The following responses are provided to questions asked by industry.
Questions
1.CHEYENNE
Clarification Question # 5 from Amendment 0006 Previous Question May the Government please confirm the quantities listed in CLIN’s 0013 through 0014AA, as the quantities for the cylinder refills do not correlate with the quantities estimated for the portable gaseous systems?
NCO-19 Response states “quantities are correct.”
New Question How can 120 portable gaseous systems (10 per month) according to the revised Price Schedule, generate 136,800 portable tank refills (11,400 per month)? Not possible? Please advise.
NCO-19 Response: CLIN’s 0013 through 0014AA numbers are based on historic usage.
2. SHERIDAN
New Question Even after reviewing revised Price Schedule, it appears the total Rental Concentrator estimates are extremely low. SOW states there are 1,040 home oxygen patients. The Rental Concentrator estimates in Line Items 001AA, 0002AA, 0003AA, and 0004AA combine for 2,2220 estimated annual concentrator rentals and broken down comes out to only 185 concentrator rentals per month? Please advise.
NCO-19 Response: Not all patients are issues a piece of equipment from Line Items 001AA, 0002AA, 0003AA, and 0004AA, many are issued a portable gaseous system Line Item 0005 or other piece of equipment that accounts for the estimated annual number of patients of 12,480.
3. OKLAHOMA CITY
New Question Even after reviewing revised Price Schedule, it appears the total Rental Concentrator estimates are extremely low. SOW states there are 1,001 home oxygen patients. The Rental Concentrator estimates in Line Items 0001AA, 0002AA, 0003AA, 0004 and 0004AA combine for 6,020 estimated annual concentrator rentals and broken down comes out to only 500 concentrator rentals per month? Please advise.
NCO-19 Response: Line items 0001AA, 0002AA, 0003AA,0004, and 0004AA have been revised for accuracy.
4. VA EASTERN OKLAHOMA
New Question There are line items (0004, 0004AA & 0004AB) pertaining to Portable Gaseous Systems. There are no line items for Portable Gaseous Contents? Please advise.
NCO-19 Response: Line item 0004AB has been revised to say the following, “Monthly Rental of Portable Gaseous System Sizes B (M6), C, D, or E: Monthly rental fees for contents of Portable Gaseous Systems that are in use covering all necessary supplies, batteries (if needed for regulator) and equipment as specified in the PWS (Consumable Supplies).”
5. PAST PERFORMANCE
Previous Question The previous due date for Past Performance Questionnaires to be submitted was 4-10-17. For contractors who had questionnaires submitted on their behalf, will another Past Performance Questionnaire request be required? If so, may you please advise on a new date for the questionnaires to be submitted to the government?
NCO-19 Response states “No additional Past Performance Questionnaires need to be sent if they were previously submitted. However, you should include the previous completed questionnaires as part of your revised proposal.”
New Question On the last page of the Past Performance Survey Form, it states in bold to the responding party “DO NOT return survey to the company you are evaluating.” Therefore, we do not have a completed questionnaire to submit as part of our revised proposal? Please advise.
NCO-19 Response: If you previously provided a past performance questionnaire to your references, and we have received, we will use those in the evaluation process. However, if you did not provide your references with the past performance questionnaire, you will need to ensure this is accomplished.
6. Any chance you would consider extending the bid due date by a week to Monday, July 17th in light of all of the recent bid amendments?
NCO-19 Response: No, the proposal due date is Monday, July 10th 2017.
7. Question number 2 asks about submitting the Operation and Service Manual for each type of equipment with any proposals regarding this contract. It also asks if this is to be considered part of the Price/Contract documents, which have no page limitations, The Government response states: “The Operation and Service Manual is not part of the price document. The Operation and Service Manual will not count towards the page count of the Price/Contract documents.” Question 1) if the Operations and Service Manuals are not part of the Price/Contract documents, then in which volume should they be submitted. Question 2) If the Operation and Service Manuals are not part of the Price/Contract documents, why do you state that they will not count towards the page count of the Price/Contract documents? This is very confusing. Please clarify. Question 3) Page 60 of 67 of Amendment A00004 item c.1.A states: “Email files shall not be larger than 3MB per email with no more than 3 emails.” Since the Operations and Service Manuals will of necessity be PDF files, they could very easily exceed the 3MB limit. Please either raise the limit on file size or delete the requirement to submit these manuals with the proposal.
NCO-19 Response: Paragraph has been revised to remove the 3 email limitation.
8. The response to Question 129 states: “paper copies will be accepted, the language in Recipe of Proposals has been updated to include that two paper copies be submitted.” Page 60 of 67 of Amendment A00004, item c.1.A states: “One hard copy proposal shall be mailed to:” Does the Government want 1 or 2 hard copies submitted and must the hard copy be received by the same date and time as the email submittal? Please clarify.
NCO-19 Response: Only electronic copy is required. The requirement for a paper copy is removed. A paper copy is not required to be mailed.
9. The response to Question 151 states: “Please include each requested survey your company sent out with the proposal, these will not count towards page count, these should be included for any reference provided.” Page 62 of 67 of Amendment A00004 item 2 states: “Point of Contact Listing: provide a list of questionnaire recipients with the POC and phone number for each contract reference provided as part of the Past Performance Volume.” Since contractors re required to submit detailed reference information and the POC information, and the questionnaires are to be submitted directly to the Government by the references, why is it necessary to also submit the questionnaire as part of the proposal? This seems like a redundancy and will just cause the Past Performance Volume to be a larger file, potentially exceeding the 3MB limit. Please remove the requirement to submit the Past Performance Questionnaire as part of the Past Performance Volume.
NCO-19 Response: Past performance questionnaire is not part of the proposal submitted by the offeror. The Past performance questionnaires should be emailed in by your references.
10. Page 60 of 67 of Amendment A00004, Table 1 – Proposal Organization shows: Volume I Past Performance, Volume II Technical and Volume III Price/Contract Documents. Page 61 of 67 item 8 Receipt of Proposals, states the same. Items 9.a and 9.b on page 61 of 67 show: Volume I, Technical Approach (Factor 1) and Volume II, Past Performance (Factor II). Please clarify which volume is which.
NCO-19 Response: Addendum to FAR 52.212-1 has been revised for consistency as follows: Volume I Technical, Volume II Past Performance, Volume III Price/Contract Documents. The revised provision is attached to this amendment.
