About this file

The document is a standard Internal Revenue Service (IRS) Form W-9 (Revised March 2024) for Request for Taxpayer Identification Number and Certification. This form is typically used by businesses and contractors to provide their taxpayer identification information to entities that will be reporting payments to the IRS.

The form provides fields for entering basic entity/individual information, federal tax classification, address details, and taxpayer identification number (either a Social Security Number or Employer Identification Number). The form includes certification statements where the taxpayer affirms the accuracy of their provided information, confirms their U.S. citizenship status, and attests to their backup withholding status. Key updates in this March 2024 version include modifications to line 3a for clarifying how disregarded entities complete the form and the addition of a new line 3b for flow-through entities to indicate the presence of foreign partners, owners, or beneficiaries.
