About this file
This is an Amendment 0002 Summary of Changes document that modifies the Statement of Work (SOW) for solicitation W912ES26QA013, a Quality Assurance contract for Lock and Dam maintenance and gate fabrication projects managed by the U.S. Army Corps of Engineers, St. Paul District.
The amendment updates personnel qualification requirements and inspection procedures for the LD 3 and 4 Miter Gate Fabrication Quality Assurance work. Key changes include: Weld Inspector must be AWS Certified Welding Inspector (CWI) certified per AWS QC1; Bolted Connection and Assembly Inspector must hold International Code Council (ICC) Structural Steel & Bolting S1 Special Inspector Certification; and NDT Technician must have ASNT CP-189 NDT Level II certification. The SOW revises fabrication inspection requirements to include one First Steel Cutting Inspection per lock to verify steel cutting, piece marking, and tracking procedures. Each miter gate leaf requires three timed weld inspections: Initial Inspection (around 30% fabrication completion with 0.1-5% weld sample size), Intermediate Inspection (around 70% completion), and Pre-Final Inspection (around 85% completion before painting). All welds must receive visual inspection with sampled welds undergoing destructive and non-destructive testing. The contractor must prepare written reports for each site visit documenting QA work completed, all visual and NDT inspection results in digital format, and photos with corrective action recommendations for noncompliant areas. The fabricator's QC program adequacy assessment must also be included in reports.

Summary of SOW Changes – Amendment 0001 Paragraph 4.2 changed to:
Weld Inspector. The weld inspector must be an AWS Certified Welding Inspector (CWI) in conformance with AWS QC1.
Paragraph 4.3 changed to:
4.3 Bolted Connection and Assembly Inspector Qualifications. The bolted connection and assembly inspector must have the following course certifications at a minimum:
· International Code Council (ICC) Structural Steel & Bolting S1 Special Inspector Certification
Paragraph 4.4 changed to:
4.4 NDT Technician. Non-destructive testing (NDT) personnel must have the following course certifications at a minimum:
· ASNT CP-189 NDT Level II.
Paragraph 5.4.1.1 paragraph changed to:
Weld & Fabrication Inspection
· Perform on-site fabrication inspections of structural fabrication assemblies. Conduct field inspections during fabrication as agreed to with the Government and detailed in the QA inspection plan. The Contractor shall review contract plans and specifications for each structural fabrication assembly, so they are familiar with the welding requirements, welds, Fracture Critical Members (FCM), and assembly procedures. The fabricator site inspections shall include review of the fabricator’s QC program; verification of QC documentation; review of equipment; proper material storage; material preparation; record keeping; welding, bolting; and QC procedures including NDT, weld repair procedures, and QC testing.
· One First Steel Cutting Inspection will be required for both the Lock 3 miter gates fabrication and the Lock 4 miter gates fabrication as follows:
· First Steel Cutting Inspection. The Contractor must schedule this inspection to verify the steel cutting, piece marking, and tracking procedure that is used by the fabricator. One steel cutting inspection is required per Lock.
· Each miter gate leaf will require three weld inspections as follows:
· Initial Inspection. The initial field inspection shall commence after substantial completion of subassemblies and comprise a reasonable sampling of each significant FCM and non FCM. This inspection shall commence around 30% completion of fabrication. The sample size will range from 0.1 to 5 percent of the length of the weld with the final approval of the size by the COR.
· Intermediate Inspection. The QA Contractor shall schedule the intermediate inspection such that a verification of final assembly process can be ascertained, but before a substantial portion of the final assembly has been completed. This inspection shall commence around 70% completion of fabrication.
· Pre-Final Inspection. The pre-final inspection shall be conducted after welding has been completed but before painting has commenced. This inspection shall commence around 85% completion of fabrication.
· All welds on each structural fabrication miter gate leaf shall be visually inspected. Identify a sample of welds suitable for QA testing-see Initial, Intermediate, and Pre-Final Inspection paragraphs above. The sample size will range from 0.1 to 5 percent of the length of the weld. QA weld testing shall be in conjunction with testing of materials and products both by destructive and non-destructive testing.
· Identify inspected welded connections that are not compliant with contract specifications.
· Prepare a written report for each structural fabrication miter gate leaf site visit that includes a summary of QA work completed and listing of all visual and NDT inspections completed. NDT results shall be submitted to the Government in digital format when the written report is submitted. If QA work indicates that welding and fabrication contract specifications are not being met, photos of noncompliant areas and recommended actions to correct the deficiencies shall be included in the report. The adequacy of the fabricator’s QC program shall be included.
