALITY CONTROL- (In addition to the information required in this question, information on personnel, duties, responsibilities and lines of authority provided in response to questions 2 and 3 will be used in the evaluation of Quality Control.)

4A: Discuss how quality issues will be dealt with on various types of construction projects. Include in the discussion as a minimum, meetings, inspections, submittal reviews, correction of non-compliant work and how you intend to ensure non-reoccurrence and reporting of quality problems to Government officials.

(Note: A full Quality Control Plan will be required if you are awarded a contract and is NOT required or desired with the proposal. If you do provide a plan, indicate if it is a draft for this contract or a sample from another project. Also, provide in the answer to this question the page numbers where the required information (list of minimum information above) can be found. If you fail to follow these instructions, your proposal cannot be evaluated in this area.) Full Quality Control Plans if provided WILL NOT be evaluated or searched to find the answers to these requirements. Also, they will be part of the page count and pages in excess of the number authorized will not be considered in the evaluation, even if your answers are on the excess pages.

4B: Discuss your procedures for contract closeout.

5. SCHEDULING METHODOLOGY - Adequate scheduling processes are necessary to ensure completion and control of the project from beginning to the end of the project.

5A: Discuss your scheduling capabilities. Include such things as programs and staff to do scheduling.

5B: Include a schedule for the prototypical project(s). (Attachment # 4) Use a Notice to proceed date of June 1, 2015. Offerors not being considered for task orders over 1 million dollars may provide schedule in any acceptable format, Offerors being evaluated for orders over 1 million dollars should provide the schedule in Critical Path Method (CPM) format to demonstrate capability. (This includes prototypical/seed projects valued at less than 1 million dollars.) If the project is less than 1 million dollars, an Offeror may provide a sample CPM from a previously completed project to demonstrate capability but must also provide a schedule in any format for the prototypical project.

6. SAFETY

6A: Discuss your Safety Program in general, and provide as a minimum, details on training, documentation, and your plan to ensure adherence to OSHA Standards, and the USACOE EM 385-1-1 (Safety Manual).

6B: Discuss the steps you took to promote safety during construction during the past three (3) years.

(Note: A full Safety Plan will be required if you are awarded a contract and is NOT required or desired with the proposal. If you do provide a plan, indicate if it is a draft for this contract, a sample from another project or a corporate plan. Also, provide in the answer to this question the page numbers where the required information (listed above) can be found. If you fail to follow these instructions, your proposal cannot be evaluated in this area.) Full Safety Plans if provided WILL NOT be evaluated or searched to find the answers to these requirements.
