About this file
This is a blank W-9 form (Request for Taxpayer Identification Number and Certification) related to a Colorado state and local contract opportunity for Robotic Arm & Accessories issued by Arapahoe Community College (ACC). The form serves as a standard tax identification document required from vendors and contractors doing business with government entities. The document requests the contractor's taxpayer identification number (either Social Security Number for individuals or Employer Identification Number for businesses), business classification information, and certification of U.S. person status under penalties of perjury.
The form includes required federal business classification certifications that contractors must complete, including designations for Large Business Concern, Small Business Concern, Woman-Owned Small Business, Small Disadvantaged Business, HUBZone Small Business, Veteran-Owned Small Business, and Service-Disabled Veteran-Owned Small Business. Contractors must certify their business status and, if applicable, provide SDB ethnicity classifications. The form emphasizes penalties for misrepresentation of small business status, including fines, imprisonment, suspension, debarment, and program ineligibility under the Small Business Act. As a blank template, this document does not contain specific contract details such as pricing, delivery dates, quantities, or award information; rather, it is a pre-bid requirement that potential contractors must complete and submit with their proposals for the ACC DQ26-3-4126 Robotic Arm & Accessories procurement.
