About this file

This is a Vendor Qualifications Response Form for Request for Qualifications (RFQ) 99SWC-S4004 issued by the State of Nevada to solicit qualified vendors for employee drug and alcohol testing services. The RFQ seeks vendors capable of providing collection and testing of urine, breath, blood, and other bodily substances for alcohol and drugs of abuse throughout Nevada. Vendors must demonstrate the ability to provide all six required employment-related testing circumstances: pre-employment, reasonable suspicion, return-to-work, follow-up, random, and post-accident/post-incident testing. Services must be available across 31 specified Nevada locations, including major cities such as Las Vegas, Reno, Henderson, and North Las Vegas, as well as smaller communities throughout the state. The RFQ requires vendors to complete a comprehensive two-stage qualification process addressing both general and technical minimum qualifications, with vendors who fail to meet any Stage One requirement being eliminated from consideration.

Vendors must possess current business licenses, professional certifications, and accreditations required to perform services, with a minimum of three years of experience providing employment-related drug and alcohol collection or testing services. The qualification response requires vendors to identify whether services will be provided directly or through subcontractors and third-party providers, including collection sites, laboratories, and Medical Review Officers. Vendors must demonstrate conformity with Federal Motor Carrier Safety Administration (FMCSA), U.S. Department of Health and Human Services/Substance Abuse and Mental Health Services Administration (HHS/SAMHSA), Nevada Revised Statutes, and Nevada Administrative Code requirements. Additional requirements include providing HHS/SAMHSA-certified laboratory documentation, implementing quality assurance and quality control programs, establishing 24/7 service availability at certain locations, delivering quarterly utilization reports, and maintaining confidentiality safeguards compliant with applicable privacy regulations. The contract is intended to commence January 1, 2027, with the State seeking to establish statewide coverage through one or more cumulative awards.
