About this file

This is a Past Performance Questionnaire (PPQ) template for solicitation W912CH-24-R-0175 issued by the Army Contracting Command-Detroit Arsenal (ACC-DTA) for Corrosion Prevention and Engineering Services. The questionnaire must be completed by references and submitted to stephanie.a.douangmala.civ@army.mil by March 10, 2025 at 5:00 PM EST.

The PPQ evaluates contractors' past performance across four main areas: technical/business relations (including coating/corrosion engineering capabilities and team effectiveness), schedule/timeliness (focusing on technical reporting and document revision capabilities), management (specifically database management for corrosion surveys), and general comments. Respondents rate performance using a scale from Exceptional to Unsatisfactory and must provide narrative comments explaining any adverse issues. The template includes sections for both the offeror to complete contract identification details and for references to provide performance evaluations that will be used in the source selection process.
