Vehicle(s) have current FMCSA Annual Mechanical Inspections within the previous 12 months.

Commercial vehicles(s) have a recent independent mechanical inspection (for example, a current CVSA inspection, FMCSA Annual Mechanical Inspection no more than 90 days old, or valid state equivalent inspection no more than 90 days old, etc.).

Driver has CDL with Hazardous Material Endorsement.

Company has a DOT registration.

Visible Frame and Body (attributes would not affect proper operation)

· Free of visual cracks, damage, missing supports.

· Van/Closed trailer: Side wall/roof intact and adequate, doors operable.

· Tie-down points adequate, no broken welds.

· Cargo area is adequate for blocking/bracing, no unknown debris or fluid.

· ISO locks functional (if needed).

· No rust other than superficial surface rust.

Tires - no ply or belt material exposed, no leaks or flats (visual), Front equal to or greater than 4/32 inch tread (no recaps), Others greater than 2/32 inch tread.

Rims - not bent, cracked, or missing lug nuts. No rust other than superficial surface rust.

Fifth wheel is fully engaged.

· Is undamaged, no missing or broken mounting bolts or brackets.

· No visible space between upper and lower fifth wheel plate when engaged.

No leaking fluids (for example, anti-freeze, fuel, oil, hydraulic fluid).

Head lamps, clearance lights, tail/brake lights, turn signals, hazard lights operational, LED lights (less than 25% of diodes in LED array out).

Reflectors: At proper locations on trailer.

Windshield wipers operational.

Windows satisfactory and not obscured by decals or other materials.

Rear view mirrors functional.

Horn operational.

Visible air and electrical lines are satisfactory (connected, no air leaks, no visible damage).

Fire extinguisher charged, emergency reflectors and flares are present.

Axles: If oil filled axles, ensure oil level is satisfactory via sight glass.

Driver’s pre-trip inspection is complete.

Deviations or deficiencies are documented above and communicated to shipping organization management prior to loading the vehicle.
