About this file

This is a Quality Assurance Surveillance Plan (QASP) for the VTC/AV Lifecycle Support contract at Hill Air Force Base that establishes the Government's framework for evaluating and monitoring contractor performance.

The QASP defines six core performance objectives with specific acceptable quality levels (AQLs) and surveillance methods. The Contractor must submit a Monthly Activity Report (MAR) by the 5th business day of each month containing system status, STIG compliance, ServiceNow tickets, and supported organizations, subject to 100% inspection by the Contracting Officer's Representative (COR). A staffing roster must be provided within 10 business days of award with updates submitted within 5 business days of personnel changes, reviewed periodically by the Security Manager and COR. User documentation and training coordination for newly installed systems is monitored through MAR review and customer feedback. Corrective maintenance requests require response within specified timelines: Emergency (4 hours), Routine (2 days), and Minor (3 days), documented in the MAR and verified through ServiceNow reports. Security compliance requires 100% adherence to DD254, security clearance investigations, and non-disclosure agreements with no security breaches, subject to 100% inspection. Personnel must complete all required training (OPSEC, STINFO) for facility and network access, reviewed periodically by the 75 ABW/SC Training Manager. Surveillance methods include 100% inspection of mandatory deliverables, periodic random checks of staffing rosters and compliance reports, and investigation of valid customer complaints. The Government will document performance assessments, and repeated failures may result in an unsatisfactory CPAR rating or further contractual action.

VTC/AV Lifecycle Support - Hill Air Force Base

1.0 Objective
