Subcontractor Summary Spreadsheet for City of St. Petersburg
This is a subcontractor summary worksheet instruction document issued by the City of St. Petersburg for use in competitive bidding processes. The document establishes the framework and requirements for prime contractors to disclose all intended subcontractors at the time of bid submission. Prime contractors must complete and submit the worksheet with their bid, identifying each subcontractor's legal name, primary business address, project supervisor contact information, trade classification, and subcontract amount. The worksheet distinguishes between first-tier and second-tier subcontractors, with restrictions on multiple-tier subcontracting unless otherwise specified in contract documents. Bidders unable to utilize subcontractors must provide a written statement indicating no subcontractors will be used for the project.
Subcontractors identified in the awarded bid may not be changed or substituted except under specific circumstances including failure to execute contracts, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure to perform, substantial unsatisfactory work quality, or substantial delays to project progress. Any requested substitutions must be submitted in writing to the Procurement & Supply Management Contracts Compliance Manager. For projects with Small Business Enterprise (SBE) or Minority Women Business Enterprise (MWBE) requirements, replacement subcontractors must also meet the same certification status unless the prime contractor demonstrates compliance through alternative means. These restrictions do not apply to FDOT LAP or federally funded projects. The worksheet requires approval and documentation of all additions or deletions with reason codes, dates, and city approver signatures.
