The following have been added by full text:
SOURCE SELECTION PLAN
SOLICITATION PROVISIONS
1.0. BASIS FOR AWARD
1.1. A single award will be made to the Offeror who provides a technically acceptable proposal with the overall lowest price.
1.2. Proposed prices will be evaluated but not scored or otherwise combined with the other aspects of the proposal evaluation.
2.0. FACTORS TO BE EVALUATED
2.1. In order for an offeror to be considered for award, the proposal must receive an “Acceptable” rating in every non-price factor. The two (2) non-price factors are Technical Certifications (Factor 1) and Past Performance (Factor 2). All three (3) evaluation factors are as follows:
2.1.1. Factor 1: Technical
2.1.2. Factor 2: Past Performance
2.1.3. Factor 3: Price
3.0. EVALUATION APPROACH
3.1. All proposals shall be subject to evaluation by the Source Selection Team (SST).
3.2. Factor 1: Technical
3.2.1. The Government will evaluate the Technical Factor on an Acceptable/Unacceptable basis.
This factor identifies the minimum requirements that are imperative to successful contract performance.
3.2.2. The Source Selection Team will evaluate the Offeror’s proposal using the ratings and descriptions outlined in Table A-1. Offerors who are determined “Unacceptable” will not be eligible for award.
3.2.3. Technical Competency Documentation:
3.2.3.1. The Technical Factor shall include the following to be considered eligible for award:
The Offeror shall submit one (1) copy of the documentation as follows:
1) Documentation of membership in the American Gear Manufacturer’s Association
(AGMA).
2) One page “fact sheet” each for fabrication of three open gears used in heavy industrial or transportation settings, to include locks and dams, bridges, mills, mines, or shipping/marine. The diameter of the open gear shall be 5’-0” or greater in diameter. Fact sheet shall include narrative of the gear, dimensions of the gear, and drawings/photos (if available).
3) Resume showing experience and qualifications of one company employee, who would be assigned to this project, who has supervised design, detailing, fabrication, and quality control activities with regard to manufacturing of gears of similar size and complexity to the existing crank gear.
3.3. Factor 2: Past Performance
3.3.1. The Government will evaluate the Past Performance factor on an Acceptable/Unacceptable basis based on the descriptions outlined in Table A-2. Offerors who are determined “Unacceptable” will not be eligible for award.
Table A-2. Past Performance Evaluation Ratings
Rating Description
ACCEPTABLE Based on the Offeror’s performance record, the Government has a reasonable expectation that the Offeror will successfully perform the required effort, or the Offeror’s performance record is unknown. (See note below)
Table A-1. Technical Acceptable/Unacceptable Ratings
Rating Description
ACCEPTABLE Proposal meets the requirements of the solicitation.
UNACCEPTABLE Proposal does not meet the requirements of the solicitation.
UNACCEPTABLE Based on the Offeror’s performance record, the Government does not have a reasonable expectation that the Offeror will be able to successfully perform the required effort.
Note: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305(a)(2)(iv)). Therefore, the offeror shall be determined to have unknown (or “neutral”) past performance. In the context of acceptability/unacceptability, a neutral rating shall be considered “acceptable.”
3.3.2. Evaluations of past performance shall be in accordance with this plan while utilizing the Past Performance Questionnaires (PPQ) and forms in Appendix D, Enclosure 2.
3.3.3. An Offeror’s past performance is a measure of the degree to which an Offeror was successful on projects of similar scope, size, and complexity. The Government will evaluate the information/performance data provided by Offerors, as well as information from other sources, such as Government databases.
3.3.3.1 The Offeror shall submit up to five (5) PPQ’s for work performed since 2010 for gear fabrications. If more than five (5) are submitted, only the five (5) most recent projects will be considered. The forms shall identify whether the contract is active or complete, whether the contract was a Government (federal, state, local, etc) or a commercial entity, and whether the Offeror was the prime contractor, a subcontractor, or member of a joint venture. The PPQ respondent must have been a client, or client representative, with whom the Offeror/subcontractor had a contract, or subcontract (if applicable) for one or more projects.
3.3.4. The PPQ respondent must hold, or have held at the time, a senior position in the client organization with responsibilities for either contract administration, quality control, engineering, or technical supervision, or was otherwise in a position to evaluate the Offeror’s performance in executing the contract or subcontract. The PPQ respondent must have first-hand knowledge of the Offeror’s performance.
3.3.5. In addition to the above, past performance information may be obtained from established systems such as the Contractor Performance Assessment Reporting System (CPARS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; and may be obtained from other sources available to the Government, such as the Defense Contract Management Agency; and interviews with Program Managers, Contracting Officers, and Fee Determining Officials.
3.3.6. The Government reserves the right to evaluate the past performance of any subcontractor identified in the Offeror’s proposal as part of its assessment of the acceptability of that Offeror’s proposal.
3.3.7. While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.
3.4. Factor 3: Price
3.4.1. Price will be evaluated and considered but not scored or combined with other aspects of the proposal evaluation. An award will be made to the lowest priced, technically acceptable proposal. Offerors shall complete and submit the Schedule of Prices with its proposal and include a price for each line item. Failure to do so may result in the proposal being rejected without further consideration.
3.4.2. The Source Selection Evaluation Board will evaluate each Offeror’s price and verify that unbalanced pricing is not present and performance risks are minimal. The Contracting Officer will evaluate each Offeror’s price in accordance with FAR 15.404-1 to determine price reasonableness. Price reasonableness will be determined by comparing all proposed prices, the Independent Government Estimate and pricing of previous work that is same or similar to the requirement.
3.4.3. The otherwise technically acceptable lowest priced Offeror may be required to confirm/validate their offered price. Certified cost and pricing data are not currently required; however, the Government reserves the right to request such data prior to award.

ADJECTIVE RATINGS AND DEFINITIONS TO BE USED TO BEST REFLECT YOUR
EVALUATION OF THE CONTRACTOR’S PERFORMANCE
Form PPQ -0
RATING DEFINITION
NOTE
(E) Exceptional
Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.
An Exceptional rating is appropriate when the Contractor successfully performed multiple significant events that were of benefit to the Government/Owner. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating. Also, there should have been NO significant weaknesses identified.
(VG) Very Good
Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
A Very Good rating is appropriate when the Contractor successfully performed a significant event that was a benefit to the Government/Owner. There should have been no significant weaknesses identified.
(S) Satisfactory
Performance meets minimum contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
A Satisfactory rating is appropriate when there were only minor problems, or major problems that the contractor recovered from without impact to the contract. There should have been NO significant weaknesses identified. Per DOD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than Satisfactory solely for not performing beyond the requirements of the contract.
(M) Marginal
Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.
A Marginal is appropriate when a significant event occurred that the contractor had trouble overcoming which impacted the Government/Owner.
(U) Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.
An Unsatisfactory rating is appropriate when multiple significant events occurred that the contractor had trouble overcoming and which impacted the Government/Owner. A singular problem, however, could be of such serious magnitude that it alone constitutes an unsatisfactory rating.
