LSIKNB, KuwaitST 901 Fluxgate differ by 45° from the Magnetic Compass.<FAULT> Discrepancy was noted during PMCS when testing Fluxgate and Magnetic Compass. Magnetic Compass and Fluxgate differ by 45°. Mag Compass current Deviation Calibration not available.

<TROUBLESHOOT> CFSR checked fluxgate compass operation finding a calibration was required. YB500 magnetic compass differed from Ritchie fluxgate compass by 45 degrees. CFSR got underway with the vessel, opened access console and performed calibration of the fluxgate compass. After calibration the magnetic compass shows a 17 degree deviation from the magnetic compass and was verified to match another heading source brought on the vessel by the harbor pilot. Magnetic compass will need to be serviced by a compass calibration facility when the vessel is again placed into service.

<PARTS>None

<CAUSE> Normal Wear and Tear

<ADDITIONAL> Magnetic Compass calibration is not performed by C4I technicians and will need to be checked and adjusted by NUG the next time the vessel is out of layup.

<COMMUNICATION> Vessel / APS5 personnel notified of system status.

LSI Tacoma, WA LSV 7 Sailor VHF Radios require installation <FAULT> LSV-2: Installation of radios on the bridge (replace Ross VHF Radios with the Sailor VHF Radios)

<TROUBLESHOOT> 03FEB2016: CFSR attended vessel and met with vessel POC to review the plan for installation. CFSR all requested to see what parts have arrived and are on hand for the installation. CFSR was only able to put hand on an antenna mount that vessel POC has received. CFSR was not able to put hands on the VHF units or the installation supplies. Further review will continue tomorrow in hopes to get hands on the needed equipment

04FEB2016: CFSR met with vessel POC and reviewed the needed items, and searched for the Army supplied radios. After some time the radios were found and accounted for. Then CFSR reviewed the drawings and found the antenna location to have the welding done and started to get access to the location. CFSR opened the head liner enough get to several of the wire runs and to assist in the planning of the installation. CFSR was also told that the FEDEX boxes had arrived and went to pick up the packages from the vessel POC. CFSR opened up the packages and brought the items to the vessel. All items are secured in the radio room at present time. CFSR departed the vessel and will continue with the start of the installation tomorrow

05FEB2016: CFSR attend vessel and continued with the installation of the VHF’s. CFSR removed old VHF one (1) and two (2) and cabling for both units. Opened up the rest of the wire runs to get access to the cable runs. CFSR’s pulled the cable up the mast, mounted the base and antenna. Completed the CSGA and stuffing pipe with heat shrink. CFSR’s pulled the other antenna cable to the needed location. CFSR’s prepared everything for the welders who are attending vessel Monday Morning 08FEB2016

08FEB2016: CFSR’s attend vessel and met with the welders. Unfortunately, the vessel did not have power and was not able to get power back to the vessel for several hours so arrangements were made with the welders to come back tomorrow (09FEB2016) at 06:00 am. CFSR’s mounted both VHF’s in the same location as the originals that were removed. CFSR’s terminated the antenna cables that were run at the VHF display units and installed the cable tags on the unit. CFSR received the strapping tool and started strapping the cables. CFSR completed the Power connection to both units verified the polarity and the available power to be 12VDC. CFSR connected the two VHF’s and tested for basic operation and the units came. CFSR removed the power. CFSR also rang out the external speaker cables and the speakers are good. CFSR’s departed the vessel.

09FEB2016: CFSR’s attend vessel and were to meet the welders on board at 06:00, CFSR were on sight at 05:30 and the welders showed up at 07:15 only to find out that then needed a different welder as the outlet the original welder was using was not suitable for the device. The welders did their stuff and completed the welding of the kick pipe and stacking studs around 12:00. CFSR over saw the welders and painted the welds with spray primer as to no leave the metal bear to the weather. This is only for temporary as the vessel is going to the ship yard to be painted.. CFSR’s completed the wiring of the data and connections for the second VHF. Unit was tested and powered on and received data. Cable tags were installed on the power and NMEA cables for both units. The antenna was mound on the starboard aft hand ail, and antenna cable was secured then terminated. The kick pipe was completed and sealed with heat shrink is in normal applications. The antenna cables were banded and secured in the head liner which was also starting to be reinstalled. CFSR’s departed the vessel.... CFSR: Labor 05:30 to 16:30; CFSR: Labor 05:30 to 16:30

10FEB2016: CFSR installed the procured screws in place of the missing knobs for the VHF mount. CFSR replace broken cover for indicator of the bow thruster. CFSR procured installation tape to replace installation around the welded kick pipe. CFSR then proceed to re-install the head liner and put label tags on the antennas and the VHF in accordance with the drawings....CFSR: Labor 08:00 to 16:30

11FEB2016: CFSR searched for the needed plugs that had arrived to vessels location yesterday 10FEB2016. Once the items were found CFSR attend vessel and proceed to make the needed connections for the speakers to the VHF’s. CFSR tested and verified the VHF are operational and the external speakers are working. CFSR then proceeded to clean up after the installation for final inspection of the system. CFSR departed the vessel.... CFSR: Labor 08:00 to 16:30.

<COMMUNICATION> None LSI KNB, Kuwait LSV 6 Sound Powered Phone on the Bridge cannot hear phone ring <FAULT> SP Phone on bridge does not ring

<TROUBLESHOOT> Went down on the vessel on Christmas day to view this ticket and other troubles called in by the vessel. this particular phone was not the one that was supposed to ring on Sta 1/a 1/b it is sta 3 DC . the 1/a and 1/b are tied together and are bridge console and radio room . they work correctly once i showed the officer that 3 was DC and is at the fwd end of the GMDSS console. the problem was resolved. it rang for both of the other stations with no problem

<CAUSE> Normal Wear and Tear

<ADDITIONAL> None

<COMMUNICATION> None LSI KNB, Kuwait ST 910 Fluxgate Compass and Magnetic Compass have a difference of 24°. <FAULT> ST 910 Fluxgate Compass and Magnetic Compass have a difference of 24°.

<TROUBLESHOOT> Found fluxgate reading minimum 270 to max 305 while in port. Vessel underway calibrated fluxgate compass. Fluxgate was within 2 degrees of magnetic while underway. Noted on return to port that fluxgate exhibited same issues once back on pier. Too much port interference for proper reading on fluxgate while in port but unit is functional outside of port.

<CAUSE> Normal Wear and Tear; drift over time

<ADDITIONAL> None

<COMMUNICATION> None LSI KNB, Kuwait ST 909 Fluxgate Compass and Magnetic Compass have a difference of 75°. <FAULT> Discrepancy was noted during PMCS, when performing operation Checks, identified that the Fluxgate Compass and Magnetic Compass have a difference of 75°.

<TROUBLESHOOT> CFSR spoke with APS5 personnel who took boat on maintenance run last week who stated they successfully calibrated the flux gate compass while underway. This fault is now resolved but please see note in additional.

<PARTS> None

<CAUSE> Normal Wear and Tear.

<ADDITIONAL> Flux gate compass will require recalibration each time vessel gets underway while currently on 1 year rotational underway schedule with APS5. The amount of time the system does not run adversely affects the the settings and calibration.

<COMMUNICATION> APS5 Vessel POC notified of repair.

LSI Port Hueneme, CA LCU 2033 Speed Log is showing speed when boat is stopped at port <FAULT> Speed log was showing incorrect speed.

<TROUBLESHOOT> CFSR on board vessel went underway to calibrate speed log, Skipper EM224. CFSR opened the unit turned the unit into the calibration mode and performed 6 runs (4 at 8 knts and 2 at 4 knts) and saved. Speed appears to be correct at this time. CFSR showed the crew how to perform a calibration so that they can store future calibration runs.

Other : CFSR on board vessel; replaced speed log transducer. Waiting for vessel to get underway on Sunday, 24 Jan 2016, to calibrate speed log.

<PARTS> SKIPPER SPEED LOG TRANSDUCER, MODEL EML40M-SKIPPER, P/N: EML224SG-SD

<CAUSE> Normal Wear and Tear

<ADDITIONAL> None

<COMMUNICATION> None LSI Bishop Point, HI LSV 4 RAI in the emergency steering system is not working <FAULT> LSV 4 RAI in the emergency steering system is not working

<TROUBLESHOOT> 02FEB2016: CFSR attended vessel and reviewed the documented problem. CFSR found the RAI’s to be available for movement but found the command for pump two (2) will not move the rudders. CFSR reviewed the system further and found that the alignments will need to be completed. CFSR then tested the steering at the wheel house and found the commands to work in both the port and starboard commands for both pumps. CFSR will continue with the troubleshooting tomorrow

03FEB2016: CFSR went through the entire steering system of the vessel and mad needed adjustment to insure proper alignments. All RAI work and are aligned to the mechanical RAI in the rudder room. The Autopilot, and Hand wheel are operational on pumps one (1) and two (2). The steering failure alarms are also operational and had to be adjusted. The only outstanding item is to find out why the tillers do not work in the Emergency steering on pump two (2). Drawings need to be reviewed for further troubleshooting

04FEB2016: CFSR went down into the emergency steering and troubleshot the steering system two (2). Found drawings in the archive of the steering system and trouble shot system according to the drawings. All voltages proper and rang out the selector switch which all checked out good. Checked out the relay and found it to be stuck. CFSR removed the relay from system one (1) and tested in system two (2) and found the EMG steering control to now work properly. CFSR tested relay from system two (2) in system one (1) and the relay worked properly. CFSR put both relays back into original location and tested the EMG steering. EMG steering control now is operational for steering pumps one (1) and two (2). This ticket is RESOLVED.

<CAUSE> Normal Wear and Tear

<ADDITIONAL> None

<COMMUNICATION> None LSI KNB, Kuwait LCU 2013 FS-2570 MF/HF Radio will not perform Scan or Manual Frequency Selection. <FAULT> FS-2570 MF/HF Radio will not perform Scan or Manual Frequency Selection.

<TROUBLESHOOT> CFSR reviewed the documented problem in the SIF and reviewed the history of the ticket. CFSR then found the vessel to be available for attendance and checked out the keys for access to the vessel. CFSR also found the parts which the DD1149 had not been signed, so CFSR took the proper measured to have the DD1149 signed so the part could be put used on the vessel. CFSR then accessed the vessel and verified the issue with the system. CFSR could not get the system to go into self-test mode and or the scan mode. The system would lock up and not respond to selection commands given. CFSR proceeded to remove and replace the Furuno FS 2550T transceiver. (Removed S/N: 35363007 Installed S/N: 35367944). CFSR tested the system and found the self-test to work perfect with all areas of the self-test pass and the frequency scanning feature is working normally. CFSR also verified the position input to the system which was present once the GPS was powered on. Thesis system is operational. This ticket is RESOLVED.

<PARTS> Furuno FS 2570T and used part control unit sn 001060 other CSFR

<CAUSE> Normal Wear and Tear

<ADDITIONAL> None

<COMMUNICATION> None Vendor - FURUNO USA KNB, Kuwait LCU 2013 FS-2570 MF/HF Radio will not perform Scan or Manual Frequency Selection. <FAULT> FS-2570 MF/HF Radio will not perform Scan or Manual Frequency Selection.

<TROUBLESHOOT> CFSR reviewed the documented problem in the SIF and reviewed the history of the ticket. CFSR then found the vessel to be available for attendance and checked out the keys for access to the vessel. CFSR also found the parts which the DD1149 had not been signed, so CFSR took the proper measured to have the DD1149 signed so the part could be put used on the vessel. CFSR then accessed the vessel and verified the issue with the system. CFSR could not get the system to go into self-test mode and or the scan mode. <CAUSE> Normal Wear and Tear

<ADDITIONAL> None

<COMMUNICATION> None LSI YND, Japan LCU 2015 Wind Tracker is displaying incorrect wind information. <FAULT> Wind bird gives false wind direction to Wind Tracker display.

<TROUBLESHOOT> Checked system and wind direction on wind tracker was off from wind bird by approximately 60 degrees. Visually checked base of wind bird from fly bridge to see if it had moved on mounting post but was still in original position. Couldn't climb mast due to excessive cracks that needed repair. CFsr changed line driver to see if that was the problem but nothing changed. After mast repair, removed and replaced wind bird with unit supplied by Connelly.

Operational check shows unit operating correctly.

<TROUBLESHOOT> Checked system and wind direction on wind tracker was off from wind bird by approximately 60 degrees. Visually checked base of wind bird from fly bridge to see if it had moved on mounting post but was still in original position. Couldn't climb mast due to excessive cracks that needed repair. CFSR changed line driver to see if that was the problem but nothing changed. After mast repair, removed and replaced wind bird with unit supplied by Connelly.

Operational check shows unit operating correctly.

CFSR re programmed the NautoConning configuration for the needed boxes and ran into problems with the sensor selection screen. CFSR change the configuration multiple times and even tested the system on CFSR’s personal laptop. CFSR called Raytheon with state side supplied phone and spoke with a Raytheon service support person to receive guidance to assist with the issue that is being experienced. After some time is was found that it would be better to load in an older “Prj” file and configure the system form there.

<TROUBLESHOOT> Sunday 24 Jan 16 CFSR CFSR onboard to perform speed log calibration and operational test. The LCU-2030 got underway for speed log calibration and operational test.

Ship traveled for one measured mile at 5 knots constant course constant speed as per calibration procedures. After one mile traveled, ship reversed course with no change in speed and sailed the reciprocal of the first leg to complete the calibration. After the completion of the calibration, CFSR verified speed log tracked speed over ground against GPS. Speed recorded was within tolerances specified in tech manual. Calibration and operational test completed satisfactorily. Ticket resolved.

<TROUBLESHOOT> CFSR checked operation of monitor and found broken and discolored video displayed on monitor. CFSR isolated fault applying a secondary video source and cable to the monitor assembly with the same fault exhibited. Ordered and installed replacement monitor, which required modification of overhead panel for proper fit up. Operational test of monitor are satisfactory.

<ADDITIONAL> An additional run error fault has been found on the nautoconn PC, an additional IR will be submitted by APS5 to address this fault.

<TROUBLESHOOT> Multiple CFSR's worked on this fault manipulating and checking GPS inputs at both the Leica DGPS units and configuration settings with the Main ECDIS itself with little to no change in operation. Fault was intermittent and was unable to be replicated when CFSR's were aboard the vessel. After consulting Raytheon Kiel, the determination was made to reimage the drive and reload the programming values. Upon completion the vessel got underway and has reported no faults in the operation of the Main ECDIS.

LSI Curtis Bay, Baltimore LT 803 NautoConning display will not turn on. <FAULT> LT 803 NautoConning display will not turn on.

<TROUBLESHOOT> Display monitor would not turn on. CFSR replaced monitor with a new one. CFSR Cardenas on board 17DEC2015. CFSR uploaded the pictures of the rip out and install onto the FTP in a folder marked Nautoconn display install pictures. The previous monitor had a cutout in the front aluminum casing. CFSR's initial intention was to widen the cut out to fit the new display. However, grinding was not permitted on the bridge. Also, the casing was riveted in place so it would have been very difficult to remove and grind in the shop. CFSR decided to mount the new, wider monitor onto the casing, attaching it to the casing and having the monitor sit recessed, just inside the opening. The new monitor has four bolt holes. The casing has a small lip under the opening, where the monitor can sit. CFSR centered the monitor in the opening, drilled out four holes in the casing, and bolted the new monitor onto the casing. POC procured and provided the bolts and the nuts that we needed and provided the specs for them. He was critical to the success of this install. The specs are as follows:

Stainless steel screws: 1/4-20 x 1" Hex Head Cap ESNA stainless steel Nylock nut: 1/4-20 1/4" Flat washer.

<COMMUNICATION> None LSI KNB, Kuwait ST 909 Depth Finder displays blank screen after power up <FAULT> Discrepancy was noted during PMCS, when performing power up and operational checks of the Depth Sounder. System Powers on but has no numerical value reading on display. Transducer may require Cleaning as first COA. Recommend requesting Army Diver support from APS-5.

<TROUBLESHOOT> CFSR spoke with POC and verified divers had checked and cleaned the depth sounder face which did not correct fault. CFSR's CFSR and CFSR performed a temporary install of a critical spares transducer and found that this is the problem with the depth not reading correctly. CFSR CFSR spoke with Mr Shupe concerning installation problems in wiring routing and the request was made to install an intermediate box for the transducer to couple to the display unit. CFSR installed transducer and installed box assembly and secured. System is now fully operational.

Once the proper documentation was completed CFSR signed vessel VHF and the key for access to the vessel. CFSR then proceeded to the vessel removed the protective door and window shielding for access to the bridge and hardware around the window. CFSR then put the VHF in place with temporary hardware (knobs for securing the radio are missing) then made the power and antenna connections. Once all was set up CFSR applied the received “Split Loom” and secured the cable run with the “Loom Clamps”. Once the cabling was secured CFSR verified the power cabling by a continuity check between the Power Supply and the VHF power cable plug. CFSR removed the VHF, secured the protective door and window shielding. CFSR then returned the VHF and the access key to the vessel to APS-5. The documented problem has been RESOLVED.

CFSR and Logistician removed VAU AM-7239/VRC S/N: 022430AA and RF AMP AM-7238B/VRC S/N: 008876 and installed VAU AM-7239E/VRC S/N: 023719 and RF amp AM-7238B/VRC S/N: 063078, Speaker, W4 and W2 cable (Fabricated locally by CFSR) in craft. All components were previously tested by CECOM and with the approval of POC the IR is complete. Complete system testing will be conducted when power is available to craft during next PMCS. Submitted Service Report to Vessel POC for digital signature. Awaiting Response. Status changed to Pending – Customer Hold
