STATEMENT OF OBJECTIVES

1.0 Project Background
1.01 The existing elevators 1-8 in Building 61801 on Fort Huachuca need a complete modernization of components. A history of repeat elevator code issues reported by the maintenance staff and independent inspectors justify action to upgrade and modernize elevators 1-8. Elevators 1 through 4 (1-4) are passenger elevators and elevators 5 through 8 (5-8) are freight elevators.
2.0 Project Objectives
2.01 The contractor shall provide design/build services and furnish all labor, material, equipment, supervision, transportation, permits, fees, service order fees and costs, and licenses necessary to modernize elevators 1-8 in Building 61801 at Fort Huachuca, Sierra Vista, Arizona. A non-specific statement of work (SOW) for this project is provided in Paragraph 4.0 of this SOO.
The contractor shall prepare a more detailed scope of work outlining their Contract Execution
Overview to include their technical approach and tentative schedule. The contractor shall field verify the area of work before submitting their SOW and proposal. No proprietary items shall be installed by the contractor that would preclude others from performing maintenance and conducting repairs to the elevator systems beyond the warranty period for work order
AAA00011-4J. SOO Attachment A contains floor level drawings of Building 61801 that shows the locations of elevators 1-8. SOO Attachment B contains photographs of elevator 1-8 entrances. SOO Attachment C contains As-Built drawings for passenger and freight elevator machine rooms, hoistways, and pits. Attachment D contains an As Constructed drawing of
Building 61801 Ground Floor and Foundation Plan that indicates the seismic zone as of
August 1959. Attachment E contains lead based paint (LBP) and asbestos containing materials (ACM) analysis reports. The contractor shall follow Attachment F which contains
Fort Huachuca Energy Efficiency, Water Conservation Design/Construction Guidelines and
Operation Standards, Last Reviewed/Revised 1 August 2014. Attachment G which contains
Fort Huachuca Office of Fire Prevention Fire Alarm Information Sheet. Attachment H contains Information Notes.
2.02 The objective is to modernize passenger elevators 1-4 and freight elevators 5-8 based on current elevator standards. The contractor shall use and implement current codes for the modernization effort of elevators 1-8. A detailed work plan is required to be submitted as part of the contractor’s technical approach.
2.03 Elevators 1-4 shall be made to comply with Architectural Barriers Act (ABA) Standards (2015) requirements.
3.0 Proposal Requirements
3.01 Technical Approach. The contractor proposal technical approach section shall address each of the line items listed in Paragraph 4.0, and subparagraphs of Paragraph 4.0, of this SOO. The contractor shall follow UFC 3-490-06 dated 8 June 2018 for the minimum technical requirements and guidance for the modernization of the elevator systems, and associated supporting structure, building systems, and components. Contractor language in their technical approach section of the proposal must include a description of the steps and methods by which each item of the SOW section of this SOO will be completed. The description of the steps and methods must adhere to the details outlined in pertinent UFC and UFGS specifications (see listing in back of this SOO), such that a clear understanding of the requirements can be demonstrated to a competent qualified person. The contractor shall also follow UFC 3-600-01 Change 2 dated 25 March 2018 for fire protection.
3.02 Lockout/Tagout. The contractor shall detail their lockout/tagout procedures and how the contractor will control electric power to each area of contractor responsibility. The contractor will need to specify their level of lockout/tagout training.
3.03 Accident Prevention Plan. The Fort Huachuca Safety Office staff will need to review the contractor’s accident prevention plan 30 days prior to work starting.
3.04 Fall Protection Program. The contractor shall submit for review by the Fort Huachuca Safety
Office a Fall Protection Program and training records 30 days prior to work starting. Also, the
Safety Office staff will need to know if the contractor will perform self-rescue or depend on the Fort Huachuca fire department to provide rescue support in the event of an emergency.
The contractor will need to coordinate with the Fort Huachuca Fire Department 30 days prior to onsite work for the Fort Huachuca fire department to provide rescue support.
3.05 Accident Prevention Plan. The contractor shall identify in their proposal foundational documents for their Accident Prevention Plan (APP), which then is supplemented to identify unusual or unique project aspects or requirements to show that an effective accident prevention and safety system is established and maintained. Certain APP elements are standard across corporate/prime level which are verifiable. The contractor will need to submit an effective accident prevention and safety system for the government to review.
3.06 Work Impact. The contractor shall acknowledge in their proposal that their SOW will have to limit the effects of their work environment and conditions on other occupants of Building
61801. The contractor will need to identify impacts of their work environment on occupied floors. Specifics can be spelled out during development of various project submittals.
3.07 Hazardous Materials. The contractor will ensure during welding operations and/or if other hazardous materials (respiratory hazards) are present that all employees will use respiratory protection when mechanical ventilation cannot remove all known hazards within the workspace. The contractor will have to provide proof that employees are on a medical surveillance and respiratory protection program prior to the confined space permit being issued as well.
3.08 Hot Work Permit. The contractor shall request Hot Work Permits from the Fort Huachuca Fire
Prevention Office prior to welding, torch work, and other open flame work.
3.09 Materials/Quantities/Equipment. The Contractor Proposal Materials, QUANTITIES and
Equipment section must provide a clear and detailed list of all of the building materials AND
ESTIMATED QUANTITIES, as well as the equipment to be used in the execution of the project. These items must be presented in a bulleted list fashion such that a competent qualified person can understand what materials, ESTIMATED QUANTITIES and equipment will be used during the execution phase of the project.
3.09.01 Materials shall include items such as chemical or mechanical fasteners, hardware, devices, fixtures, valves, piping, sealants, structural members, electrical wiring and all other permanently installed materials at the project location.
3.09.02 Equipment shall consist of all tools, machinery, motorized or engineer driven equipment, and all other non-permanent items which will not be left on site.
3.09.03 The contractor shall follow Fort Huachuca Energy Efficiency, Water Conservation
Design/Construction Guidelines and Operation Standards, Last Reviewed/Revised 1
August 2014, Chapter 3A(8), in Attachment F that prohibits the installation of aluminum electrical wiring within buildings.
3.10 Labor Hours. The contractor proposal labor hours’ section must give a detailed breakdown of labor hours, to include an estimated number of hours assigned to each trade or supervisory position including design, such that a clear understanding of the division of labor can be determined. In addition, the total number of hours estimated for the project should be listed.
A full time qualified safety person will be on the project site and inspections by the competent safety person is also required on a periodic basis.
3.11 Tentative Schedule. The Contractor Proposal Tentative Schedule section must provide a detailed estimate of all activities to be conducted during the project period of performance.
Scheduled actions must be listed in a logical order to demonstrate a clear timeline of the proposed project activities. Provide the tentative schedule as a print out from Microsoft
Project. The schedule shall have an expected award date as the first scheduled event.
Manually choose the notice to proceed date ten calendar days. The tentative schedule will be submitted as a printout from Microsoft Project, or similar.
3.12 Weekly Progress Reports. Weekly progress reports shall be provided by the contractor.
3.13 Technical Requirement. The contractor will need to be able to place each elevator car out of service in order to inspect and to perform any necessary work in each respective elevator shaft and pit. The contractor shall only remove from service one set of passenger elevators so as to keep the only other set of passenger elevators in service.
3.14 Work Requirement. Elevators sets 1-2 and 3-4 shall not be removed simultaneously from service for a modernization effort. Elevators 5 and 6 shall not be removed simultaneously from service for a modernization effort, nor shall elevators 7 and 8 be removed simultaneously from service for a modernization effort. The contractor shall initially modernize elevators 3-4, 5, and 8 simultaneously and place into service.
3.15 Sprinkler Systems. Sprinkler systems are not installed for elevators 1-8 machine rooms and associated elevator shafts and pits. Sprinkler systems are not to be installed as part of this
SOO.
3.16 Exception. Elevators 1-4 do not meet the physical dimensions to satisfy emergency medical services and, therefore, the requirement has been waived.
4.0 Statement of Work
4.01 The following subparagraphs of Paragraph 4.01 are not detailed in scope; the contractor shall provide a more detailed and specific scope of all work for Elevators 1-8 that satisfies all paragraphs in this SOO Statement of Work. All elevator cars will remain within the respective elevator shafts. The contractor shall perform all work for elevators 1-8 to bring each respective elevator and the elevator system up to current code.
4.01.01 The contractor shall replace all elevator controls, hoisting machinery, and governors in the elevator machine rooms. The new elevator controls need to duplicate existing elevator operations. The contractor shall replace the guide rails, hoist cables, governor cables, and counter weight cables. The government user POC requests one set of passenger elevators be modernized first and then the second set of passenger elevators.
Passenger and freight elevators can be modernized simultaneously, though.
4.01.02 The contractor shall disassemble the existing hoisting machinery and remove the disassembled machinery through the machine room doorway. The contractor shall assemble the new elevator hoisting machinery inside each machine room under supervision and guidance of the hoisting machinery manufacturer representative so as to ensure proper assembly.
4.01.03 Integrate the new elevator controls with the existing Simplex addressable fire alarm control system. The contractor shall ensure all smoke and heat detectors are operational for the fire alarm test and replace detectors if not operational. If smoke and heat detectors are not already installed, the contractor shall install and make operational smoke and heat detectors in elevator cars, elevator shafts, elevator pits, and at elevator lobbies if required by code. Repair the two-way emergency communications equipment and communications wiring within elevator cars 1-4.
Replace the two-way emergency communications equipment and communications wiring within elevator cars 5-8 with the same make and model of two-way emergency communications equipment as currently installed in elevator cars 1-4. The two-way emergency communications per ASME A17.1-2016, Part 2.27.1, are already configured for each elevator car emergency telephone to contact the Fort Huachuca
Dispatch Center. Provide non-removable placards inside each car adjacent to the emergency communications equipment that identifies the building number and the elevator car number. Contractor will need to submit a fire alarm programming worksheet as a submittal in advance for Fort Huachuca Fire Prevention to review and approve. After receipt of approval of the fire alarm programming worksheet, the contractor will need to schedule a fire alarm demonstration that will be performed at the direction of a Fort Huachuca Fire Prevention officer. The contractor shall follow
Attachment G which contains Fort Huachuca Office of Fire Prevention Fire Alarm
Information Sheet and also to provide approved drawings of all fire alarm system work.
4.01.04 Elevators 1-4 shall be equipped and made operational for Firefighters’ Emergency
Operation (FEO) as required in UFC 3-490-06.
4.01.05 Elevator hoistway ventilation does not exist for elevators 1-8 and hoistway ventilation construction is not part of this contract effort.
4.01.06 Replace existing luminaire fixtures in the machine rooms and within each elevator car with light emitting diode (LED) luminaire fixtures, and associated conduit, wires, and switches. Follow lighting and receptacle requirements as stated in UFC 3-490-06, Chapter 6-1.3.
4.01.07 Attachment E contains LBP and ACM analysis reports. LBP and ACM samples were taken from only specific machine rooms and specific elevators. Therefore, all positive
(POS) results for LBP and for ACM shall apply to all elevator machine rooms and for elevator cars 1-8, not just the specific elevator machine rooms tested and the specific elevator cars from which samples were obtained and tested. The contractor shall remove and dispose of all LBP and ACM in accordance with hazmat rules and regulations while ensuring that the contractor follows their construction schedule. The contractor shall provide all labor, material, equipment, supervision, transportation, permits, fees, licenses necessary to perform LBP removal and ACM abatement. The contractor shall remove all LBP and ACM that has been identified in the LBP and
ACM analysis reports in Attachment E. The contractor shall provide all LBP and
ACM disposal reports as stated elsewhere in this SOO to the DPW Construction COR.
4.01.08 Replace all existing cloth covered electrical cables for supply and backup electrical feeds. A government funded ACM analysis report of the cloth covered electrical cables is included in Attachment E where ACM analysis results are negative.
4.01.09 Replace electrical disconnect switches for each elevator machine room along with damaged or underrated upstream and downstream electrical cables, conduits, and associated hardware.
4.01.10 Replace electrical safety switches for the hoisting equipment and machinery along with damaged or underrated upstream and downstream electrical cables, conduits, and associated hardware.
4.01.11 All equipment within the machine rooms shall have engraved brass labels screwed or riveted into place. The labels shall be made of brass identifying the name of equipment, voltage, phase, frequency, DC or AC, equipment number designation, fuse sizes and types, transformer identification, alarm system junction boxes, safety switches, and transfer switches. Include on the engraved labels the respective elevator number for that equipment dedicated to the elevator.
4.01.12 Remove the fan coil units in the elevator machine rooms. Remove the associated thermostats, associated piping, associated electrical cables, and associated conduits of each fan coil unit from each elevator machine room. Remove associated piping, associated electrical cables, and associated conduits to the respective sources. Follow
UFC 3-490-06, Chapter 5-1 and associated subchapters of Chapter 5-1, to install and make operational an independent, dedicated HVAC system for each elevator machine room. The temperature and relative humidity ranges stated in UFC 3-490-06 shall be met from each new HVAC system. The contractor shall follow Fort Huachuca Energy
Efficiency, Water Conservation Design/Construction Guidelines and Operation
Standards, Last Reviewed/Revised 1 August 2014, Chapter 3C(13), in Attachment F for installation of HVAC equipment to use direct digital control, open protocol
LonWorks®. Fort Huachuca does not have a post-wide EMCS.
4.01.13 The machine room floors shall be stripped, patched, sealed, and coated with epoxy.
4.01.14 Replace control panels in elevator cars 1-4. Replace all controls, safeties, and equipment that allows the elevator to level before door operation. Repair all elevator doors for proper operation.
4.01.15 An existing fault condition is identified that allows the cab and hoistway doors to open completely if the car is out of range and not matching the landing. The contractor shall replace the door sensors to detect the out of range condition as required in ASME
A17.1-2016. Also, prevent the doors from being pushed open without the proper key.
4.01.16 OPTION: The contractor shall include the replacement of the doors for freight elevators 5-8 as an option.
4.01.17 Replace hoistway door unlocking devices to conform to ASME A17.1-2016, Part
2.12.6. Hoistway pull chain boxes shall not be accessible to the public.
4.01.18 Replace signal devices, and floor level selector and position indicator panels. Repair wall openings and elevator car openings left from replacement of signal devices, and floor level selector and position indicator panels. Replace labels of each exterior elevator floor level entrance with the respective floor level and elevator number.
4.01.19 The contractor shall replace the traveling cables for each elevator.
4.01.20 The contractor shall replace or install luminaire fixtures in each elevator pit with LED luminaire fixtures with guards. The new lighting in each elevator pit shall meet minimum requirements of 108 lux (10 foot-candles) of light at the pit floor in all areas of the pit as stated in UFC 3-490-06, Chapter 6-2.1. None of the elevator pits have sump pumps and sump pumps are not part of this contract effort.
4.01.21 Complete repair and renovation of any areas in the elevators shafts and pits that do not meet fire codes shall be performed by the contractor.
4.01.22 All elevator machine room entrance doors, door frames, and door locking mechanisms shall be made code compliant.
4.01.23 The contractor shall provide and install a Firefighters’ Emergency Operation (FEO) key box as required in UFC 3-490-06, Chapter 3-5.1, at the 2nd Floor. If the FEO key box cannot be flush mounted as required in UFC 3-490-06, the Fort Huachuca fire department allows the key box to be surface mounted. The contractor shall order and install KnoxBox® Model 3261 per the Fort Huachuca fire department's ordering instructions to be used as the FEO key box. The government will provide the ordering instructions to the contractor after contract award.
4.01.24 Elevator inspections per ASME A17.2-2017 by a Qualified Elevator Inspector (QEI) need to be provided by the contractor. Elevator inspection reports need to be submitted to the government as submittals on each repaired elevator per ASME A17.2-2017.
4.01.25 Provide a minimum one-year warranty for each elevator system that has been modernized, renovated, and repaired. Provide 24-hour emergency service with two hour response time during the one-year warranty period for each elevator system that has been modernized, renovated, and repaired as part of this SOO as stated in UFGS
14 21 23, Chapter 1.4, and UFC 3-490-06, Chapter 2-4.2.2.
4.01.26 The contractor shall provide As-Built drawings as required in SOO Paragraphs
17.02.03.02 and 17.06.
4.01.27 Contractor shall properly dispose and recycle all removed material and waste material off Fort Huachuca. Fort Huachuca does not have recycling drop areas. Provide reports indicating the tonnage of disposed materials and of recycled materials.
5.0 Schedule
5.01 Notice to Proceed (NTP) for the project shall be issued no later than 10 calendar days after award. The duration of the Period of Performance (POP) is 255 working days. The project shall be 100% complete and ready for final inspection no later than 255 working days after the
NTP-construction.
5.02 The contractor’s proposed schedule shall be submitted with the proposal. A revised schedule shall be submitted at the time of the preconstruction conference for approval as a Microsoft
Project file, or similar. The schedule shall be in the form of a progress chart of suitable scale to indicate the percentage of work scheduled for completion at any given date during the period. The schedule shall take into consideration anticipated adverse weather that could preclude progress, long-lead equipment or materials critical to the completion of the project, review periods for submittal items requiring Government review and approval, air quality control permitting process and other events and milestones as necessary. The schedule shall be dated and signed by the preparer. The schedule shall be updated, revised and submitted for approval each time in the event that work falls behind. The revision number shall be shown on any re-submittal. The intent is to show in the event that construction is behind just how the contractor intends to finish within the approved contract time period. Also, an outlay schedule shall be submitted at the time of the pre-construction meeting. The outlay schedule shall be in the form of a plotted S curve. This schedule shall be revised in the event that construction falls behind and shall be signed and dated by the preparer.
6.0 Occupancy
6.01 All floors of Building 61801 affected by the scope of this contract will be occupied. The contractor shall only remove from service one set of passenger elevators at a time for modernization so as to keep the only other set of passenger elevators in service.
7.0 Material, Standards, Codes, Regulations, and Workmanship (FAR 52.236-5, Material and
Workmanship)
7.01 All materials and equipment shall be new unless otherwise specified. All work shall be completed in accordance with the plans and specifications, applicable industry standards, UFC, UFGS, NFPA, ASHRAE, NEC, SMACNA, NEBB, AABC, MUTCD, SDDCTEA, and
AASHTO standards; federal, state and local codes, regulations of EPA, OSHA, NEPA, and
MSHA, if required; Network Enterprise Center (NEC) and SSVEC requirements; Fort
Huachuca Energy Policy and Army Energy and Water Management Program. All government permits, certifications, lead paint and asbestos analyses, if required, etc. shall be obtained by the Contractor prior to commencement of work or prior to beginning construction and are the specific responsibility of the Contractor (Federal, State and local governments). Air quality control permitting efforts must be coordinated with DPW ENRD Air Quality Program prior to the start of any construction and prior to making any irrevocable decisions (i.e. purchasing equipment) related to equipment, activities or processes that have the potential to emit air pollutants such as combustion exhaust, dust, asbestos, evaporative emissions, refrigerant, etc.
All certifications are the responsibility of the contractor (potable water health certifications, gas weld certifications, refrigerant technician certifications, asbestos removal certifications, etc.) and must be provided to the COR prior to final payment.
7.02 The contractor shall obtain specific approval for all materials and equipment that vary from the standards established herein. All work shall be performed in a skillful and workmanlike manner to current applicable industry standards and in accordance with manufacturer instructions. Work requiring certified craftsmen shall be performed by certified personnel.
Proof of such certification, training, or other qualifications shall be made available to the
Government upon request. If the manufacturer of any product used in accomplishing this project has published an installation and/or operation manual, such manual shall be followed in its entirety. If any organization such as, but not limited to, APA or ANSI publish any construction guide, it will be followed as applicable to this project. If the manufacturer’s manual deviates from any established code, regulation, local requirement or this scope of work, such deviation shall be brought to the attention of the government representative before award, for written resolution. A revised scope of work will be issued to specify such resolution and incorporated in any resulting award.
7.03 Commissioning of major systems (e.g., HVAC, fire protection, etc.) is required, unless otherwise noted.
7.04 The contractor shall comply with Federal directives by recycling at least 60% of Construction waste.
7.05 The contractor shall comply with all Federal, State and Army Environmental requirements.
Ensure Contractor responsibilities and operating limitations are noted in proposal [i.e. Storm
Water Pollution Prevention Plan (SWPPP) shall be initiated and paid for by the contractor, contractor must conduct and record dust control for all dust-generating activities, contractor cannot use photo-chemically-reactive solvents in the application of architectural coating, etc.].
Contractor shall keep all required records as stated in the following (and any others as required by Federal/State authorities) and submit to the government.
7.06 Air, water, asbestos, lead based paint and hazardous waste, among others, need to be covered in any construction, demolition or renovation contract; thus, the contractor is required to maintain all record-keeping and submit it to the appropriate government entity at the appropriate time, with a copy sent to the DPW COR.
7.07 Work on historic buildings must follow the Secretary of Interior’s Standards for the Treatment of Historic Properties, unless otherwise specified. Repair of historic fabric of the building must be completed with in-kind materials and color. If repair is not possible, replacement must be with in-kind material and color to match the historic fabric to the extent possible. All historic fabric of the building is Government property and those items that cannot be reused
(such as doors, windows, door/window hardware, hand rails, etc.) shall be returned to the
Government after removal from the building.
7.08 Environmental work statement clause for all exterior work (SWPPP)
7.08.01 Through the Arizona Pollutant Discharge Elimination System (AZPDES) permitting program, the Arizona Department of Environmental Quality (ADEQ) regulates the discharge of storm water from construction sites throughout the state including those on military installations.
7.08.01.01 "Construction" refers to actions that result in a disturbance of the land, including clearing, grading, excavating, and other similar activities. It also includes "construction-related activities," areas that support the construction project such as stockpiles, borrow areas, concrete truck washouts, fueling areas, material storage areas and equipment storage areas.
7.08.01.02 Stockpiles, borrow pits, etc. need to be weed-free areas. Contractors shall use certified weed-free fill or contractors must sterilize the fill before bringing it onto the installation.
7.08.02 The AZPDES Storm water Program requires operators of construction sites one acre or larger (including smaller sites that are part of a larger common plan of development) to obtain authorization to discharge storm water under an AZPDES construction storm water permit. The Contractor is responsible for determining whether its construction site will exceed this minimum threshold with consideration of the definition provided above and if so, obtaining coverage under an AZPDES construction storm water permit.
7.08.03 Obtaining coverage under an AZPDES permit entails preparing and implementing a
Storm Water Pollution Prevention Plan (SWPPP) in accordance with ADEQ requirements and submitting a Notice of Intent (NOI) for coverage under the applicable AZPDES general permit. Once ADEQ receives the NOI, ADEQ then issues an authorization certificate to the Contractor. If a contractor has a sub-contractor working together on the same project, then the contractor and sub-contractor(s) will have to submit a NOI for the same job and separate authorization certificates will be issued to each party.
7.08.04 If the Contractor determines that the project will not exceed the minimum threshold and does not submit an NOI and it is later determined that the threshold is exceeded, the Contractor will be required to stop all work until they have completed the above process. If the appropriate documentation is not provided to the Government prior to the start of construction or once the threshold is exceeded, the Contracting Officer will be notified of the failure to comply with a contract requirement, the ADEQ
Enforcement Section will be notified, and other appropriate action will be taken to resolve the matter. The Contractor and any sub-contractor(s) will be liable for any fines levied by ADEQ.
7.08.05 A critical component of the SWPPP is final site stabilization. This generally involves restoring the disturbed area to at least 70% of its former vegetative state. After implementing all aspects of the SWPPP, to include final site stabilization, the contractor submits a Notice of Termination to ADEQ, ending coverage under the
AZPDES permit.
7.08.06 The government highly recommends that contractors visit the ADEQ website at https://www.azdeq.gov, under water programs and type in "Construction
Permits/SWPPP" in the search box for all relevant information on obtaining coverage.
7.08.07 It is the responsibility of the Contractor to develop a SWPPP that meets applicable requirements; the Government will not review or approve the Contractor SWPPP for compliance with applicable laws. The Contractor and any sub-contractor(s) shall provide a copy of the ADEQ Notice of Intent (NOI) and a copy of the SWPPP to the
Government employee performing construction oversight prior to any construction or construction-related activities as defined above. Contractor may request a courtesy inspection by the Government of their site as it conforms to their SWPPP.
8.0 Clean Up (FAR 52.236-12, Cleaning Up)
8.01 The contractor shall at all times keep the work area, including storage areas, free from accumulation of waste materials. All trash will be removed from the site on a daily basis.
Upon completion of the project, all equipment, materials and other items associated with the project shall be removed from the project site and the site shall be left clean. Any and all waste products created during performance of the work shall be removed from the project site, packaged in an environmentally approved manner and disposed of OFF-POST at an environmentally approved disposal site. It is the contractor’s responsibility to properly maintain the site during the life of the contract and the warranty. Any damage caused by nature beyond the contractor’s control will be addressed as a separate issue.
9.0 Utilities
9.01 Where available, onsite utilities will be provided to the contractor at no expense. These utilities shall be used only in the performance and support of the project.
9.02 Any temporary connection to the utility system will be coordinated with the contracting officer’s representative prior to hook-up. The contractor shall complete the temporary connection and all temporary electrical work in accordance with applicable codes and standards. Temporary electrical wiring and devices shall comply with the current National
Electrical Code. Temporary wiring and devices shall be removed at project completion. All temporary electrical work shall be at the contractor’s expense.
9.03 Expansions/tie-ins to the potable water system and/or sewer collection system must be coordinated with the DPW. Some projects for expanding the potable water system and/or the sewer collection system will require an ADEQ 4.01 permit/NOI, prior to breaking ground.
10.0 Storage Areas/Laydown Yards
10.01 The contractor and the Contracting Officer’s Representative will agree on temporary storage areas and lay down yards in the general location of the project. Request for proposed storage area shall be submitted with the proposal documents. The contractor shall be responsible for the safety, security and maintenance associated with storage areas. Storage areas shall be kept clean and in a neat and organized appearance at all times. If the storage area is not maintained, the Contracting Officer may direct the contractor to store materials off the installation. Storage areas and lay down yards shall be returned to the existing stable state, which may require some rehabilitation of the land, like re-seeding with native grass mixtures.
11.0 DD Form 1354 Preparation
11.01 The contractor shall submit a draft DD Form 1354 as a submittal during the construction phase. The contractor shall submit a final DD Form 1354 to the Government as a submittal near completion of the project but prior to final inspection and acceptance. The contractor shall reference UFC 1-300-08 in preparing the draft DD Form 1354 and the final DD Form
1354. The contractor may obtain a DD Form 1354 information worksheet from the DOC CS or the DPW COR.
12.0 Controlled Areas
12.01 The contractor will be escorted for any and all work performed in controlled or secured areas.
The occupant of the facility requiring the work will provide an escort for the contractor.
13.0 Waste Disposal and Borrow Pits
13.01 The contractor shall comply with the Fort Huachuca Waste Disposal/Borrow Pit Plan and submit a project specific Environmental Protection/Borrow Pit Plan.
14.0 Contractor Quality Control (CQC)
14.01 The contractor is responsible for quality control and shall establish and maintain an effective quality control system and shall include the following:
14.01.01 The Quality control system shall consist of plans, procedures, and organization necessary to produce an end product that complies with the specified requirements of this project. The system shall cover all construction activities, both on site and off site, and shall be coordinated with the proposed construction schedule.
14.01.02 The contractor shall submit to the COR for review and approval a Contractor Quality
Control Plan proposed to implement the specified requirements. The plan shall identify personnel, procedures, controls, instructions, tests, records, and forms to be used. Construction will be permitted to begin only after acceptance of the CQC plan.
The plan shall include, as a minimum, the following to cover all construction operations and features.
14.01.02.01 A description of the CQC organization and lines of authority (e.g., Organization
Chart).
14.01.02.02 Names, qualifications, duties, responsibilities, and authorities of CQC personnel.
14.01.02.03 Procedures for scheduling, reviewing, certifying, and managing submittals.
14.01.02.04 Procedures for conducting CQC inspections to include routine inspections, follow-up inspections on deficient work, pre-final inspections, and final inspections.
14.01.02.05 Procedures for tracking construction deficiencies from the time of identification through acceptable corrective action (e.g., Deficiency Tracking Log).
14.01.02.06 Documentation and reporting procedures, including proposed reporting format.
14.01.03 The contractor shall maintain current records providing factual evidence that the required QC activities and/or tests have been performed. These report records shall be furnished to the Government daily and within 24 hours after the date covered by the report (except when no work is performed; then one report shall be submitted for every seven days of no work). The reports shall include the work of sub-contractors and suppliers and shall be on an acceptable form that includes, as a minimum, the following information:
14.01.03.01 Contractor/subcontractor and their area of responsibility.
14.01.03.02 Work performed each day, giving location, description, and by whom.
14.01.03.03 Weather conditions encountered.
14.01.03.04 Any delays encountered.
14.01.03.05 Test and/or control activities performed with results and references to specifications/drawings.
14.01.03.06 Materials received at the site with statement as to quantity and quality, acceptability, and storage.
14.01.03.07 Submittals and deliverables reviewed.
14.01.03.08 Job safety evaluations stating what was checked, results, and instructions or corrective actions taken.
14.01.03.09 Instructions given or received and conflicts in plans and or specifications.
14.01.03.10 Contractor’s verification statement that equipment and materials incorporated into the work and the workmanship comply with the specified requirements.
14.01.03.11 Air Quality Control – the contractor shall comply with all requirements of the
Fort Huachuca Air Quality Control Permit and other Clean Air Act regulations by submitting all required records, including but not limited to the following, as applicable, in accordance with applicable pre-construction/construction/post-construction timelines, to the DPW ENRD Air Quality Program:
14.01.03.11.01 Detailed equipment specifications and activity/process descriptions
14.01.03.11.02 Documentation of equipment that was installed in accordance with manufacturer instruction
14.01.03.11.03 Specifications of removed equipment
14.01.03.11.04 Dust control logs
14.01.03.11.05 Fuel certifications
14.01.03.11.06 Report of run times occurring during equipment installation (i.e.
generator hour meter readings)
14.01.03.11.07 EPA engine certifications
14.01.03.11.08 Records of Asbestos NESHAP Notification for Renovation and
Demolition Activities forms filed with the appropriate state/county entity and all supporting documentation
14.01.03.11.09 Miscellaneous chemical usage
15.0 Inspection
15.01 All work is subject to inspection and testing by the Government at all places and at all reasonable times before acceptance, in accordance with FAR 52.246-12, Inspection of
Construction. Government inspections and tests do not relieve the contractor of the responsibility for providing adequate quality control measures, nor does it constitute or imply
Government acceptance of the work. The presence or absence of a Government inspector does not relieve the contractor of complying with the specified requirements, nor is the
Government inspector authorized to change any term or condition of the specification without the Contracting Officer’s written authorization.
16.0 Accident Prevention
16.01 The contractor shall comply with 29 CFR part 1926, 29 CFR part 1910, all state and local occupational safety and health requirements, and the requirements of EM 385-1-1, U.S. Army
Corps of Engineers Safety and Health Requirements Manual.
16.02 The contractor is responsible for accident prevention and safety and shall establish and maintain an effective system.
16.03 The plan shall be developed by a “qualified person” (EM 385-1-1, A-1, 1.a.) and be signed by a representative of the prime contractor’s “corporate office authorized to obligate the company.” The plan shall meet the minimum basic outline requirements contained in EM
385-1-1, Appendix A. The plan shall be job-specific and shall include work to be performed by subcontractors and measures to be taken by the contractor to control hazards associated with materials, services, or equipment provided by suppliers. The accident prevention plan shall include procedures for frequent safety inspections, conducted by competent persons, of the work sites, material, and equipment to ensure compliance with the plan and other referenced standards. Unless otherwise approved, Quality Control personnel shall conduct and document daily safety inspections.
17.0 Submittals
17.01 The contractor shall provide information and/or submit samples of materials that the contractor contemplates incorporating in the work. All submittals shall be at the contractor’s expense.
17.02 The following submittal items are required for this project:
17.02.01 Submittals required prior to start of construction (normally at the PRECON) shall include:
17.02.01.01 Construction schedule
17.02.01.02 Project designs, specifications, design analysis
17.02.01.03 Shop drawings
17.02.01.04 Submittal Register (ENG FORM 4025 and 4288)
17.02.01.05 Catalog cuts (equipment and materials)
17.02.01.06 Finish material samples and/or product data
17.02.01.07 Sub-contractor personnel list (POC with phone number)
17.02.01.08 Quality Control, Accident Prevention, Disposal/Borrow Pit Plans
17.02.01.09 Permits and certifications (Dig Permit, SWPPP/NOIs, 4.01 permits, Hot Work, etc.)
17.02.01.10 Air Quality Control and other Environmental submittals as appropriate

17.02.03.09 Maintenance paint
17.02.03.10 Air Quality Control and other Environmental submittals as appropriate
(construction water used, Dust control records, etc.)
17.03 If any of the equipment/material submittals are disapproved, the contractor shall resubmit the disapproved submittals for review and comment within five (5) working days of notification of their disapproval. This process shall continue until all equipment/materials are approved by the government.
17.04 If contractor uses or installs materials or equipment without approval, he does so at the risk of subsequent rejection, and replacement at his cost.
17.05 Final Inspection and Acceptance
17.06 Final inspection and acceptance shall be conducted after the pre-final inspection is completed by the CQC representative and the pre-final inspection punch list items are completed by the contractor. The CQC representative compiles the pre-final punch list immediately after the work is completed. The Government’s PM/COR/QA representative will sign for completion documents after the final inspection and its punch list items are corrected; and upon receipt of acceptable As-Built Drawings and any delayed work (seeding prior to rainy season, Air
Conditioning Test, Adjustment and Balancing in summer months, etc.).
18.0 Warranty Requirements
18.01 All work shall be covered under the one-year warranty of construction (FAR 52.246-21, Warranty of Construction).
18.02 In addition to the one-year warranty of construction, the contractor shall obtain all warranties that would normally be given in commercial practice and, upon final acceptance of the work
(or before), provide a list to the Government of each warranted equipment item, feature of construction, or system. The list shall indicate the following:
18.02.01 Name of item.
18.02.02 Model and Serial Number.
18.02.03 Installed location.
18.02.04 Name and phone numbers of manufacturers or suppliers.
18.02.05 Names, addresses, and phone numbers of sources of spare parts.
18.02.06 Warranties and terms of warranty. Items with warranties extending beyond the one-year warranty shall be indicated with separate warranty expiration dates.
18.02.07 Start date and duration of warranty period.
18.02.08 Summary of maintenance procedures required to continue the warranty in force with cross-reference to specific pertinent operation and maintenance manuals as applicable.
18.03 The list shall include the status of delivery of all Certificates of Warranty for extended warranty items, to include roofs, HVAC balancing, pumps, motors, transformers, and for all commissioned systems such as fire protection and alarm systems, sprinkler systems, lightning protection systems, etc.
18.04 At approximately four and nine months after acceptance the Government may conduct warranty inspections. The inspections will be scheduled by the Government COR/Project
Manager and conducted with the user, and, if possible, contractor personnel attending. A list of deficiencies existing at the time will be compiled and furnished to the Contractor for correction.
