Government furnished equipment repairs will be a cost reimbursable expense under TP-8. Equipment replacement will be accomplished by the Corps or under level II services, as determined by the COR.
TP-1.11.1 Durables: The contractor will be provided with additional government furnished equipment that is considered expendable or durable property and thus is not listed on property records as accountable property. This equipment includes items such as power tools, hand tools, etc. that are currently being used on the project by the existing Operations and Maintenance (O&M) contractor. Replacements will be accomplished by the Corps or under level II services, as determined by the
COR.
TP-1.12 Records and Cost Schedule Plan: The contractor will have a cost schedule plan with a work breakdown structure of anticipated cost estimates incurred for each TP for each month of the contract (reference Appendix F).
The plan will be updated monthly. Daily written records will be maintained for all maintenance, repair, and operation activities. The contractor will keep accurate and complete records of these costs by business line and TP and provide to the COR monthly, as well as cumulatively for each contract year. Cost data for each activity will be maintained and is subject to government audit. This cost data will include labor (in hours and dollar value), supplies, materials, or parts costs for each separate scheduled work task. Separate work repair records and reports will be maintained on vehicles and equipment, to include maintaining separate records for vehicles the Corps leases from GSA.
TP-1.12.1 Records: The contractor will maintain as-built sketches of any changes occurring as a result of any repairs to structures, facilities and/or systems. Copies of these records will be provided to the COR.
TP-1.12.2 Capital Improvements: The contractor will provide to the COR specific information relating to the description and cost of each capital improvement accomplished during the performance of this contract. Specifics, as to necessary information to be included in the form and submittal frequency, will be established at the pre-work conference.
TP-1.13 Emergency Services: The contractor will furnish, and keep current, an on-call roster (a list of personnel to contact) in the event emergency repairs or services become necessary 24 hours per day, 7 days per week. The contractor will ensure contact can be made with these individuals by phone within one (1) hour of notification.
Response with appropriate personnel to perform necessary repairs will take place within two (2) hours of initial notification and work will continue until an emergency no longer exists. All unscheduled/emergency services in TPs 1-5 will be handled as Level II services per TP-1.26.2.
TP-1.14 Pesticide Applications: All pest control products and application methods will be in accordance with ER- 1130-2-540, a copy of which will be provided to the contractor at the pre-work orientation. The contractor will submit, NLT 30 days after award of the contract, a pesticide application plan detailing chemicals to be used, application rates for each, user and public safety measures, applicator licensing procedures, safety data sheets, a disposal plan for unused chemicals and areas anticipated to be treated. Areas to be treated may be grouped by use, i.e., beaches, playgrounds, food plots, buildings, etc. No pesticides will be applied prior to approval of this plan.
TP-1.14.1 The contractor will provide to the COR, NLT 30 November annually, a description of the anticipated quantity of the various chemicals intended for use the upcoming year. The contractor will also provide to the COR, NLT 15 January of each calendar year, a complete listing of actual pesticide applications performed during the previous year. Applications will be recorded on the form as attached in Appendix G.
TP-1.15 Contractor Quality Control (CQC): The contractor will develop and maintain an effective quality control (QC) plan incorporating a preparatory, initial, and final inspection process. A preliminary draft of the plan will be submitted for approval at the pre-work conference. The contractor will complete and resubmit the QC plan revision within 30 calendar days after receiving the COR’s review comments. The plan must include an organizational chart identifying lines of authority, letters of appointments detailing specific duties, responsibilities and limitations, and positions within the contractor’s organization.
TP-1.15.1 The CQC will provide for the preparatory planning and inspection of all work activities to ensure conformance to specifications, preparatory inspection agreements, drawings, and referenced documents. The CQC will also be responsible for inspection and documentation of findings for contract work, inventory control, procurement activities, safety implementation and other areas of contract performance. The CQC will not be directly responsible for the direction of on-site corrective actions related to performance deficiencies.
TP-1.15.2 Government quality assurance personnel will assure that contractor quality control is complete, but the presence or absence of government quality assurance will not relieve the contractor of its responsibility for the proper execution of work in accordance with the contract.
TP-1.15.3 The contractor will propose a compliance form that will be approved by the COR for use for all compliance inspections. This recorded information will be provided to the superintendent, and a copy furnished to the COR, within 24 hours following the period of performance the report covers. The form should make provisions to address safety, equipment usage, weather, personnel, type of work being performed, performance results, QC testing and results, discussions with government personnel, directions and agreements, as a minimum.
TP-1.15.4 The text of compliance inspection reports will address acceptable, as well as deficient, performance in management, quality control and workmanship. The CQC is to evaluate performance in all respects and report to management, through daily reports, their findings and other observations concerning the above. The reports must also recommend corrective action to the superintendent, as well as document superintendent-directed corrective actions.
If, in the opinion of the COR, recurrent deficiencies indicate that the quality control system is inadequate, corrective action will be taken as directed by the COR.
TP-1.15.5 Contractor proposed changes to the quality control plan will be submitted to the COR prior to its implementation.
TP-1.16 Preventive Maintenance Plan (PMP): The Contractor will develop a PMP for mechanical equipment and operational systems at the various project facilities. The plan must designate the unit, machine or system to be inspected, frequency and date of planned inspections. Records of inspection findings and services performed will be maintained. A summary report of repairs will be provided to the COR at the end of each contract year.
TP-1.17 Job Order Accounting System: The contractor will establish a system to effectively prioritize and schedule the performance of tasks, whether initiated through the contractor’s own initiatives or through a COR-initiated work order. The system will incorporate a method by which the contractor provides feedback to the COR that the work order has been scheduled and/or services have been performed.
TP-1.17.1 The system will be capable of establishing a budgeted cost as well as capturing all actual costs associated with the completion of the above work tasks. Costs will be appropriately charged to the applicable technical provision as referenced in Section H, Cost Schedule Requirement. In addition, costs will be appropriately charged to the applicable business line and park or location and cost reports submitted with monthly invoice.
TP-1.17.2 Completion and cost data for each work order will be entered by the contractor in the project’s Facility and Equipment Maintenance system (FEM), a Maximo database. The contractor will enter all contractor-initiated and a portion of Corps-initiated work orders in FEM as directed.
TP-1.18 Schedules: The contractor will prepare and submit logical and accurate work schedules to the COR for maintenance, service and construction as described below. Revisions to schedules will be provided to the COR within 24 hours of that revision.
TP-1.18.1 Cleaning schedules, identifying scheduled arrival and departure times for each location identified with peak and non-peak cleaning in Appendix H, will be provided to the COR for approval.
TP-1.18.2 Weekly work schedules for facility maintenance tasks will be submitted each Monday identifying all scheduled routine maintenance. Schedules should indicate task, location, duration, special equipment and tool needs and personnel requirements.
TP-1.18.3 Grass mowing schedules will be provided to the COR for approval, indicating location and date when mowing activities will be accomplished, so as to meet the requirements of TP-4, Mowing.
TP-1.18.4 Schedules for complex repairs and construction tasks, that address start and end dates for different phases of the task, will be provided to the COR for approval. Adequate breakdown of the task will clearly indicate anticipated duration of repairs and construction activities.
TP-1.19 Incidental Work: Incidental work items related to other specific work requirements throughout these specifications, but not specifically identified, described or quantified, may be required of the contractor. Such incidental work will be limited to support maintenance for the operation of existing facilities that cannot be anticipated. Due to the unknown quantities under this TP, it should not be addressed in the contractor’s technical or cost proposal.
TP-1.19.1 The contractor may be required to provide similar O&M support to other Corps projects or activities in the event of emergency situations or the inability of the local O&M contractor to meet contractual obligations. Due to the unknown quantities under this TP, it should not be addressed in the contractor’s technical or cost proposal.
TP-1.20 Improvement or Modification: A facility improvement or modification will not be initiated without the express approval of the COR. An improvement or modification is defined as any change to the original design of a building or structure or installed systems. Improvements or modifications not specifically approved in advance will be considered unauthorized and the contractor may be required to make restoration at no cost to the government.
TP-1.21 Accident Prevention: All accidents will be verbally reported to the COR immediately. The contractor will maintain an accurate record of, and will report to the COR, all accidents within 24 hours of the occurrence.
TP-1.21.1 The contractor will submit to the COR accident exposure and experience incident to this work, in accordance with EM 385-1-1. An exposure report will include all Contractor employee work hours performed during the reporting period. The work hours should be separated as "Work" and "Administrative" and will include all lost time accidents and equipment accidents over $500.00. The report will be submitted to the COR NLT the 30th of each month.
TP-1.21.2 Public Safety: The contractor will be responsible for safeguarding construction and maintenance sites in public recreation use areas. Appropriate signs, warning devices, and barriers will be used to protect the general public, contractor employees, and government employees in accordance with applicable provisions of
EM 385-1-1.
TP-1.22 Hazardous Waste Disposal: All hazardous waste materials as defined in 40 CFR 260, will be disposed of IAW all applicable county, state, and federal regulations. The contractor will submit information regarding the proposed disposal method and facility to the COR for approval a minimum of one week prior to transportation of wastes.
