Small Business Participation Commitment Document

All offerors (both large and small businesses) are required to complete a Small Business Participation Commitment Document. Offerors should propose the level of participation of small businesses (as a small business prime, joint venture, teaming arrangement, and/or small business subcontractors) in the performance of the acquisition relative to the objectives set forth in the evaluation of this area.

(a) Check the applicable size and categories for the PRIME Offeror only -- Check all applicable boxes:

(d) Identify the Prime Offeror and type of service/supply that the Prime Offeror will provide.

Then list each of the intended subcontractors and principal supplies/services to be provided by that subcontractor. Provide the Commercial and Government Entity (CAGE) code for the Prime and each intended subcontractor. Also, provide the anticipated NAICS codes(s) that the Prime Offeror believes best describes the product or services being acquired by its subcontracts with each intended subcontractor. Small business Primes and small business subcontractors that qualify as small businesses in multiple small business categories should be listed in each applicable small business category.

Example: If a Small Business qualifies as a WOSB and a SDVOSB, you can add them to each category below in which they qualify.

Notes:

Pursuant to Sections 8(d) of the Small Business Act, a business is considered small for Government procurements if it does not exceed the size standard for the NAICS code that the prime contractor believes best describes the product or services being acquired. In other words, the size of the prime’s suppliers is determined by the applicable NAICS code of their joint venture, teaming partner, or subcontract, which may or may not be the same NAICS code as the one for your prime contract with the Government.

(e) Describe the extent of commitment to use small businesses (e.g., what types of commitments, if any, are in place for this specific acquisition either – small business prime, written contract, verbal, enforceable, non-enforceable, joint venturing, mentor-protégé, etc.)

Provide documentation regarding commitments to small business for this effort. Full copies

(all pages) of such agreements must be provided as part of your Small Business Participation Commitment Document but will not count against the page limitation for this volume. Any written agreements must also contain what work share the subcontractor will perform and a statement of work that defines the work that will be performed by the subcontractor.

(f) Provide a description of the methodology utilized to determine the proposed level of Small Business Participation for this specific acquisition. Offerors may propose less than the %/$ objectives specified herein, however they must provide sufficient justification supporting their level of proposed Small Business Participation based on historical evidence.

(g) Submit a list of up to three (3) Government contracts (prime and major subcontracts) performed or awarded by the offeror during the past three (3) years, which are relevant to the efforts required by this solicitation and which support the proposed level of Small Business Participation for this specific acquisition. This information shall be organized into the following sections per contract:

1) Contractor/Subcontractor place of performance, CAGE Code and DUNS Number. If the work was performed as a subcontractor, also provide the name of the prime contractor and Point of Contact (POC) within the prime contractor organization (name, and current address, e-mail address, and telephone and fax numbers).

2) Government contracting activity, current address, Procuring Contracting Officer's name, e-mail address, telephone and fax numbers.

3) Government’s technical representative/COR, current e-mail address, telephone and fax numbers.

4) Government contract administration activity, the Administrative Contracting Officer's name, current e-mail address, telephone, and fax numbers.

5) Contract Number (in the case of Indefinite Delivery type contracts, GSA contracts, and Blanket Purchase Agreements, include Delivery/Task Order Numbers).

6) Contract Type (specified type, such as Fixed Price (FP), Cost Reimbursement (CR), Time & Materials (T&M), etc.). In the case of Indefinite Delivery contracts, indicate specific type (Requirements, Definite Quantity, or Indefinite Quantity) and secondary contract type (FP, CR, T&M, etc.).

7) Award contract price.

12) Small Business Subcontracting Plan Required: Y/N

If Yes, provide approved Small Business Subcontracting Plan, Individual Subcontracting Reports (ISRs), Summary Subcontracting Reports (SSRs), and DD 294s (if applicable), as well as CPARS, Individual Subcontracting Agreements, and any other relevant documentation if applicable.

(h) All offerors shall submit information substantiating the offerors’ past performance in complying with FAR 52.219-8 Utilization of Small Business Concerns, FAR 52.219-9 Small Business Subcontracting Plan, as well as the offeror’s compliance with their Small Business Participation proposal and commitments (DFARS 215.304). Offerors shall also provide a statement indicating whether any negative information has been reported in the past six years concerning the offerors’ past compliance with FAR 52.219-8, FAR 52.219-9 and Small Business Participation commitments. If any such negative information has been reported, the offeror may submit explanations or comments responding to such negative information. Offerors with no prior contracts containing FAR 52.219-8, FAR 52.219-9 and/or Small Business Participation commitments shall certify the same.

(i) All offerors shall submit information relative to their ability to incorporate Small Business Innovation Research/Small Business Technology Transfer Program (SBIR/STTR) Program Technologies IAW DoD Instruction 5000.02.
