SUBJECT: Amendment No. 0002 to Specifications and Drawings for Construction of Middle Logan Creek, Flood Risk Management Project, Phase 2, Randolph NE.
1. The specifications and drawings for subject project are hereby changed as follows (revise all specification indices, attachment lists, and drawing indices accordingly).
a. Specifications. (Descriptive Changes.)
(1) Section 01 57 20.00 10 Page 10, delete paragraph 3.1.2 and substitute the following:
“3.1.2 Nebraska Department of Environment and Energy Title 123 Rules and Regulations for the Design, Operation and Maintenance of Wastewater Works
The State of Nebraska requires a permit for addition to any wastewater works. This project includes a siphon and will require a permit. See http://deq.ne.gov/RuleAndR.nsf/RuleAndReg.xsp?documentId=9A59462AE6FE4CF306256BB9005475E1& action=openDocument for a copy of the Nebraska Administrative Code for Title 123 Rules and Regulations for the Design, Operations and Maintenance of Waste water Works for information on the required permit.
The US Army Corps of Engineers will apply for this permit with two sets of plans, specifications, calculations, and the Construction Permit Application Form Wastewater Works from Nebraska DEE. There are no fees associated with this permit. A copy of the permit will be provided to Contractor when obtained by the Corps of Engineers. “
(2) Section 02 82 00 Page 4, remove PART 1 GENERAL and substitute the following:
“Part 1 General
Attachment: Hazardous Materials Survey Technical Memorandum, 212 South Douglas Street, Randolph, NE
Structures at 212 S. Douglas Street, Randolph, NE will be demolished, including the residence containing the asbestos-containing building materials identified in the attached technical memorandum. Approximate quantities and locations of the materials are identified in the attachment. Asbestos-containing materials identified in the residence include wall and ceiling plaster, 9”x9” floor tiles, window glazing on the main porch windows, door window glazing on the main porch door, air cell pipe insulation on pipes in the basement, pipe insulation wrap on pipe joints in the basement, and roof flashing around roof penetrations. In addition, materials containing less than 1% asbestos include door window glazing on the breezeway door exterior and floor tile adhesive associated with the asbestos-containing floor tiles. As an alternative to removing asbestos-containing glazing from doors and windows, the Contractor may choose to remove doors and windows with asbestos-containing glazing intact. Materials containing less than 1% asbestos are not required to be removed, however, the Contractor shall comply with the applicable work practices required by 29 CFR 1926.1101(g)(1)(ii) and 29 CFR 1926.1101(g)(1)(iii) to protect workers during any tasks that may disturb such materials.
No asbestos-containing materials were identified in the outbuildings for this residence. Remove asbestos-containing materials from the residence prior to demolition in accordance with this specification and applicable regulations.”
(3) Section 02 83 00 Page 3, remove PART 1 GENERAL and substitute the following:

Structures at 212 S. Douglas Street, Randolph, NE will be demolished. In addition, the interior paint in the residence contains poly-chlorinated biphenyls (PCBs) (see the Hazardous Materials Survey Technical Memorandum). The concentration of PCBs in the paint is not at regulated levels and does not require special disposal action. Protect workers and the environment from exposure to lead and PCBs during the demolition process in accordance with this specification and applicable regulations. Removal of paint from the structures prior to demolition is not required.”
