(1) TAB A - SF 1442. The SF 1442 shall be submitted fully completed. The offeror is cautioned that the

SF 1442 shall contain an original signature in block 30B of the form. Acknowledgment of amendments can be made on the face of the SF 1442 in block 19 or signed copies of the amendments can be included in this Section.

(2) TAB B - Section K (Representation, Certifications, and Other Statements of Offerors). The offeror shall ensure that Section K is submitted with all blocks in each certification/representation completed truthfully and completely.

(3) TAB C - Transmittal Summary Letter. A summary cover letter to the proposal shall provide the following: (1) the name, title and telephone number of the company/division point of contact, who is authorized to make decisions with respect to the proposal and obligate the company contractually, (2) Identify those individuals authorized to negotiate with the Government, (3) A list of the proposal package contents, and (4) A listing of subcontractors and/or teaming partners that the offeror historically uses when involved in contracts of this type or magnitude.

The Contracting Officer will review any mitigation plans submitted in response to this Request for

Proposal before the proposal is evaluated or considered for award. The Contracting Officer will determine whether there is an OCI issue based on the information submitted and if the mitigation plan resolved any OCI concerns. If the Contracting Officer determines that there is an OCI issue and that it cannot be mitigated, the Contracting Officer will inform the Offeror that the proposal will not be evaluated or considered for award. Statements that the offeror understands, can, or will comply with the SOW (including referenced publications, technical data, etc.); statements paraphrasing the SOW or parts thereof (including applicable publications, technical data, etc.); and phrases such as “standard procedures will be employed” or “well known techniques will be used,” etc., will be considered

W91247-15-R-0004 unacceptable. Offerors shall assume that the Government has no prior knowledge of their facilities and experience.

The Government will base its evaluation on the information presented in the offeror's proposal. Elaborate brochures or documentation, binding, detailed art work, or other embellishments are unnecessary and are not desired.

(2) The Mission Capability Volume shall, at a minimum, be prepared in a form consistent with the SOW and the evaluation criteria for award set forth in Section M of this solicitation. The section shall be prepared in an orderly format and in sufficient detail to enable the Government to make a thorough evaluation of the contractor’s technical competence, and ability to comply with the contract job requirements specified in the SOW. The offeror shall address as specifically as possible the actual methodology to be used for accomplishing the SOW requirements. The offeror shall describe the ability to manage and integrate subcontractors into their organization. The offeror shall describe the working relationship between the prime and their subcontractors at the corporate level and site level and how they will ensure adequate oversight of subcontractors when subcontractors are performing the work, to include performance monitoring and timely resolution of any issues that may arise.

(ii) The offeror shall describe in its Management Plan how the Contractor’s own workforce, and the required percentage (at a minimum at least 15% at all times) of “in house” work performance, will be integrated into the JOC program. The offeror shall describe how the efforts of this workforce will (or will not) integrate with subcontractor efforts. The offeror shall demonstrate the capability to meet, sustain, and grow the workforce, including skilled craft labor, required to successfully perform the JOC requirements. The offeror shall demonstrate how they will ramp up to support task orders that will be completed, particularly during busier times, such as the end of the fiscal year (July - September) or when subcontractors are unable to perform. The offeror shall describe in its Management Plan the division of authority, if any, between the home /corporate office and the site office and the offeror’s plan for daily and strategic management of resources. The staffing plan shall be outlined through a narrative and a diagrammed organizational chart. The staffing plan for the onsite staff shall include the identification of proposed key personnel positions with regards to this contract. The offeror’s narrative shall provide the proposed job title, specific duties, responsibilities, experience, education, and qualifications for each key personnel position identified. c. Volume III – Price Proposal (Coefficients). Part III must be clearly marked “VOLUME III – PRICE

PROPOSAL, RFP W9124E-16-R-0002.”

(1) TAB A, ID/IQ Price Proposal: Price proposal shall consist of Schedule B plus all costs included in development of the coefficients. Schedule B shall be submitted fully completed and error free. It shall contain the offeror’s prices (coefficients) for the established format in Schedule B. Coefficient means a numerical factor that represents costs (generally indirect costs) not considered to be included in the UPB prices. The coefficients are proposed as a percentage (in decimal format) increase or decrease to the UPB prices. For example, a coefficient of

1.2 would result in an additional 20 percent to the listed unit price; 0.9 would result in a 10 percent decrease of the listed prices, and 1.00 would leave the unit price as the proposed unit price. The offeror shall limit the Coefficients to two decimal places. The offeror shall propose Base Year and Option Year Coefficients. All components of price shall be included in the offeror’s coefficient, and shall be fully supported as no allowance for omitted cost will be made later. The offeror’s Coefficients shall be all inclusive and shall include, but not be limited to, the following costs:
