The specific contract opportunity to which this affidavit applies is for Tree Trimming and Removal Services, which includes provision of all labor, materials, supervision, equipment, and related items necessary to complete work according to specifications.
The affidavit requires contractors to certify that they do not engage in prohibited coercive labor practices, including use or threat of physical force, unlawful restraint or confinement, debt bondage arrangements, confiscation of identification documents, financial harm threats, fraud or deception for labor exploitation, or provision of controlled substances for exploitation purposes. Contractors must execute this affidavit as a condition of contract award and performance with HCPS. No specific pricing, set-asides, incumbent information, funding amounts, or contract term details are addressed in this affidavit document, as it serves solely as a compliance certification requirement separate from the substantive contract terms and conditions.
