FOR INITIAL OUTFITTING AND TRANSITION (IO&T)

Scope: This Accident Prevention Plan established the Accident Prevention Plan for the contractor that is to minimize or eliminate the hazards associated with the handling, transporting, off loading, and installation of furniture and furnishings for military installations of the Armed Forces of the United States.
Purpose: The Accident Prevention Plan is a document that details how the safety and health requirements of OSHA; other regulatory codes and documents; and the U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385‐1‐1 are to be accomplished.

Responsibilities:
Project Manger shall:
‐ Oversee and direct the work activities to include plan for safety and health of all persons involved.
‐ Document that employees are trained in the phases of the work activities to be performed.
‐ Document that all equipment is maintained in proper working order and used correctly.
‐ Document that personal protective equipment as lumbar supports, gloves, and others equipment needed is available and used.
‐ Perform an Activity Hazard Analysis of new or unique jobs or tasks and the safety and health measures to be used to minimize hazards.
‐ Conduct Safety meeting with the crew prior to task activities beginning.

Accident Prevention Plan
Employees shall:
‐ Follow the prescribed methods for performing the task to be performed.
‐ Wear protective equipment as required for each task of the work. Perform task in manners to protect themselves and others in the area. Protect the public from work activities and the equipment being used.
‐ Inspect equipment prior to its use and remove defective equipment from service immediately.
‐ Use personal protective equipment such as lumbar support, gloves, and others as required.
Accident Reporting:
‐ The Project Manager shall report all accidents to the U.S. Army Corps of Engineers, Huntsville
Engineering and Support Center Project Manager immediately.
‐ All injuries, including First Aid cases, are to be recorded on OSHA Log 300 or similar report.
‐ All accidents shall be reported and submitted to the U.S. Army Corps of Engineers, Huntsville
Engineering and Support Center Project Manager.
‐ The Contractors man‐hours worked and accident experience will be reported to U.S. Army Corps of
Engineers, Huntsville Engineering and Support Center Project Manager by the first of each calendar month.
‐ Each accident will be investigated and reported on ENG Form 3394 and the report will be reported to CEHNC within 5 days. http://www.hnd.usace.army.mil/safety/Safety_
Work Activities:
The following activities are principal in the work of this contract. Additional hazards and activities may require a Hazard Assessment of the additional work and an Activity Hazard Analysis of the work, the associated hazards, and control measures to be taken when performing the activity.
1. Equipment/Furniture Handling – Manual ‐ ‐ Employees shall be trained in safe lifting techniques.
‐ ‐ Mechanical or motorized equipment will be utilized when loads are too heavy or bulky in shape or size – by trained and bonded third party vendors.
‐ ‐ Material shall not be suspended above workers or the public.
‐ ‐ Children and other persons shall not be allowed in the area work activities. Personal
Protective Equipment as lumbar support belts or other devices shall be used to minimize the risk of sprains or strains.
2. Hand and Power Tools ‐ Employees shall be trained in safe use of hand and power tools.
‐ Hand or power tools will not be utilized by IAP personnel when work is being performed by the installers.
‐ Children and other persons shall not be allowed in the area work activities.

Training:
The Contractor is responsible for briefing employees as well as any subcontractor personnel on the site, in all areas of the Accident Prevention Plan and special training as listed below:
‐ Material Lifting – Manual. Proper lifting techniques and the use of the “Buddy system” ‐ Accident reporting for injuries and property damage.
‐ Emergency Procedures for knowing and reporting accidents, fires, and other emergencies.
Job Safety Analysis & Activity Hazard Analysis (AHA) A Job Safety Analysis form must be completed to analyze jobsite conditions and identify any worksite/environmental hazards or challenges and an Activity Hazard Analysis (AHA) is to be developed for each principal steps of the work. AHA’s are to detail the work phase, the associated hazards, and the corrective measures used to minimize or eliminate the hazards.
AHA’s for the principal job steps are enclosed below. If other activities are identified, an AHA for this step or phase, an AHA is to be developed, reviewed, and accepted for this work. The newly created AHA is to be reviewed by the Supervisor and Approved prior to performing the work.

IAP will employ the services of a Furniture vendor for all furniture installation to include loading/unloading. The following is still provided:
Employees are to be trained in proper lifting techniques.
Forklift operators will be trained, carry appropriate certification and perform pre‐operational checks on the equipment.
Forman to use more than one person for size and weight of load.
