About this file

This is an Addendum to a Request for Quotes (ARFQ) issued by the West Virginia Department of Environmental Protection (DEP) for the Phillips Plugging Project (ARFQ DEP24-41). The addendum adds Commodity 38.0 Temporary Air Bridge to Section VI Technical Specifications and publishes a pre-bid sign-in sheet and vendor questions with responses to the vendor community. The project involves plugging and remediating an abandoned oil and natural gas well (API# 47-103-03628) located at 296 Plantz Hill Road in Hundred, West Virginia. The well, with a total depth of approximately 3,218 feet, requires comprehensive site preparation, plugging operations, and site reclamation in accordance with West Virginia Code 22-6-23 and 22-6-24. The scope of work includes constructing a service road with a temporary culvert, installing safety barriers, performing multi-stage cement plugging operations, and restoring the site to its original condition. Work is limited to Monday through Friday during standard business hours (maximum 10 hours per day), with the contractor responsible for all equipment, materials, labor, and professional services.

The technical specifications outline 38 distinct line items spanning equipment rental and operation (including rig and crew, heavy equipment, vacuum trucks, and specialized tools), material purchases (casing, tubing, cement, gravel, and topsoil), and disposal services (gel, liquid waste, and solid waste). Key specifications include a mobile rig capable of pulling 150 percent of anticipated hook loads with a three-person minimum crew, various pumps and drilling tools, and tie mats for site stabilization. Payment is structured through multiple billing mechanisms including hourly rates for equipment and labor, per-day or per-unit charges for specialized tools, and lump-sum fees for site preparation and reclamation. The Department of Environmental Protection will obtain and pay for all local, state, and federal permits, with a designated Project Coordinator overseeing contract execution. Vendors must acknowledge receipt of all addenda and submit acknowledgement forms with their bids to avoid disqualification.

IV. PROJECT DESCRIPTION

1.0 Site-based considerations:

The well is located in the side yard and 50’ from the landowner’s house. The wellsite is flat and covered in grass, no brush or trees. The well is currently leaking gas from the 6” surface casing at ground level. There is no storage tank located at the well site. The 2” production line shall be disconnected, capped, and abandoned below final grade. A service road will be constructed from the county road to the wellsite at an estimated length of 350’. The service road will require a 16” x 80” culvert. Stone and tie mats will be used to construct the road. All stone must be removed upon completion. A wall shall be constructed between the well and house for the safety of the occupants. OOG will contact locate 811 to have all subsurface utilities marked.

2.0 Well-specific considerations:

The Office of Oil and Gas does not have a well record for the subject well (API# 47-103-03628).

An offset well record (API# 47-103-00152) will be used as a reference for downhole conditions.

The record shows well total depth (TD) at 3218’. Additionally, the record shows 10” conductor casing and 6” surface casing, depth and cemented location unknown. The record shows 2” tubing and packer, depth and length of tubing and packer are unknown. Gas was recorded at 3160’, 2320’, and 2230’. Coal was recorded at 875’ and freshwater at 120’. Elevation at the wellsite is 1099’.

V. PROJECT WORK PLAN

Contract work may require site remediation of the well site prior to the commencement of plugging operations.

Project will commence, generally, as follows:

1. Pre-mobilization site remediation, if necessary,

2. Site preparation,

3. Connect rig to well,

4. Clean out well to total depth,

5. Set prescribed plugs and monument,

6. Reclaim site including removal of stream crossings, and

7. Submit invoice to DEP.

Site Preparation Plan

Five hundred (500) feet of silt fence will be installed around the well site and service road to isolate the disturbed areas from the landowner’s home, yard, and stream. The area around the well (50’ x

100’) may be covered with tie mats to stabilize the plugging equipment, and no stone may be used around the well. A service road (approximately 350’) will need to be constructed with stone or tie mats to accommodate plugging equipment; all stone must be removed upon completion. A 16” x

80’ culvert will be temporarily installed to bridge a drainage ditch in the service road. All production equipment shall be dismantled and removed from the well site. A minimum 24” cellar will be installed around the well head. A temporary wall will be constructed between the well and the landowner’s house. The wall shall be a minimum of 8’ in height and 50’ in length. The wall may be constructed from wood, metal, plastic, or other materials that have been pre-approved.

Proposed Plugging Work Plan:

1. Notify Project Coordinator (PC) within 24 hours of plugging commencement.

2. Move in and rig up to existing 6” casing.

3. Trip in hole (TIH), pull tubing and packer if found.

4. TIH and check total depth (TD) at or below 3200’. If TD cannot be reached, begin cleaning out hole to approved depth / TD by inspector.

5. Once TD has been reached / approved, a bond log will be ran inside the 6” casing to determine cement bond locations and free points.

6. TIH to TD with tubing, spot 6% gel followed by cement from 3200’ to 3000’. Trip out hole

(TOH), wait on cement (WOC), and tag top of plug. Add cement if needed.

7. TIH with tubing, spot 6% gel from 3000’ to 2400’, followed by cement plug from 2400’ to 2150’. TOH, WOC and tag top of plug. Add cement if needed.

8. TIH with tubing, spot 6% gel from 2150’ to 1150’ followed by cement. Plug from 1150’ to 850’. TOH, WOC and tag top of plug. Add cement if needed.

9. TIH with tubing, spot gel from 850’ to 500’ followed by cement plug from 500’ to 400’.

WOC, and tag top of plug. Add cement if needed.

10. TIH with tubing, spot gel from 400’ to 200’ followed by cement plug from 200’ to surface.

11. Rig down, install monument, and reclaim site.

Note: Any casing found in the well without cement in the annulus that cannot be retrieved must be perforated and cemented at all saltwater, freshwater, coal, oil, gas, casing shoe and elevation points.

Site Reclamation Plan

All production equipment shall be removed and disposed of. All silt fences shall be removed and disposed of. All tie mats and/or stone shall be removed from the well site and service road. The

16” x 80’ culvert shall be removed from the service road and disposed of. The cellar shall be removed from around the well head. The temporary wall constructed between the well and house shall be removed and disposed of. All disturbed areas will be smoothed, seeded, and mulched per
