About this file
This is a vendor affidavit document required by the School Board of Hillsborough County, Florida (Hillsborough County Public Schools) for waterproofing services. The affidavit must be signed by an authorized representative of any nongovernmental contractor executing, renewing, or extending a contract with HCPS and submitted under penalty of perjury in accordance with Section 787.06(13) of the Florida Statutes. The affidavit certifies that the contractor does not engage in human trafficking or use coercion for labor and services, specifically prohibiting practices such as physical force, unlawful restraint or confinement, exploitative debt practices, document confiscation, financial harm threats, fraud, deceit, or provision of controlled substances for exploitation purposes.
No specific pricing terms, contract amounts, award dates, or contract duration information is contained within this affidavit document. This is a compliance document that serves as a prerequisite certification requirement rather than a standalone solicitation. Contractors must complete and submit this affidavit as part of their proposal response for the waterproofing services opportunity, which involves identification of water intrusion sources, building envelope repairs, surface preparation, and application work in compliance with the Florida Building Code, Florida Statutes, OSHA regulations, ASTM standards, manufacturer recommendations, SMACNA standards, and HCPS Facilities Maintenance Department requirements.
