About this file

This is a Tax Compliance Form issued by the State of Georgia, specifically for suppliers bidding on state and local contract opportunities. The form requires suppliers to provide comprehensive tax information, including their name, physical address, Federal Identification Number (FEI), and any prior registration history with Georgia. Suppliers must disclose their State Taxpayer Identification Number (STI), Sales and Use Tax Number, and Withholding Tax Number if applicable. The form also requests details about the type of service to be performed, whether tangible personal property will be sold, and information regarding any affiliate entities. Additionally, suppliers must identify the person responsible for handling their tax issues, including contact information.

Suppliers are notified that upon contract award consideration, the submitted information will be forwarded to the Georgia Department of Revenue (DOR) to verify that the supplier is not a "prohibited source" as defined by O.C.G.A. 50-5-82 and to identify any outstanding tax issues. The form emphasizes that missing, incomplete, or erroneous data may delay or prohibit contract award eligibility, and suppliers are strongly encouraged to resolve any outstanding tax liabilities or missing tax returns prior to submission. State entities must submit the completed form to the DOR via email at tsd-state-contractors@dor.ga.gov for processing in accordance with the Georgia Procurement Manual.
