Vendor shall certify that item quoted meets the capabilities, features and specifications listed below and that vendor can assume all contractor responsibilities stated below by initialing at the bottom. In order to be considered for award, this must be completed and submitted with the vendor quote.

Shall be provided with the following capabilities, features and specifications: Yes No

Contractor capable of performing/meeting all requirements/specifications scope of work as cited within the Statement of Work (Attachment B through F)

Contractor has an active DOT Number and meets government requirements for Out of Service averages based on the DOT SAFER website.

Contractor meets government requirement for experience transporting radioactive material or hazmat.

Contractor availability: capable of meeting the June need by dates.
