Phillips Plugging Project Technical Specifications Summary

This is a Technical Specifications document issued by the West Virginia Department of Environmental Protection (DEP), Office of Oil and Gas, for the plugging and remediation of an abandoned oil and natural gas well located at 296 Plantz Hill Road, Hundred, West Virginia (API# 47-103-03628). The project encompasses comprehensive well plugging services including site preparation, well plugging operations, site reclamation, and monument installation. The well, situated in Wetzel County on a 3.14-acre parcel, is located 50 feet from the landowner's house and requires construction of a 350-foot service road with a 16" x 80" culvert, installation of erosion and sediment controls, and erection of an 8-foot by 50-foot safety barrier. The project involves plugging a well to a total depth of approximately 3,200 feet using prescribed cement and gel spacer plugs at multiple intervals, with the work to be performed Monday through Friday during standard business hours with a maximum ten-hour workday, subject to Department approval for variations.

The contract is structured as a fixed-price bid for services with half of the mobilization and demobilization costs payable upon mobilization and the remainder due with final invoice. Pricing includes numerous line items for equipment rental and services such as rig operations, heavy equipment, specialized tools (power tongs, casing spears, overshoots, impression blocks), materials (cement, aqua gel, gravel, topsoil, tie mats, silt fence), and disposal services for gel, liquid waste, and solid waste. The Department will obtain and pay for all Local, State, and Federal permits necessary for project completion, with permit conditions becoming mandatory contract inclusions. The contractor must provide a minimum three-person crew and is responsible for daily tracking of time and material utilization for invoicing purposes, with all expenditures to be confirmed by the Department at the conclusion of each workday. The project is subject to compliance with West Virginia Code 22-6-23 and 22-6-24, as well as the West Virginia Erosion and Sediment Control Field Manual (May 2012), and will be overseen by a DEP inspector specialist/project coordinator.

IV. PROJECT DESCRIPTION

1.0 Site-based considerations:

The well is located in the side yard and 50’ from the landowner’s house. The wellsite is flat and covered in grass, no brush or trees. The well is currently leaking gas from the 6” surface casing at ground level. There is no storage tank located at the well site. The 2” production line shall be disconnected, capped, and abandoned below final grade. A service road will be constructed from the county road to the wellsite at an estimated length of 350’. The service road will require a 16” x 80” culvert. Stone and tie mats will be used to construct the road. All stone must be removed upon completion. A wall shall be constructed between the well and house for the safety of the occupants. OOG will contact locate 811 to have all subsurface utilities marked.

2.0 Well-specific considerations:

The Office of Oil and Gas does not have a well record for the subject well (API# 47-103-03628).

An offset well record (API# 47-103-00152) will be used as a reference for downhole conditions.

The record shows well total depth (TD) at 3218’. Additionally, the record shows 10” conductor casing and 6” surface casing, depth and cemented location unknown. The record shows 2” tubing and packer, depth and length of tubing and packer are unknown. Gas was recorded at 3160’, 2320’, and 2230’. Coal was recorded at 875’ and freshwater at 120’. Elevation at the wellsite is 1099’.

V. PROJECT WORK PLAN

Contract work may require site remediation of the well site prior to the commencement of plugging operations.

Project will commence, generally, as follows:

1. Pre-mobilization site remediation, if necessary,

2. Site preparation,

3. Connect rig to well,

4. Clean out well to total depth,

5. Set prescribed plugs and monument,

6. Reclaim site including removal of stream crossings, and

7. Submit invoice to DEP.

Site Preparation Plan

Five hundred (500) feet of silt fence will be installed around the well site and service road to isolate the disturbed areas from the landowner’s home, yard, and stream. The area around the well (50’ x

100’) may be covered with tie mats to stabilize the plugging equipment, and no stone may be used around the well. A service road (approximately 350’) will need to be constructed with stone or tie mats to accommodate plugging equipment; all stone must be removed upon completion. A 16” x

80’ culvert will be temporarily installed to bridge a drainage ditch in the service road. All production equipment shall be dismantled and removed from the well site. A minimum 24” cellar will be installed around the well head. A temporary wall will be constructed between the well and the landowner’s house. The wall shall be a minimum of 8’ in height and 50’ in length. The wall may be constructed from wood, metal, plastic, or other materials that have been pre-approved.

Proposed Plugging Work Plan:

1. Notify Project Coordinator (PC) within 24 hours of plugging commencement.

2. Move in and rig up to existing 6” casing.

3. Trip in hole (TIH), pull tubing and packer if found.

4. TIH and check total depth (TD) at or below 3200’. If TD cannot be reached, begin cleaning out hole to approved depth / TD by inspector.

5. Once TD has been reached / approved, a bond log will be ran inside the 6” casing to determine cement bond locations and free points.

6. TIH to TD with tubing, spot 6% gel followed by cement from 3200’ to 3000’. Trip out hole

(TOH), wait on cement (WOC), and tag top of plug. Add cement if needed.

7. TIH with tubing, spot 6% gel from 3000’ to 2400’, followed by cement plug from 2400’ to 2150’. TOH, WOC and tag top of plug. Add cement if needed.
