This is a Technical Response form (PMT-59) for the Florida Department of Children and Families' Elevator Maintenance and Repair solicitation in the Northwest Region. The document serves as the technical submission component of an Invitation to Bid for comprehensive elevator maintenance and repair services covering three facilities located in Tallahassee, Quincy, and Marianna. Respondents must submit this form along with Required Certifications (PMT-58) and Cost Proposal (PMT-60) by January 8, 2026, at 2:00 PM, with a public bid opening scheduled for 3:00 PM the same day via video conference. An optional pre-bid site visit period runs from November 24 through December 12, 2025. The contract term spans five years from February 1, 2026, through January 31, 2031, with the Department intending to award to the responsible and responsive respondent submitting the lowest responsive bid.
The Technical Response form requires respondents to disclose related parties and affiliates, identify any conflicts of interest involving State of Florida employees, and list any State or Federal government contracts terminated for cause or by the respondent within the previous five years. Respondents must demonstrate capability to perform the work by describing their organization and years in business, providing experience information for similar services, and submitting details on three comparable entities served. The form includes sections for proposed subcontractors and statements regarding the respondent's and any subcontractor's ability to successfully complete the work. Respondents must hold current certification from the Florida Building Code and Inspectors Board and maintain general liability insurance with minimum limits of $1,000,000 per occurrence and $2,000,000 general aggregate, along with workers' compensation and employer's liability insurance.
