Re: Hazardous Materials Report - ACBM, Mold, LWP, PCB’s, and Hg Building 2263 JBSA, Fort Sam Houston, Texas dse Project No. 1749804

On Oct. 27 and Nov. 15 & 16, 2017, Dougherty Sprague Enterprises, Inc. (dse) performed a Limited Asbestos-Containing Building Material (ACBM), Mold, Lead-Containing Paint (LCP), Polychlorinated Biphenyl (PCB’s), and Mercury (Hg) inspections of the referenced building (the “Building”). The inspections were limited to a review of the Asbestos and Lead-Based Paint Survey, prepared by Argus Environmental Consultants, LLC for Weston Solutions Project No.

1210291ARG for buildings 2263, dated June 2013 (“Prior Reports” or if pertaining specifically to Asbestos, the “Argus Asbestos Survey”) A walk-through of the Building was performed followed by collection, and analysis of additional bulk and air samples to either clarify the previous sampling results or evaluate newly identified homogeneous areas of concern.

The limited inspections, sample collection, and observations were performed by dse employees Paul Heidgerd, John Dougherty, and Cathy Dougherty. At the time of the inspection, the Inspector was licensed by the Texas Department of State Health Services (TDSHS) or Texas Department of Licensing and Regulation (TDLR). dse is licensed by the TDSHS as an Asbestos Consultant Agency, Lead Firm, and Mold Assessment Company.

The material identified the following suspect areas that were either not addressed in the Prior Reports or were previously sampled and required clarification.

Limited ACBM Inspection:

The Limited ACBM Inspection was performed by dse employee Paul Heidgerd (the “Inspector”).

At the time of the inspection, the Inspector was licensed by the Texas Department of State Health Services (TDSHS) as an Individual Asbestos Consultant and dse was licensed by the TDSHS as an Asbestos Consultant Agency.

Limited ACBM, Mold, Pb, PCB, & Hg Inspection 2 dse Project No. 1749806

The Inspector identified nine homogeneous areas of suspect ACBM that were not addressed in the Argus Asbestos Survey:

Brown Vinyl Composition Tile (VCT) / Black Mastic Light Brown VCT / Black Mastic Gray Window Glazing Compound on Wood Windows 2-Part Cement Plaster Gray Window Glazing Compound on Metal Windows Attic Firestop Exterior Textured Paint Clay Tile Mortar Brick Mortar

Forty-two (42) bulk material samples were collected from the nine newly identified homogeneous areas of suspect ACBM and from several of the homogeneous areas of suspect ACBM identified in the Argus Asbestos Survey. The bulk sample locations are shown on Figure 1 in Appendix A.

The 42 suspect ACBM bulk samples were delivered by dse to Moody Labs and Cates Laboratories in Dallas, Texas for analysis of asbestos content using polarized light microscopy (PLM). Moody Labs and Cates Laboratories are licensed by the TDSHS as PLM Asbestos Laboratories and accredited by the National Voluntary Laboratory Accreditation Program (NVLAP). The bulk samples were analyzed by PLM coupled with dispersion staining techniques in accordance with the 1982 Federal Regulations in 40 CFR 763, Subpart F. Copies of the PLM Analysis Laboratory Reports and Chains of Custody / Bulk Sample Logs are attached in Appendix B.

Asbestos was detected in two of the newly identified homogeneous areas of suspect ACBM:

Brown VCT / Black Mastic 5% Chrysotile Asbestos in the VCT / 3% Chrysotile Asbestos in the Black Mastic Gray Window Glazing Compound on Wood Windows 2% Chrysotile Asbestos

No asbestos was detected in the remaining seven newly identified homogeneous areas of suspect ACBM. The only ACBM identified in the Argus Asbestos Survey was Black Mastic under non- ACBM VCT in several areas of the Building.

The Brown VCT/Black Mastic ACBM (most likely 9” x 9”) appears to be the original layer of flooring throughout all four floors of the Building. The Black Mastic ACBM identified in the Argus Asbestos Survey is most likely remnants of the original flooring that was not abated before the installation of new flooring. Most areas of the Building have two layers of VCT. Several areas have one layer of VCT or only black mastic remaining. Some areas have one or two layers of carpet on top of VCT. VCT/Mastic on raised wood floors on former open-air porches on the south side of Building are not ACBM. The original flooring on the open-air porches is unknown. The large central area of the Third Floor occupied by the Navy at the time of the inspection was recently renovated including abatement of the ACBM flooring. See attached Limited ACBM Inspection Report for further clarification and recommendations.

Limited ACBM, Mold, Pb, PCB, & Hg Inspection 3 dse Project No. 1749806

Limited LWP Inspection:

The Limited Paint with Lead Inspection was performed by dse employee John Dougherty. dse is licensed by the TDSHS as a Lead Firm.

Concrete Walls & Ceilings (interior) Wall Exterior Surfacing (exterior) Wooden Window Frame and Casing (interior)

The two (2) paint samples (P1 & P) collected and analyzed for lead content and (73) XRF readings taken from the 2263 Building support the findings of the Former Report. The basement room 107B sample was taken from the ceiling above the drop ceiling, from a concrete surface and was found to contain 0.085 % by weight. The laboratory analysis of the exterior west entrance wall of the building (paint chip) sample, on the concrete wall, contained the highest concentration of lead reported at 3.6% by weight. These results confirmed the identification of lead in paint from the previous report above EPA and OSHA levels. The Consumers Product Safety Commission considers paint with less than 0.06% to be lead free which dse uses to establish OSHA Paint with Lead (PWL) limits (0.12 mg/cm2 or 0.06% by weight.)

The highest concentration paint chip of 12 positive samples taken in the Former Report was reported at 1.9% by weight from a Wood Window Frame. 11 of the 12 reported above regulatory guidelines. dse’s findings agree with the recommendations of OSHA requirements stated in the former report as well. A copy of the XRF Readings, Lead Analysis Laboratory Report, and Chain of Custody is attached in Appendix B. See attached Appendix B – Limited Lead with Paint (LWP) Inspection Report for specifics.

Recommendations:

Any demolition or construction activities would be subject to OSHA regulations, specifically 29- CFR 1926.62 – Lead Exposure in Construction. These OSHA regulations define specific training requirements, engineering controls and working practices for construction personnel subject to this standard.

Compliance with applicable OSHA lead regulations is the responsibility of the contractor performing the work and it is recommended that they be required to communicate potential lead hazards to their workforce and utilize lead-safe work practices such as outlined in the EPA Renovation, Repair, and Painting Final Rule (40 CFR 745) or applicable portions of the structural Steel Painting Council (SSPC) Guidelines. It is further recommended that activities such as flame/torch dismantling, dry sanding and/or dry grinding of any components with lead-containing materials applied should be prohibited as part of any repair, renovations, or demolition activity.

Limited Mold Inspection:

The limited air and bulk sample(s) collection was performed by dse employee Cathy Dougherty.

dse is licensed by the TDSHS as a Mold Firm. At the time of the inspection, the accessor was licensed by the Texas Department of State Health Services (TDSHS).

Currently, there are no federal regulations for acceptable mold levels in air. The American Conference of Governmental Industrial Hygienists (ACGIH) has guidelines for acceptable levels of mold in air for the general public. However, immuno-compromised individuals, the very young, the elderly and mold allergic persons may experience symptoms at low levels. Also, it is important to note that allergic reactions can come from dead spores and live spores alike.

Staining was observed to be limited to 4-6 ceiling tiles. The base personnel said that the past mildew smell was detected in the past. However, evidence of staining was identified in close proximity to a ceiling return air vent. See Photo at right. A sample was submitted for lab analysis. The water source was not readily identified.

Based on the amount of mold observed in the building interior inspected during the assessment for visible mold does not exceed 25 contiguous square feet per Texas State Regulations nor 10 square feet per Federal statute, therefore a protocol is not required.

Visible mold should be removed. Eliminate the water and clean the adjoining material using approved EPA Guidelines (attached). Maintenance and cleaning personnel can address any visible mold utilizing appropriate biocides and cleaning products appropriate for indoor use, and replace the building materials (sheet rock, ceiling tiles, etc.) in question. It should be noted that sampling

Limited ACBM, Mold, Pb, PCB, & Hg Inspection 6 dse Project No. 1749806 was performed on surface materials without intrusive means. Additional contamination is possible from the back side of these materials if wall cavities are opened.

Recommendations:

Base personnel should dispose of the stained tiles in proper PPE and techniques along with the regular outgoing trash. Eliminate the water source and clean the wall using approved EPA Guidelines (attached).

OTHER HAZERDOUS MATERIALS:

Limited PCB Inspection:

The limited observations were performed by a dse employee John Dougherty.

Based on the conversations between dse and AES, the scope of service for the renovations did not include a lighting upgrading.

Ballast & Florescent Tube (various)

Recommendations:

Contractors should dispose of florescent bulbs as Special Waste in the State of Texas.

Limited Hg Inspection:

The limited inspections and observations were performed by dse employee John Dougherty for Mercury switches, ballasts and similar materials. No suspect thermostats were observed, however in the process of renovation a Contractor could encounter a suspect thermostat, switch or similar device.

No or zero (0) suspect thermostats were observed.

Recommendations:

If encountered, dispose of mercury containing material properly by labeling and following local statutory regulations.

CONCLUSION

If the renovation plans for the buildings 2263 are modified or expanded, this Limited Inspection Report will need to be reviewed to determine if additional investigations, sample collection and analyses are required. The JBSA Operations and Maintenance (O&M) Plan for the Building should be updated to include the findings in this Inspection.

LIMITATIONS

This Limited Hazardous Materials Inspection of Building 2263 on Fort Sam Houston, San Antonio, Texas was performed by dse on Oct. 27 and Nov. 15 & 16, 2017 for Asbestos-Containing Building Material (ACBM), Mold, Lead With Paint (LWP), Polychlorinated Biphenyl (PCB’s), and Mercury (Hg) inspections of the referenced building (the “Building”). The inspections were limited to a review of the Asbestos and Lead-Based Paint Survey, prepared by Argus Environmental Consultants, LLC for Weston Solutions Project No. 1210291ARG for buildings 2263, dated June 2013 (“Prior Reports”) or if pertaining specifically to Asbestos, the “Argus Asbestos Survey”). The inspection conformed to generally accepted industry standards.

The entire Building 2263 was accessible during the inspection. No effort was made to open spaces such as pipe chases, crawl spaces, and attics that did not have access doors or hatches. In addition, no Building components such as electrical equipment, pipes or plumbing fixtures that would require specially trained workers to remove or reinstall were disturbed. The potential exists that a suspect material was not observed by the Inspector because it was located within structural cavities, under multiple layers of flooring, or was otherwise obscured or inaccessible. If suspect material that was not observed by, or was inaccessible to, the Inspector at the time of the inspection is encountered during the renovation project, this inspection will need to be updated to include that suspect material.

The assessment, sampling and analysis of suspect materials are highly interpretive activities.

Variability can be experienced in sampling results due to the nature of construction materials and techniques, even with experienced personnel and careful sample collection. This inspection evaluated the Building and conditions only at the time of the site visit in the areas inspected and the locations sampled. Conditions at other locations and times may vary significantly from these results. dse conducted this inspection using trained professionals following applicable government regulations and guidelines, and utilizing a reasonable “Standard of Care”, but cannot represent guarantees or warrantee results.

In order to understand all of the implications of this report, this entire report, including all attachments and appendices, must be read and understood. Any reader failing to read the entire report cannot hold dse responsible for any liabilities arising from this failure. If a reader has any questions about this report, its contents and/or conclusions, the reader should contact dse for clarification.

No warranty is expressed or implied by this report of the Limited Materials Inspections described herein. The limit of liability for omissions or errors, if identified, shall be the cost of these services rendered by dse to AES Group. Re: Limited Asbestos Containing Building Material Inspection Report Joint Base San Antonio (JBSA), Fort Sam Houston, Texas Building 2263 dse Project No. 1749804

On Oct. 27 and Nov. 15 & 16, 2017, Dougherty Sprague Enterprises, Inc. (dse) performed a limited asbestos-containing building material (ACBM) inspection of the referenced building (the “Building”). The inspection was limited to a review of the Asbestos Survey performed by Argus Environmental Consultants, LLC for Weston Solutions June 5, 6 & 25, 2013 (the “Argus Asbestos Survey”) followed by a walk-through of the Building and the collection and analysis of additional bulk samples to either clarify the previous sampling results or evaluate newly identified homogeneous areas of suspect ACBM.

Copies of both licenses are attached in

ATTACHMENT C.

The Inspector identified nine homogeneous areas of suspect ACBM that were not addressed in the Argus Asbestos Survey:

Brown Vinyl Composition Tile (VCT) / Black Mastic Light Brown VCT / Black Mastic Gray Window Glazing Compound on Wood Windows 2-Part Cement Plaster Gray Window Glazing Compound on Metal Windows Attic Firestop Exterior Textured Paint Clay Tile Mortar Brick Mortar

Limited ACBM Inspection 2 dse Project No. 1749804

Forty-two (42) bulk material samples were collected from the nine newly identified homogeneous areas of suspect ACBM and from several of the homogeneous areas of suspect ACBM identified in the Argus Asbestos Survey. The bulk sample locations are shown on Figure 1 in ATTACHMENT A.

Copies of the PLM Analysis Laboratory Reports and Chains of Custody / Bulk Sample Logs are attached in

ATTACHMENT B.

Asbestos was detected in two of the newly identified homogeneous areas of suspect ACBM:

Brown VCT / Black Mastic 5% Chrysotile Asbestos in the VCT / 3% Chrysotile Asbestos in the Black Mastic

Gray Window Glazing Compound on Wood Windows 2% Chrysotile Asbestos

Other Suspect ACBM Based on our inspections of buildings of similar design and age, the following suspect ACBM should be assumed to be ACBM at this time. These suspect ACBMs were not sampled because they were either not observed or were inaccessible.

Fire Doors, Grout and Thinset associated with Ceramic Floor Tile, Grout and Mastic associated with Ceramic Wall Tile, Mirror Mastic behind mirrors throughout the Building, Non-fiberglass thermal system insulation (TSI) on abandoned or active pipes.

Limited ACBM Inspection 3 dse Project No. 1749804

If the renovation plans will include disturbance of these other suspected ACBMs, this Limited ACBM Inspection will need to be reviewed to determine if additional bulk sample collection and analyses are required.

The JBSA Operations and Maintenance (O&M) Plan for the Building should be updated to include the findings in this Limited ACBM Inspection.

The Areas of ACBM identified are shown on Figure 2 in ATTACHMENT A.

Notes:

1. Consulting is estimated at 25% of the abatement cost plus $1,000 for Abatement Specifications preparation.

2. Only one Mobilization / Demobilization (Mob./DeMob.) would be incurred if more than one area was abated during a designated project.

3. Does not include flooring in areas of the 3rd Floor occupied by the Navy that were previously abated.

4. There are several abatement methods for Window Glazing Compound ACBM. The cost estimate provided most likely reflects the most expensive option. Considerable cost savings may be possible by coordinating the abatement contractor’s work with the work of the contractor that is selected to replace the windows.

Limited ACBM Inspection 4 dse Project No. 1749804

RECOMMENDATIONS

The Black Mastic and the Window Glazing Compound ACBM on wood windows identified in the Building should be removed and disposed if they will be disturbed during renovation activities. Multiple layers of ACBM and non-ACBM VCT on top of the Black Mastic ACBM must be handled as ACBM during the abatement.

In Texas, renovation of facility structures, such as the Building, is subject to the National Emission Standards for Hazardous Air Pollutants (NESHAP) rules and regulations, which are administered by the TDSHS. ACBM that will become Regulated Asbestos-containing Material (RACM) must be removed before it is disturbed during renovation activities.

The Texas Asbestos Hazard Protection Rules (TAHPR) do not apply to Federal facilities structures, such as the Building. However, to assure compliance with the NESHAP regulations and the Occupational Safety and Health Administration (OSHA) regulations for worker protection, abatement of ACBM in the Building should be performed by a TDSHS-Licensed Asbestos Abatement Contractor employing TDSHS-Registered Asbestos Abatement Workers monitored by a TDSHS-Licensed Air Monitoring Technician / Asbestos Project Manager supervised by a TDSHS-Licensed Individual Asbestos Consultant. In addition, the Individual Asbestos Consultant should prepare and sign Asbestos Abatement Specifications for the abatement project. The TDSHS must be notified in writing a minimum of ten working days before any identified ACBM is abated or before a facility structure is demolished, even if no ACBM was identified or the ACBM has been removed. All waste material generated during ACBM abatements must be loaded, transported and disposed following the applicable NESHAP, TDSHS and Texas Commission on Environmental Quality (TCEQ) rules and regulations.

Request for Information (RFI):

Records of all ACBM inspections and abatements previously performed in Building (including the Navy’s).

Records of ACBM inspections or abatements in buildings of similar design and age on the base.

Sample Description/Location Asbestos Content

These samples were analyzed by layers. The overall percent asbestos for the sample is reported when relevant. The EPA considers a material to be asbestos containing only if it contains greater than one percent asbestos by Calibrated Visual Area Estimation (CVAE). EPA regulations also indicate that Regulated Asbestos Containing Materials (RACM) – materials that are friable or may become friable – be further analyzed by point counting when the results indicate less than ten percent asbestos by CVAE. CatesLab utilizes CVAE on a routine basis and does not include point counting unless specifically requested by the client. The results may not be reproduced except in full.
