About this file

This is a Past Performance Assessment questionnaire issued by the U.S. Army Contracting Command-Redstone Arsenal (ACC-RSA) in support of Request for Proposal (RFP) Number W58RGZ-26-R-0008. The questionnaire is designed to collect performance information from previous government, state, local, or commercial contract customers of offerors competing for the contract. Responses must be submitted to ACC-RSA ATTN: CCAM-AVA-D (Ms. Antionette R. McClinton) no later than 15 days after receipt of the request or the RFP closing date, whichever occurs first. Submissions may be made electronically to usarmy.redstone.acc.mbx.acc-rsa-afm-iii-east@army.mil or usarmy.redstone.acc.mbx.acc-rsa-afm-iii-west@army.mil, with questions directed to Mrs. McClinton at (520) 674-8258.

The questionnaire contains two primary assessment formats: a written questionnaire and a telephone interview template, both evaluating contractor performance across five key criteria using a four-point rating scale (Substantial Confidence, Satisfactory Confidence, Limited Confidence, and No Confidence). Assessment areas include: compliance of products, services, documents, and deliverables to specification requirements and standards of good workmanship; effectiveness of project management including subcontractor control; timeliness of performance for services and product deliverables; effectiveness in forecasting and controlling estimated costs; and commitment to customer satisfaction and business-like concern for customers' interests. Each response requires a written rationale. The questionnaire also requests general comments, identification of other relevant past efforts by the company, and respondent identification information. This is a template document designed to standardize past performance evaluation for the stated RFP.
