About this file
This is a Supplemental Bidder Attestation form issued by the Rhode Island Division of Purchases under R.I. Gen. Laws 37-2-13.1(b) for use in state procurement solicitations. The form is applicable to the MPA 636 RIDE High Quality Curricula and Professional Learning in Social Studies solicitation, which seeks proposals from vendors offering K-12 core and/or supplemental curriculum materials in Social Studies or Social Studies professional learning services. Bidders are required to complete and submit this attestation as part of their proposal response.
The attestation requires the bidding entity and its parent corporation, subsidiaries, affiliates, and subcontractors to certify the absence of conflicts of interest with any state official, officer, or agency involved in the solicitation, and to confirm that they did not materially participate in or consult on the development of the Request for Proposal. The form further requires certification that the solicitation does not pertain to any audit, examination, or evaluation of the bidder's work, financials, or operations. An authorized signatory must complete all required fields, including the vendor or entity name, solicitation number or title, date, and signature, with the attestation made under penalty of perjury. The document clarifies that prior vendor status with the state or participation in a Request for Information does not automatically create a disqualifying conflict of interest.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the
State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
