Supplemental Bidder Attestation Summary
This is a Supplemental Bidder Attestation form issued by the Rhode Island Division of Purchases that applies to the Rhode Island Department of Environmental Management (RIDEM) Request for Quotation 26006077 for memorial benches and commemorative plaques. Bidders are required to complete and submit this attestation certifying that their entity and all parent corporations, subsidiaries, affiliates, and subcontractors have no conflict of interest as defined under Rhode Island General Laws Chapter 14 of Title 36 with any state official, officer, or agency involved in the procurement. The form requires certification that the bidder has not materially participated in or been consulted regarding the requirements, technical aspects, or formation of the RFQ. The solicitation period runs from April 6, 2026, through May 4, 2026, with vendor questions accepted through April 20, 2026. The initial contract period will commence approximately July 1, 2026, and extend through June 30, 2027, with options for renewal for up to two additional 12-month periods.
The attestation form must be signed by an authorized representative of the bidding entity and submitted as part of the bid proposal. Bidders should note that simply being a current or past vendor to the State does not automatically create a conflict of interest requiring preclusion, and prior responses to a Request for Information (RFI) do not constitute participation or consultation that would disqualify a vendor from bidding on a subsequent RFP. A 15 percent minority business enterprise (MBE) participation requirement applies to the total dollar value of the procurement. All vendors must be registered in Ocean State Procures to submit responses, and a contract administrative fee of 0.33 percent of annual spend will be withheld by the State at invoice payment.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the
State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
