About this file

This document provides detailed instructions for completing two forms related to a Service Order (SO): the FMSR Form: SO Validation and the KO Form: SO Submission. The forms are designed for tracking and documenting maintenance work for fuel systems, primarily for military facilities. Key details include:

The FMSR Form requires comprehensive information such as service order number, contractor details, facility information, military service branch, contract number, maintenance category, and service order description. Part II involves FMSR validation signatures, while Part III focuses on evaluating the contractor's quality of work. The KO Form captures additional submission details like sub-contractor information, expected work start dates, long lead items, and unique identifiers. Contractors must submit these forms as part of a PDF portfolio including the completed forms, complexity matrix, Activity Hazard Analysis (AHA) sheet, and work plan, with all required digital signatures from authorized representatives.
