About this file
This is a Performance Assessment Questionnaire used by the U.S. Army United States Military Academy to evaluate past performance of offerors competing for Request for Quote (RFQ) W911SD-26-Q-A118 for Harbor Q-Boat LRC. The questionnaire is sent to references identified by competing contractors and must be completed and returned to the West Point Contracting Office within 15 days of receipt, with responses submitted to ignacio.j.cordova2.civ@army.mil.
The questionnaire is structured in three parts. Part I requires the offeror to provide contract identification details including contractor name, address, program title, contract number, contract type, prime contractor information, award and completion dates, nature of work, and total contract value, along with identification of the offeror's representative. Part II, completed by the point of contact/respondent, contains five evaluation criteria with mandatory rationale statements for each response: (A) Compliance of Products, Services, Documents, and Related Deliverables to Specification Requirements and Standards of Good Workmanship; (B) Effectiveness of Project Management including subcontractor use and control; (C) Timeliness of Performance for Services and Product Deliverables; (D) Effectiveness in Forecasting and Controlling Estimated Costs (for cost reimbursement contracts only); and (E) Commitment to Customer Satisfaction and Business-like Concern for Customers' Interest. Each criterion is rated using one of four confidence levels: Substantial Confidence (exceeds many requirements with few problems), Satisfactory Confidence (meets requirements, exceeds some with minor problems), Limited Confidence (fails to meet some requirements with serious problems), or No Confidence (fails to meet most requirements with ineffective corrective actions). Part II also includes sections for general comments, identification of other relevant past efforts and points of contact, and respondent identification information. Part III directs return of the completed questionnaire to the identified Contracting Officer.

Work was accomplished with some minor problems for which corrective actions taken by the contractor were effective. The offeror has successfully performed the required effort.
