SMALL BUSINESS PARTICIPATION PLAN
(a) Check the applicable size and categories for the PRIME offeror only -- Check all applicable boxes:

(b) Submit the total combined percentage of work to be performed by both large and small businesses (include the percentage of work to be performed both by Prime and Subcontractors):
Note: All offerors should assume a total contract value of $450,000,000.00. For example, if a large business prime proposes a MATOC level plan of $450,000,000.00 (including all options), then small businesses must provide a minimum of $67,500,000.00 (15%) of the services/supplies as subcontractors.
For small business primes to be eligible to compete for small business set-aside task orders, the small business prime must self-perform 51 percent of the effort as indicated in the small business participation plan. A small business’ eligibility to compete for set-aside work will be evaluated from its proposal on the base contract.
For small business primes to compete for full and open task order requirements, the small business prime must self perform or subcontract to small businesses a minimum of 15 percent of the effort.
For the plan, use this format, as follows:

(c) Indicate the total percentage of participation to be performed by each type of subcategory small business. If the prime offeror is a small business, they should include the dollar and percentage they will self perform in the appropriate category in the table below, as well as any subcontracted small businesses. The percentage of work performed by Small Businesses that qualify in multiple small business categories may be counted in each category as demonstrated in the following example:
Example: Victory Prop Mgt (WOSB and SDVOSB) performing 2% and Williams Group (SDB, HUBZone and WOSB) performing 3%. Results equate to: SDB 3%, HUBZone 3%, WOSB 5%, SDVOSB 2%, and VOSB 2%.
SDVOSBs are also automatically VOSBs; however VOSBs are not automatically SDVOSBs.

(e) Describe the extent of commitment to use small businesses (for example, what types of commitments if any are in place for this specific acquisition either – small business prime, written contract, verbal, enforceable, non-enforceable, joint venturing, team agreement, mentor-protégé, etc.)
(f) Past Performance Information: Provide any information substantiating the offeror’s track record of utilizing small businesses on past contracts. For large business include Administrative Contracting Officer (ACO) rating and at a minimum the two most recent SF295s or Electronic Subcontract Reporting System (eSRS) reports.
