About this file

This is a Substitution/Value Engineering Request Form for the City of Pasadena's FY27 CDBG Street Resurfacing and ADA Improvement Project. The form is used to request approval for substituting specified materials or products with alternative items during the construction of the street resurfacing project, which includes work on Fair Oaks Avenue from Washington Boulevard to Mountain Street and Los Robles Avenue from Washington Boulevard to Orange Grove Boulevard. Contractors submitting substitution requests must provide comprehensive technical documentation, including product descriptions, manufacturer catalogs, certified laboratory test reports, installation instructions, and manufacturing warranties. The form requires detailed comparison of the proposed substitution with the original specification, identification of any impacts to project drawings and dimensions, and documentation of effects on other trades and code compliance.

The form requires contractors to provide cost estimates for the substitution, identify any impacts to contract time and critical path scheduling, and submit references from three similar projects where the proposed product was installed. Contractors must include manufacturer letters certifying fitness for intended use and performance equivalence with specified items. The contractor submitting the request must execute an affidavit under penalty of perjury confirming personal investigation of the substitution, acceptance of warranty and correction responsibility equivalent to the specified item, completeness of cost data, agreement to pay all redesign and engineering costs, and assumption of all responsibility for direct or indirect costs and time impacts resulting from the substitution. The City of Pasadena maintains final approval authority and may accept, accept with conditions, reject, or deny requests received after specified deadlines.
