Seed Task Order Scope and Instructions

Commercial Vehicle/Visitor Access Control Facility, Robins Air Force Base, Georgia

1. In accordance with Local Clause 52.216-4008, Multiple Award Fair Opportunity Task and Delivery Order Contracts, subject project is being offered to all Offerors in the MATOC pool, as identified below, giving each fair opportunity to compete for this action by issuance of this Request for Proposal (RFP) letter. Any Offeror who does not wish to be considered for this particular task order is requested to notify this office in writing, within seven (7) calendar days of receipt of this letter, indicating reason for non-participation.

Those who do wish to compete must submit a proposal by the date and time indicated in paragraph 24 below, and in accordance with the criteria specified herein.

2. Scope of Work.

2.1. Brief Description of the Scope of Work: The existing commercial truck gate (Gate 4, Robins AFB, GA) is sub-standard and does not meet the current requirements for ATFP. The contractor shall utilize the design-build request for proposal (RFP) to provide design and construction (complete) to construct a new commercial truck gate to meet AT/FP requirements. The design-build RFP provides an approved layout of the new access and exit roadways, denial barriers, inspection canopies, and driver check-in station, lighting, signage, related supporting features, and infrastructure. Demolition of existing facilities within the footprint of construction is also required.

2.2. Interested participants must submit a price and technical proposal for work detailed in the scope of work, drawings and specifications posted on FedTeDs. To access the full scope of work, specifications and drawings go to the following link:

3. Technical Inquiries:

3.1. Technical Inquiries are to be submitted via Bidder Inquiry in ProjNet at www.projnet.org\projnet, Bidder Inquiry Key: A2YXZX-XJQW7P. All questions regarding this RFP must be submitted in writing via aforementioned method no later than five (5) calendar days prior to the date established for receipt of proposals as shown in paragraph 24 below. The Government reserves the right to not respond to questions/inquiries received after this date.

3.2. To submit and review bid inquiry items, bidders will need to be a current registered user or self-register into system. To self-register go to web page, click BID tab select Bidder Inquiry, select agency USACE, enter Key for this solicitation listed below, and your e-mail address, click login. Fill in all required information and click create user. Verify that information on next screen is correct and click continue.

4. If an Offeror believes the requirements in this RFP contain an error, omission, or are otherwise unsound; the offeror shall immediately notify the Contracting Officer in writing, to include supporting rationale. Such communication may be submitted via the POC identified in paragraph 20 below.

5. Wage Decision GA180152 dated 09/07/2018 GA152 (Attachment 2) applies to CLINS 0001 through 0007 of attached Task Order Pricing Schedule (Attachment 1). Offerors are reminded that wage determinations are subject to change prior to award. All proposed pricing shall include the most current wage rates. All offerors are encouraged to review wage rates established by the Department of Labor prior to submitting proposals.

6. The contractor shall be assessed the amount of $986.18 liquidated damages per calendar day for failure to complete the prescribed work within the performance period.

7. A Construction Cost Limitation (CCL) for MCAF Funded Line Items CLIN NUMBERS 0001, 0002, and 0003 is $8,428,000. This limitation includes both design and construction costs and shall not be exceeded. Offers which exceed the CCL risk being rendered ineligible for award.

8. Site Visits.

8.1. In accordance with FAR Clause 52.237-2, Site Visit (Construction), A site visit will be conducted at the Corps of Engineers Resident Office, at Robins Air Force Base, Georgia on 20-December-2018 beginning at 1330 hours. Participants will be required to check in at the main gate via Watson Blvd, and arrive there by 1230hrs. Clearance forms to be completed before hand by the participants will be provided. The POC for the site visit is Craig Harrell and he can be contacted at phone number 912-652-5150 or e-mail at craig.l.harrell@usace.army.mil.

8.2. Prior to the submission of any proposals, all offerors are required to visit the project site location to become familiar with the project requirements. A proposal will not be rejected only because the offeror fails to visit the project site. Failure to visit the project site will not relieve the bidder from responsibility for performance of the work and compliance with the terms and conditions of any resultant contract. In no event will a failure to inspect the site constitute grounds for a claim after award of the task order.

16. Excavation/Dig Permit: The contractor shall obtain through the Base Civil Engineer, a completed and approved Excavation Permit prior to any excavation, to include sign or fence-post holes. The Contractor shall schedule an appointment to locate utility lines prior to any excavation with the Corps of Engineers. Excavation/Dig Permit meetings are held Monday at 0800 hours in Building 1555.

17. Disposal Permits: No on-base disposal of land clearing debris and inert debris is available. The disposal of metal from building demolition is described in the RFP.

The contractor will be required to coordinate any permit(s) required to access off-base land clearing and inert debris and demolition landfills. Only materials produced on the project for which the permits are issued may be disposed of in the land clearing and inert debris and demolition landfills. The Contractor shall keep a copy of the completed permit with the vehicle throughout the contract disposal operation.

18. Borrow Permits: Borrow material is available at an on-base construction site approximately one mile from this project’s site. Review RFP description of the intended usage of this fill material for this project.

19. Haul Routes: The preponderance of the work is to be accomplished with the project site fenced such that site access is to be directly from the Highway 247/129 adjacent to Robins AFB. Comply with Georgia DOT highway/road construction safety requirements as well as EM 385-1-1. For any on-base haul activity, the Contractor is required to obtain approval from the Resident Office for the routes proposed for transportation of borrow materials, construction debris, or demolition materials unless otherwise permitted in writing by the Resident. The axle load of earth-hauling equipment operating on paved streets shall not exceed 12,000 pounds.

20. Utility Outages and Road/Parkling Lot Closures: Utility, road/parking lot and railroad closures require a minimum 10 working days advance written notice to Resident Office. In the case of a road closures, a sketch shall be provided showing the closure location and all necessary signs and barricades. Necessary signage, barricades, flag persons, lights (including temporary traffic control lights), and markings for the safe movement of the public during construction shall be in accordance with the Manual on Uniform Traffic Control Devices, and shall be provided at no additional expense to the Government. Utility outages shall be scheduled with Base Civil Engineering, through the Resident Office. Utility Outage Meetings are held Wednesday at 1030 hours in Building 1555. Road and parking lot closures shall require a minimum of forty-five (45) days notice to Base Civil Engineering.

21. Availability and Use of Utility Services: Utility services required on the job site for the accomplishment of the work

Will be furnished at no cost to the Contractor Will NOT be furnished at any cost to the Contractor

However, the Government will make no connections or alterations to the existing utility systems for the Contractor. Utilities for offices and/or storage buildings or areas will be billed to the Contractor monthly and will not be furnished free of charge. The Contractor shall be responsible for installing meters or other connections at no cost to the Government. At the conclusion of the contract, the Contractor shall remove all temporary connections, distribution lines, meters and associated paraphernalia unless otherwise directed by the Resident Office. Prior to installing any utility connections at an office/storage site, the plan will be approved by the Resident Office. When utility meters are installed, the Contractor shall notify the Resident Office for the initial meter reading. Failure to obtain this initial reading will result in the Contractor being charged for the entire amount shown on the meter.

22. Contractor Storage and Trailers: The Contractor shall place or paint a sign on all of his storage trailer(s) and building(s) used on this contract. At a minimum, the sign shall contain the name of the Contractor and a telephone number at which the contractor can be reached. The trailer(s) and building(s) shall be completed with gates and/or doors which can be locked. Only material for this project shall be stored in the trailer(s) or building(s). The Contractor shall remove the storage trailer(s) or building(s) within 30 days after completion of the contract and prior to submitting his final invoice. The area around the storage trailer(s) and building(s) shall be kept clean.

23. Safety: Safety will be in compliance with the Corps of Engineers Safety Manual EM 385-1-1. Use of appropriate safety equipment is mandatory and not limited to hard hats and steel-toed shoes. Contractor is responsible for daily clean up and complete restoration of the area once the contract is complete.

24. Hours of Work: Work shall be accomplished between the hours of 0700 thru 1700 hours daily, Monday through Friday on non-Government holidays. Legal holidays falling on Saturday are observed on the proceeding Friday and those falling on Sunday are observed on the following Monday. Work schedule and facility security to be coordinated with the Resident Office for facility access and security maintenance during duration of work. Contractor shall not work outside of the stated hours of work, without first obtaining approval from the Corps of Engineers Resident Office.

25. Warranty: The contractor shall provide a minimum of one (1) year warranty on all materials and workmanship from the date of the Government’s acceptance of the work.

26. Proposals.

27.1. Bid Guarantee: In accordance with FAR Clause 52.228-1, Bid Guarantee, Offerors are required to submit a bid bond with their proposal by the time and date set for receipt of proposals. The Bid Guarantee shall be 20% of the bid price or $3 Million, whichever is less. Bid bonds must be submitted in original form and contain original signatures.

Photocopied, facsimile, scanned or otherwise mechanically reproduced bid bonds will not be accepted. Failure to submit a proper bid bond may be cause for rejection of an Offeror’s proposal.

27.2. Evaluation of Options: In accordance with FAR Clause 52.217-5, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options will not obligate the Government to exercise the option(s).

Proposals sent by U.S. Mail or delivered by commercial carrier must be received by mailroom personnel on the first floor of 100 West Oglethorpe Avenue by the time specified in Block 13 of the SF 1442 for receipt of proposals. The date and time of delivery established by these carriers is not the official time of receipt by the Government. Security Guards posted at the loading dock entry for the building do not receive bids and will contact Mailroom personnel. Once Mailroom personnel are available, the proposal will be processed and the Security Guards using established security procedures and subsequently picked up by Mailroom personnel. After receipt by the Mailroom personnel, a record will be made of the receipt of the proposal.

Completion of these actions will constitute delivery of the bid by the date and time designated for receipt of proposal. Contractors are responsible for allowing sufficient time for the bid to be processed for receipt and are cautioned that sufficient time may be as much as 24 hours prior to the date and time designated for Express Mail, Federal Express and United Parcel Service are required to be sent to 100 West Oglethorpe Avenue, Savannah Georgia 31401

Offerors are cautioned that proposals sent via United States Postal Service Express Mail are first delivered to the Savannah District Post Office Box instead of 100 West Oglethorpe Avenue, “the office designated for receipt of proposals” therefore, allow sufficient mailing time.

28.3. The Government will not be responsible for proposals or bid bonds delivered to any location or to anyone other than those designated to receive proposals on its behalf.

Offerors are responsible for ensuring that proposals are submitted so as to reach the designated recipient of proposals. Offerors are responsible for allowing sufficient time for the proposal and the bid bond to be received in accordance with the information provided.

28.4. Offerors shall acknowledge all revisions to the RFP issued prior to the due date for the submission of proposals in writing via a cover letter accompanying the proposal, unless instructed to do otherwise by the Contracting Officer.

28.5. For all revisions to the RFP issued after the submission of proposals, the offerors shall acknowledge the revision in writing, at the time and place directed by the Contracting Officer in the revision letter.

29. Instructions to Offerors: In accordance with FAR Clause 52.215-1, Competitive Acquisitions, the Government reserves the right to make award without discussions.

Therefore, Offerors should submit their best technical and price terms in their initial offer and not automatically assume that they will have an opportunity to participate in discussions, if later determined to be in the best interests of the Government, or to submit a revised offer. Discussions, if held, may be held with only the highest rated Offerors or Offeror.

PROPOSAL SUBMISSION REQUIREMENTS AND INSTRUCTION

FOR PHASE II

PHASE TWO – The submission requirements relate to the Seed Task Order.

PROPOSAL TABBING REQUIREMENTS. Phase Two proposals shall be organized and tabbed as follows:

VOLUME I – CONTRACT REQUIREMENTS AND PRICE PROPOSAL

TAB A – Standard Form 1442, completed and signed by authorized individual(s) of the Offeror. Offers submitted in the name of a Joint Venture must be signed in accordance with the terms and conditions specified in the joint venture.

TAB B – Proposal Pricing Schedule, Section 00 10 00 - Schedule A Base Contract Bid Schedule & Schedule B, Seed Task Order Pricing.

TAB C – Bid Bond – Solicitation Requirement: The Offeror is required to submit a bid bond validating the Offeror’s bonding capability for the Seed Task Order. Submit originals; photocopied or scanned copies are not acceptable. FAR clause 52.228-1, “Bid Guarantee” will be included in the contract.

TAB D – Subcontracting Plan – FOR LARGE BUSINESS OFFERORS ONLY.

Subcontracting Plan shall be prepared in accordance with FAR 52.219-9, and SBA’s Electronic Subcontracting Reporting System (eSRS) located at http://esrs.gov .

9. PHASE TWO – TAB E – FACTOR 4 - DESIGN TECHNICAL – BUILDING

FUNCTION AND AESTHETICS

9.1 SUBMISSION REQUIREMENTS.

9.1.1 PRESENTATION DRAWINGS

a. Presentation of exterior elevations of the facility clearly noting proposed materials and colors.

b. Schematic floor plans for each facility demonstrating adjacencies, room size requirements, and function.

c. Presentation of interior of the facility clearly noting proposed materials and colors.

d. A color board including primary interior and exterior finish materials.

9.1.2 TECHNICAL APPROACH NARRATIVE

a. Provide technical approach narrative, both qualitative and quantitative, defining the various technical elements of the proposal such as but not limited to: Fire and Life Safety Planning, ATFP Considerations, Mechanical Planning Considerations, Electrical Planning Considerations and other Major Systems Planning. Preface the narratives with a design concepts narrative, providing the design rationale and basis of the proposal.

Whether individual narratives or a single combined narrative is provided, the maximum total length for narratives under this Factor shall not exceed ten (10) single sided pages.

b. Architectural Theme and Materials. Submit a narrative not to exceed two (2) pages of the ten (10) pages allowed for this Factor. Describe the architectural themes of the various facilities and spaces which demonstrate how the proposal achieves the results desired by the Statement of Work. Narrative should address how the selection of materials and colors enhances the exterior and interior aesthetics of the facilities and improves the living and/or working conditions for the populations who will utilize the facilities. This narrative is not intended to be a material listing, but to explain/reflect how the selections were made and how they address the requirements in the Statement of Work.

9.2 EVALUATION CRITERIA:

9.2.1 FACTOR 4 – DESIGN TECHNICAL – BUILDING FUNCTION AND AESTHETICS:

The following elements will be evaluated:

9.2.1.1 Do the facilities provide acceptable life safety and fire safety measures? (i.e.

Does the proposal describe fire suppression systems, mass notification systems and type of fire alarm systems?)

9.2.1.2 Does the proposal describe ATFP Considerations, Mechanical Planning Considerations, Electrical Planning Considerations and other Major Systems Planning as they relate to the function of the building?

9.2.1.3 Building Aesthetics: This element considers the overall “appeal” of the facility and the desire that both the interior and exterior of the facilities present a professional, attractive appearance. The following will be considered under this component:

a. Exterior Considerations:

1) Does the proposal address design of facades, roof lines, and delineation of entrances, proportions of fenestration in relation to elevations, shade and shadow effects, materials, textures, exterior color schemes.

2) Is the building complimentary of the adjacent structures? Is the building an attractive addition to the installation? How well does the building harmonize with its environment, including surrounding facilities? (Does it look like all the other buildings adjacent to it/on the installation?)

3) Do the windows and doors meet the ATFP?

4) Does the exterior color proposed match the IDG?

b. Interior Considerations:

1) Are the proposed colors and material finishes conducive to the working environment of the facility?

2) For administrative areas, does the interior design provided establish a positive working environment free from congestion and

3) Has the proposal addressed/provided for natural and artificial light in the living and working spaces and is the arrangement of fenestration and lighting fixtures in the spaces conducive to furniture placement and space usage?

4) Do the proposed ceiling material, elevation, and design enhance the environment?

5) Does the proposals provide for acoustic control of noise from service/support spaces to administrative areas? (i.e. walls and ceilings between administration areas and maintenance bays.)

9.3 Design Technical – Building Function and Aesthetics will be rated using the following adjectival descriptions.

10 PHASE TWO – TAB F – FACTOR 5 – DESIGN TECHNICAL – QUALITY OF

BUILDING SYSTEMS

10.1 SUBMISSION REQUIREMENTS:

a. Presentation Drawings. There are no specific drawings requirements for this TECHNICAL SECTION. However, the Offeror has the option of providing concept level drawing information for specific materials and/or systems which the Offeror feels are necessary to describe the proposed systems or materials.

b. Technical Approach Narratives. Provide technical approach narrative, both qualitative and quantitative, defining the elements of the proposal. It is acceptable to include all the elements below into a single combined narrative for the entire facility. It is the responsibility of the Offeror to ensure that all aspects identified in the evaluation criteria below are addressed. Whether individual narratives or a single combined narrative is provided, the maximum total length for narratives under this-TECHNICAL SECTION shall not exceed eight (8) typewritten pages

c. Architectural Finishes: Describe how the materials selected provide for a suitable environment for the expected population of the facility. Discuss how these selections provide value to the Government and how they address the minimum requirements.

Narrative should focus on durability and maintenance of the finishes proposed.

d. Mechanical Systems: Describe how the mechanical systems selected provide for a highly efficient environmental control system including information about provisions for indoor air quality maintenance. Discuss how these selections provide value to the Government and how they address the minimum requirements. Narrative should focus on maintenance considerations, limiting energy consumption, and suitability of the proposed systems for the expected usage.

e. Plumbing Systems: Describe how the plumbing systems selected provide for a highly efficient domestic hot water system and an efficient piping system. Discuss how these selections provide value to the Government and how they address the minimum requirements. Narrative should focus on maintenance considerations, energy consumption, and suitability of the proposed systems for the expected usage.

f. Electrical Systems: Describe how the electrical power and lighting systems, telephone, data, and cable television systems selected provide for a highly efficient electrical system. Discuss how these selections provide value to the Government and how they address the minimum requirements. Narrative should focus on maintenance considerations, energy consumption, and suitability of the proposed systems for the expected usage.

g. Site Utilities and Site Systems: Describe how the site utility systems selected provide for an efficient piping system. Discuss how these selections provide value to the Government and how they address the minimum requirements. Narrative should focus on maintenance considerations and suitability of the proposed systems for the expected usage. Include information regarding coordination with privatized utility providers where applicable.

h. Interoperability: Describe how systems integrated into the new facilities which require connection and interface with existing installation wide systems will be accommodated in the proposed project. Narrative should address the following systems as minimum:

Fire Alarm, Telephone, Cable Television, UMCS, and privatized utility companies where applicable.

i. Energy Conservation: Describe the strategy to achieve required energy savings as specified in the RFP.

j. ATFP Considerations: Describe how the proposed materials, systems, and designs address the mandatory building ATFP requirements included in the Statement of Work.

k. Proposed Material Identifications: In order to evaluate and rate the quality of the materials being proposed, including any material or equipment warranties exceeding the one year warranty in the contract clause “Warranty of Construction”, the Offeror shall include in the proposal material identification for major materials in each of the areas shown below:

l. The Government will evaluate and consider materials and equipment proposed by brand name and model number as a quality standard. Unless substitution of a manufacturer, brand name or model is otherwise specifically prohibited in the contract, if the successful Offeror desires to substitute manufacturers, brand names or models after award, the substituted product must meet the contract requirements and be approved by the designer of record and the Government as equal in function, performance, quality and salient features to that initially proposed.

m. Acceptance of the proposal is not a guarantee that the proposed products meet the contractual requirements.

10.2 EVALUATION CRITERIA

10.2.1 FACTOR 5 – DESIGN TECHNICAL APPROACH – QUALITY OF BUILDING

SYSTEMS:

a. It is the Army's objective that these buildings will have a 50 year useful life. The design and construction should provide an appropriate level of quality to ensure the continued use of the facility over that time period with the application of reasonable preventive maintenance and repairs that would be industry-acceptable to a major civilian sector project OWNER. The facility design should consider that the Army may repurpose the use of the facility over the 50 year life. Within that overriding theme the Government will evaluate the Offeror selected systems and components proposed in terms of warranties provided, maintenance considerations (frequency, estimated cost, access, equipment locations), operability (ease of use, placement of control features, simplicity), durability (withstand troop usage, ease of cleaning), sustainability, and energy consumption (HVAC, lighting, power, etc.). The minimum acceptable level of quality for finishes and materials for these buildings are those materials suitable for the expected population and usage. Residential or similar grade finishes and materials are not acceptable.

1) The Government encourages the Offeror place emphasis on those design features which optimize and emphasize functional/operational requirements; interior/exterior finishes and systems; and life cycle/ energy efficiency. The following items will be considered in the evaluation of the building systems and materials of the various facilities:

Proposals will receive additional consideration for components and systems offered that include extended warranties, longer life expectancies, sustainability, durability (stand up to troop usage), have low maintenance requirements, and enhance the overall life cycle cost efficiency of the facility.

4) Mechanical Components and Systems. Proposals include components and systems that provide the basic environmental control function necessary. Proposals will receive additional consideration for components and systems offered that include extended warranties, longer life expectancies, reduce energy consumption, sustainability maintainability (cyclical maintenance, access, equipment placement), and enhance the overall life cycle cost efficiency of the facility.

5) Plumbing Components and Systems. Proposals include components and systems that provide the basic function necessary. Proposals will receive additional consideration for components and systems offered that include extended warranties, longer life expectancies, sustainability, durability (stand up to troop usage), have low maintenance requirements, and enhance the overall life cycle cost efficiency of the facility.

6) Electrical Components and Systems. Proposals include components and systems that provide the basic function necessary. Proposals will receive additional consideration for components and systems offered that include extended warranties, longer life expectancies, sustainability, durability (stand up to troop usage), have low maintenance requirements, and enhance the overall life cycle cost efficiency of the facility.

7) Site Utilities Components and Site Systems. Proposals include components and systems that provide the basic function necessary. Proposals will receive additional consideration for components and systems offered that include extended warranties, longer life expectancies, and sustainability, have low maintenance requirements, and enhance the overall life cycle cost efficiency of the facility.

8) Interoperability of Systems: Proposals will include a description of how new or renovated facility connections will interface with existing installation wide systems for the project. At a minimum Fire Alarm systems should be addressed. Proposals will receive additional consideration for addressing all applicable systems of a project such as Telephone, Cable, UMCS and privatized utility.

9) Energy Conservation: Proposals describe a feasible strategy to achieve required energy savings stated in the RFP. Proposals will receive additional consideration for describing how they can achieve energy savings greater than required by the RFP.

10) ATFP Considerations. This consideration describes the inclusion/compliance with the building related elements such as structural design for stand-off distances and windows. Proposals are compliant with all ATFP requirements.

10.2 Design Technical Approach – Quality of Building Systems. Will be rated using the following adjectival descriptions.

11. PHASE TWO - TAB G - FACTOR 6 - Small Business Participation Plan

11.1 SUBMISSION REQUIREMENTS.

11.1.1 Small Business Participation Plan: ALL Offerors, regardless of size status, are required to address two elements: (1) planned small and small disadvantaged business usage for this procurement, and (2) past history of using small and small disadvantaged businesses on any type projects during the last 3 years.

11.1.2 Planned Small and Small Disadvantaged Business Usage for this Procurement.

All Offerors are required to provide a narrative discussion of no more than two (2) pages at minimum 10 font addressing their corporate approach and methodology for acquiring, soliciting and using small and small disadvantaged businesses in the performance of this contract. All narratives should address the Offeror's commitment to providing subcontracting opportunities, as well as evidence of planned and /or continued outreach efforts to encourage and use small and small disadvantaged businesses.

11.1.3 All Offerors (both large and small businesses) will be evaluated on the level of proposed participation of U.S. small businesses in the performance of the proposed acquisition (as small business prime Offerors or small business subcontractors) relative to the objectives and goals established herein. The Government will evaluate the extent to which Small Businesses (SBs) Veteran-Owned Small Businesses (VOSBs), Service- Disabled Veteran-Owned Small Businesses (SDVOSBs) HUBZone Small Businesses, Small Disadvantaged Businesses (SDBs), Woman-Owned Small Businesses (WOSBs) and, when applicable, Historically Black Colleges/ Universities/Minority Institutions (HBCU/MIs) have been identified for participation as part of the Offeror's team. This should include a general description of the type of work, product or service anticipated to be supplied via a small business concern along with the estimated percentage each subcontracted portion is in relation to the overall project. Identification of the names of specific subcontractors is not required for the small business utilization evaluation;

however, successful Offerors will ultimately be required to name subcontractors as a part of their subcontracting plan submission. Using the template provided in Attachment 4, provide the above stated information. The Offeror with higher percentages for Small Business participation will receive a higher rating than the Offeror who has lower percentages. Submission shall be limited to the Attachment 4, in addition to the two (2) page narrative.

(Example: Demolition Work- 10% of total work effort- SDVOSB identified)

11.1.4 Past History of Using Small and Small Disadvantaged Businesses as Subcontractors. Provide a description of the Offeror’s demonstrated small business achievements in the various small and small disadvantaged businesses areas on no more than five (5) completed projects during the past three (3) years. Information about each project shall be limited to one (1) page. For each project, Offerors should provide a brief project description, dollar magnitude of the project, location, and the percentage of total work that was subcontracted to small businesses, identifying specific Small Business categories used on each project. For this submission, large businesses may submit no more than one (1) page (10 font minimum) of information pertaining to their goals and actual accomplishments under approved Small Business Subcontracting Plans.

11.2 EVALUATION CRITERIA.

11.2.1 All Offerors, regardless of size, will be evaluated in these two elements and must submit the information requested. In element (1), all Offerors will be evaluated on the extent to which the Offeror identifies and commits to small business participation in the performance of this contract. The Offeror’s percentage of work committed to small business will be evaluated.
