About this file

This is a Past Performance Questionnaire template designed to evaluate contractor performance for the Translation and Interpretation Services contract at Brooke Army Medical Center (BAMC), Fort Sam Houston, Texas under solicitation W81K0026RA042. The questionnaire is completed by references identified by offerors and submitted to the Medical Readiness Contracting Office - West (MRCO-W) via email to dulce.i.bares.civ@health.mil and salameya.paulouskaya2.civ@health.mil.

The questionnaire collects respondent and offeror information, contract details, and market segment data covering document transcription/translation, telephonic/video interpretation, on-site language interpretation, and American Sign Language services. References assess contractor performance across three key rating areas aligned with CPARS metrics: personnel certification (Quality of Service), response time (Schedule), and emergency request handling (Management), using a five-point scale from Exceptional to Unsatisfactory, plus Not Applicable. The questionnaire also requires a yes/no determination on whether the reference would award another contract to the firm and whether the prior contract was terminated for default or cause. This template is non-substantive content that serves as an evaluation tool rather than addressing specific contract requirements or deliverables.
