SCOPE OF WORK

CA010 REPAIR POV AND MEP PARKING

EL Monte USARC 1200 Potrero Ave

South El Monte, CA 91733

1. GENERAL PRE-AWARD:

1.1 The intent of this project is to provide the Government with a complete and usable product at the completion.

1.2 The Contractor shall furnish and install all plant, labor, materials; tools, equipment, and supervision necessary to provide a complete and usable project per the Scope of Work (SOW), Plans, Specifications and manufacturers recommendations. All work shall be free from defects.

1.3 The Plans and Specifications are to prescribe the details for the construction and completion of the SOW, which the Contractor undertakes to perform in accordance with the terms of the contract.

1.4 The Contractor shall visit the work site to verify the site condition. If the Contractor discovers any errors or omissions in the SOW, Plans or Specifications, the Contractor shall immediately call it to the attention of the Contracting Officer (KO) and the Contracting Officer’s Representative (COR).

1.5 Where the Specifications and Plans describe portions of the work in general terms, but not in complete detail, it is understood that all work and materials will be of the highest standard using generally accepted industry practices.

1.6 The Contractor shall not interrupt the service function or disturb the support of any utilities without prior coordination with the COR. If any utilities are disturbed or damaged, the Contractor shall bear costs of repair or replacement.

1.7 The Contractor shall be responsible for and repair any damage to the grounds and structures caused by the Contractor or his/her sub-contractors during the performance of this project.

1.8 Any said work in the Plans and Specifications and not in the written SOW or in the SOW and not in the Plans or Specifications shall be as though shown or mentioned in both.

1.9 The Contractor shall verify all dimensions, site conditions, and any other items required to install a complete and ready to use product, prior to submitting a bid proposal for construction.

1.10 The Contractor shall have at least one qualified superintendent and competent alternate that is capable of reading, writing, and conversing fluently in the English language. The superintendent shall be on the job-site at all times during the performance of contract work. The Contracting Officer may request proof of the superintendent’s qualifications at any point during the course of this contract if the performance of the superintendent is in question.

1.11 The Contractor shall perform all the services of this contract during the normal operating hours of 7:30 A.M. to 4:30 P.M. (0730 to 1630) Monday-Friday. Facilities will be closed on all Federal Holidays.

1.11.1 Federal Holidays are as follows;

New Year’s Day – January 1 Martin Luther King Jr. Birthday – third Monday of January Washington’s Birthday – third Monday of February Memorial Day – final Monday of May Independence Day – July 4 Labor Day – first Monday of September Columbus Day – second Monday of October Veterans Day – November 11 Thanksgiving Day – fourth Thursday of November Christmas Day – December 25

1.11.2 Actual day observed for the following holidays is subject to change when the holiday date falls on a weekend; New Year’s Day, Independence Day, Veterans Day and Christmas Day.

1.12 The Contractor shall provide signs, barriers, and barricades to provide a safe working area. Requirements of Occupational Safety and Health Act, latest edition and U.S. Army Corps of Engineers "Safety and Health Requirements Manual", latest edition, shall be strictly enforced and adhered to at all times at the job site.

1.13 The Contractor shall comply with Army Regulation (AR) 600-63, Army Health Promotion, for controlling smoking in Government buildings and facilities. In general NO SMOKING is allowed within building or within 50’ of any exit or door. Facility personnel can direct personnel to authorized smoking areas.

1.14 The Contractor shall, without additional expense to the Government, be responsible for obtaining any necessary licenses and permits, and for complying with any Federal, State, and municipal laws, codes, and regulations applicable to the performance of the work. This can include storm water permits, US Army Corps of Engineers permits, state water resources permits, US Fish and Wildlife permits, etc.

1.15 Antiterrorism (AT) Level I Training. All contractor employees, to include subcontractor employees, requiring access Army installations, facilities and controlled access areas shall complete AT Level I awareness training within 30 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR or to the contracting officer, if a COR is not assigned, within 30 calendar days of contract award or prior to submission of final invoice and within 45 calendar days after completion of training by all employees and subcontractor personnel. AT level I awareness training is available at the following website: http://jko.jten.mil.

1.16 iWATCH Training. The contractor and all associated sub-contractors shall brief all employees on the local iWATCH program (training standards provided by the requiring activity Antiterrorism Officer. This local developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the COR. This training shall be completed within 30 calendar days of contract award or prior to submission of final invoice and within 30 calendar days of new employees commencing performance with the results reported to the COR no later than 45 calendar days after contract award.

1.17 Access and general protection/security policy and procedures. Contractor and all associated sub-contractors employees shall provide all information required for background checks to meet installation access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9, Personal Identity Verification of Contractor Personnel) as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, shall the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.

1.17.1 Contractors requiring Common Access Card (CAC). Before CAC issuance, the contractor employee requires, at a minimum, a favorably adjudicated National Agency Check with Inquiries (NACI) or an equivalent or higher investigation in accordance with Army Directive 2014-05. The contractor employee will be issued a CAC only if duties involve one of the following: (1) Both physical access to a Department of Defense (DoD) facility and access, via logon, to DoD networks on-site or remotely; (2) Remote access, via logon, to a DoD network using DoD-approved remote access procedures; or (3) Physical access to multiple DoD facilities or multiple non-DoD federally controlled facilities on behalf of the DoD on a recurring basis for a period of 6 months or more. At the discretion of the sponsoring activity, an initial CAC may be issued based on a favorable review of the FBI fingerprint check and a successfully scheduled NACI at the Office of Personnel Management.

1.17.2 For contractors that are not eligible for CAC, but require access to a DoD facility or installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR 190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.

1.18 The contractor employees must be adequately trained in the handling and disposal of hazardous waste, in particular, lead paint and asbestos. Hazardous materials must be handled and disposed of appropriately by the contractor. Disposal is at contractor’s expense. The contractor must comply with all local, state, and federal environmental laws, regulations, and policies prior to final approval of work completed. National Environmental Policy Act (NEPA) documents must be completed prior to award of the contract or notice to proceed.

2. GENERAL POST AWARD:

2.1 Where any conflict occurs between the requirements of federal, state, and local laws, codes, ordinances, rules and regulations, the most stringent shall govern.

2.2 The Contractor shall, at all times, keep the premises free from accumulation of waste material and rubbish caused by his work, or his subcontractors. At the completion of work, the Contractor shall thoroughly clean the work site. All debris, rubbish, and waste material shall be removed from the property at the Contractor’s expense and disposed of in an official disposal site.

2.3 The Contractor shall make no changes or deviate from the approved design of this project without written permission from the KO. In the case where a change is required, the Contractor will notify the COR of the requirement and shall not proceed with the change until such time as a modification to the contract is issued by the KO.

2.4 The Contractor shall submit a written construction schedule of work for this project to the KO and COR prior to the start of the project.

2.5 The Contractor shall be responsible for the conditions of the job site, including safety of all persons, and property during performance of the work. Safety shall conform to all applicable Federal, State, local laws, codes and regulations established by the El Monte, Los Angeles, CA Division of Safety.

2.6 All work shall be in compliance with the current applicable federal, state and city codes.

2.7 All work shall be completed within 120 days upon receipt of Notice To Proceed (NTP).

3. SUBMITTALS:

3.0 Prior to ordering any materials, the Contractor shall submit to the COR for approval submittals for all new items being installed on this project. Submittals may be cut sheets, samples, spec sheet, so long as it is clear and easily readable. Submittals must be e-mailed, faxed or sent directly to the office of the COR using Engineer Form 4025-R, dated March 2012, as the coversheet. Where variations in color, finish, pattern, or texture are unavoidable due to nature of the materials, submit sets of samples of not less than three units showing extremes and middle of range. Mark each unit to describe its relation to the range of the variation.

3.4 All materials used for this project will be subject to UFC 1-200-02 Section 2-6.1 for recycled content, biologically-based products, other green products, and ozone depleting substances, and Section 2-5.3.3 for low-emitting materials. Projects will also comply with applicable requirements for the purchase of water efficient (e.g., Water Sense), Energy Star or FEMP-designated (or an "A" or better European Union Energy Label, EU energy efficiency class), and Electronic Product Environmental Assessment Tool (EPEAT) designated products. In addition, project materials will comply with the requirements in ASHRAE 189.1-2014 Section 9.4.1.2 (Regional Materials). Submittal requirements packages will clearly outline the applicable sustainable materials being used. The owner reserves the right to reject any product that does not meet the requirements of the specification list in the SOW.

3.5 The contractor shall maintain proper construction waste management. Waste management must maximize the materials to be salvaged, recycled, or reused. All materials removed shall become the property of the contractor, and be disposed of daily. The contractor shall provide reports of all materials salvaged, recycled or reused.

4. SITE WORK:

4.1 OVERVIEW

4.1.1 This is intended to be a Government-Provided Design/Design-Build Project. The government shall provide a 35% design package which includes drawings and a SOW. From this design stage the design-build contractor shall provide a more detailed drawing & specification set to include the requirements for the 90%, and “For Construction” design.

4.1.2 The Contractor is to repair any pre-existing defective pavement problems to include all dips, cracks, low spots, poor drainage, base or sub-base damage, holes, and/or soft spots. Any and all pre-existing defective pavement problems shall be properly repaired, resolved and then resurface POV and MEP parking lot pavement.

4.1.3 Contractor is responsible for adequate traffic and parking control in construction And paving areas for the duration of the project.

4.1.4 If needed, the contractor may need to complete the project in stages, giving ample Time between segments for material to cure and be striped before allowing traffic and Parking on new surface.

4.1.5 The estimated size of the lots and roadways are for planning purposes only and are Shown on the attached drawings. The drawings are estimates only. The contractor shall be Responsible for their own measurements and estimates. The contractor shall determine all The necessary equipment and materials to provide a complete and usable product.

4.1.6 All materials and methods used shall conform to California DOT Specifications for the Intended use and application of all phases of the job. Material delivery and application Shall conform to the recommended standards and quality control of materials.

4.1.7 Contractor shall avoid damage and contamination to any areas adjacent to project Work areas. To include but not limited to curbing, signage, turf, plants/vegetation, drains, Grates, manholes, light poles, flag poles, etc. Any damaged or contamination to any areas During the project shall be restored to original or better conditions. This is to include any Paving material and overspray, which shall be immediately removed and area thoroughly Cleaned.

4.1.8 The contractor shall measure all spacing of parking stalls and supply a plan with The maximum number of stalls obtainable. (This follows the previously painted and Striping on existing pavement, with straight lines and uniform parking places). The Contractors striping plan shall be submitted to the COR for approval.

4.1.9 All existing parking bumpers shall be removed and reinstalled once paving is complete.

4.1.10 New ADA complicate signage and markings as required shall be used and Installed where applicable. When complete, all aspects of the finished project shall Meet the Americans with Disabilities Act.

4.1.11 Any and all waste materials generated shall be removed off-site from property and Properly disposed of by contractor. This to include a final cleaning of the work sites of all Construction debris and equipment, leaving the areas clean, safe and ready for use.

4.2 POV Parking Re-Surface

4.2.1 Repair any pre-existing defective pavement problems to include all dips, cracks, low spots, poor drainage, base or sub-base damage, holes, and/or soft spots. Any and all pre-existing defective pavement problems shall be properly saw cut, excavated, sub-grade repaired, and resolved before resurfacing pavement.

4.2.2 Any and all cracks in existing pavement overlay area shall be properly prepared and cleaned of any vegetation, or debris, and filled with a crack filler per California DOT Specifications for roadways/driveways.

4.2.3 All existing manholes, covers, inlets, catch basins, and grates, shall be raised to keep the level of the new pavement.

4.2.4 Contractor shall thoroughly sweep and remove all debris, vegetation, and any loose surface material before paving. Any vegetation present during the initial cleaning shall be sprayed to kill roots below ground level, using an approved herbicide and dispensed by a licensed applicator following applicable State Regulations and manufacturers guidelines.

4.2.5 Once the surface has been properly prepared and repaired, a liquid tack coat shall be used to create a proper bond between existing pavement and new pavement.

This shall be followed by a minimum of a 2” thick hot asphalt mix surface course and laid per California DOT Specifications. Once surface course has been laid, the area shall be rolled smooth. Final finished course shall be unyielding, free from in-equites, with a smooth tight even surface, true to grade. Finished paving shall surface drain correctly and shall have no depressions that retain runoff water or water pooling.

4.2.6 Areas of finished paving shall have smooth transition with any and all existing adjacent and adjoining surface areas. (Concrete pads, sidewalks, footings, slabs, curbs, and any adjoining hard surface). Where newly finished paving meets any area of terrain (lawn, gravel, dirt, etc.), these areas shall be have a round smooth edge.

4.2.7 All new pavement shall be allowed to cure for proper time. Once cured, contractor Shall stripe the POV parking and any roadway areas. MEP area does not require striping.

Contractor shall apply thermoplastic pavement markings, as recommend by the manufacturer, following the determined striping plan. Vehicular traffic shall not be allowed on surface until proper curing of roadway markings.

4.2.8 Once paving is complete, all existing gate closures and catches shall be checked to be in good working condition, unobstructed by the new pavement. If not, these shall be corrected.

4.3 Military Equipment Parking Lot and Driveway

Remove top layers of asphalt in Military Equipment Lot as shown on drawings.

(Approximately 72,275 SF)

4.3.1 Contractor shall properly prepare ground surface and repair base for new asphalt. Final prepared sub-base in MEP Parking area shall be rated for high traffic and heavy loads per California DOT Specifications. Once base is properly prepared, the contractor shall maintain the base surface in this condition until the paving process begins.

4.3.2 New MEP Lot asphalt, when completed, shall be rated for high traffic and heavy loads per California DOT Specifications. Place and roll a hot asphalt mix base course, intermediate course, and then a final surface course with a tack coat applied between each course. After compaction and rolling, the final surface shall be at least a minimum of 2” inches thick.

4.3.3 Both finished and surface elevations shall follow the slope of the graded terrain and shall have no depressions that retain runoff water. Areas of finished paving shall have smooth transition with any and all existing adjacent and adjoining surface areas. The final surfaces shall also be free from defects, with a smooth tight even surface, true to grade.

4.3.4 All existing gate closures and catches shall be checked to be in working condition, unobstructed by the new pavement. If not, these shall be corrected.

4.4 Curbs:

4.4.1 Demo and remove areas to be replaced to the nearest joint or saw cut. Dispose of all debris off site. Install forms as necessary to retain and support concrete. Concrete surface shall be light broom finished.

5. WARRANTY:

5.1 All work specified in this contract shall have a warranty of a minimum period of one full calendar year for parts and labor from the customer’s acceptance date. At the final acceptance walk-through, the Contractor shall provide to the COR a signed warranty letter.

6. QUALITY CONTROL AND INSPECTIONS:

6.1 All work shall be inspected by the contractor’s quality control person and final inspection shall be performed by the COR prior to final payment.

6.2 Contractor shall maintain a clean, clear, and safe working environment at all times.

6.3 Only qualified personnel familiar with the proper construction techniques shall be used.

6.4 All site coordination shall be accomplished through the 63d RD COR.

6.5 At the final acceptance walk-through, the Contractor shall provide to the COR a complete set of as-build drawings, on disc, and in either .DWG or .DGN format.
