About this file
This document is an environmental atlas that provides information about institutional controls and administrative controls related to various sites at an Air Force base. The atlas shows the approximate locations of restoration sampling and site locations, including well types such as monitoring, bioventing, piezometer, and injection wells.
The key details include soil and groundwater use restrictions, such as prohibiting the installation or use of drinking water wells, securing monitoring wells with locks, and requiring approval for any activity that may result in access to contaminated groundwater or disturbance of remedial actions. The document also outlines base administrative controls, including the requirement to obtain approval for any soil-disturbing activities, and decision document controls, such as restrictions on the installation or use of drinking water wells and the disposal of any removed contaminated soil or groundwater. The information in this atlas is intended to be used to maintain an accurate definition of the area of contamination and ensure proper management of the sites.

Institutional Controls
Soil and Groundwater Use Restrictions
Sites WP045, SS057, SS534, SS086, TU106, SS303P, SS306P, SS311P, SS314P, SO116
• Development of a site map showing the areas currently and potentially impacted by groundwater contaminants to determine where the controls are to be implemented
• A prohibition on the installation or use of drinking water wells
• A requirement that all monitoring wells are secured with locks to prevent unauthorized access to groundwater
• Any activity that may result in access to contaminated groundwater or affect the movement of contaminated groundwater requires approval by Environmental Flight (CEIE)
• Any activity that may result in the disturbance of any remedial action requires approval by
CEIE
• Any activity that may result in exposure to or removal of contaminated soil requires approval by CEIE. In the event that contaminated soil or groundwater is removed from the source area, it will be disposed of or treated in accordance with applicable state and federal regulations
• A requirement of notice to and approval by ADEC and EPA of any proposal to add to or alter land use controls
• A requirement to notify ADEC and EPA of any proposal to change the existing land use
• Groundwater monitoring is conducted under the IWMP to maintain an accurate definition of the area of contamination
Base Administrative Controls
General Base Administrative Controls
Sites SS534, SS086, TU106, SS303P, SS306P, SS311P, SS314P, SO116
• Any activity that may result in the disturbance of soil requires approval by Installation Management (CEIE). Complete Base Civil
Engineering Work Clearance (Form 103) and obtain approval prior to any work at the site.
In Addition to the General Administrative Controls
Sites SS057, WP045
• To the extent that groundwater contamination remains above unacceptable levels, deed restrictions or equivalent safeguards will be implemented in the event that property containing such contamination is transferred by the Air Force
• Notification to and approval by ADEC and EPA will be required for any proposed addition or alteration to land use controls or changes to existing land use, or of potential ground‐disturbing activities within 5 days of receiving request
• Development and approval of applicable SAPs, WPs, and waste management plans for ground disturbing activities prior to implementation of work
• In the event of base closure, any remaining contaminated sites will be addressed in accordance with CERCLA Section 120
Decision Document Controls
General Decision Document Controls
Sites SS057, WP045, SS086
• The installation or use of drinking water wells is restricted
• Secure all monitoring wells with locks to prevent unauthorized access to groundwater
• Any activity that may result in access to contaminated groundwater or affect the movement of contaminated groundwater requires approval by Installation Management (CEIE)
• In the event that contaminated soil or groundwater is removed from the source area, it will be disposed of or treated in accordance with applicable state and federal regulations
In Addition to the General Decision Document Controls
Site SS534
• There is a 200 foot buffer boundary on the site until better delineation is defined
*Plume boundaries are approximated based upon the most recent groundwater monitoring results. Plume boundaries shown on the map represent the point at which contaminant concentrations in the groundwater equal their respective cleanup levels. Where multiple contaminant concentrations are present, boundaries represent the furthest areal extent of contamination above cleanup levels. See Table 1a for more information about specific plumes.
**Labels on buildings indicate Building Numbers on the top line and Facility ID on the bottom line.
