About this file

This is a Subcontractor Summary Worksheet for the City of St. Petersburg, Florida, designed to accompany bid submissions for a Shelter project. The document serves as a standardized form requiring prime contractors to disclose all intended subcontractors, their contact information, business addresses, trade classifications, and subcontract amounts prior to bid award. The worksheet establishes that subcontractors may comprise up to 80 percent of the contract value for this shelter bid. Prime contractors must complete the form with the name and date of the employee responsible for its preparation and identify whether each subcontractor qualifies as a Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) contractor. The document specifies that subcontractors may only be changed or substituted post-award under seven enumerated circumstances, including failure to execute a contract at the specified price, lack of appropriate licensing, bankruptcy or insolvency, ineligibility, failure to perform, substantial unsatisfactory work, or material project delays or disruption.

The worksheet restricts subcontractor substitutions to maintain workforce stability and contract integrity following award. SBE subcontractors used to meet percentage requirements must be replaced with other SBE subcontractors unless the prime contractor demonstrates SBE obligations are satisfied through alternative means. All requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management office, Attention: Contracts Compliance Manager, at Stephanie.Swinson@stpete.org. The form includes columns for tracking additions and deletions with corresponding reason codes and city approver signatures and dates. The worksheet requires a total subcontract amount calculation and computation of the allowable subcontract amount based on the contract price. The document was revised in January 2020 and applies to City of St. Petersburg construction projects unless otherwise specified in contract documents or for FDOT LAP or federally funded projects, which are exempt from subcontracting percentage restrictions.
