About this file
This is a Subcontractor Summary Worksheet template issued by the City of St. Petersburg, Florida, for use by prime contractors bidding on municipal construction projects. The worksheet requires prime contractors to disclose all intended subcontractors, including their legal names, business addresses, contact information, trade classifications, and subcontract amounts prior to bid submission. The form mandates that contractors identify whether subcontractors are Small Business Enterprises (SBEs) or Minority/Women Business Enterprises (MWBEs), designate them as first or second-tier subcontractors, and indicate whether entries represent additions or deletions to the subcontractor list. Once a prime contractor is awarded a project and the pre-construction meeting is held, subcontractors cannot be changed or substituted except under specific circumstances including failure to execute a contract, lack of proper licensing, bankruptcy or insolvency, ineligibility to perform work, failure to perform the subcontract, substantial unsatisfactory work performance, or substantial project delays or disruption.
The worksheet imposes restrictions on subcontracting percentages and prohibits multiple-tier subcontractors beyond the second tier unless otherwise specified in contract documents, with exceptions for FDOT LAP or federally funded projects. If SBE subcontractors are used to meet participation requirements and subsequently replaced, they must be replaced with other SBE subcontractors unless the prime contractor demonstrates that SBE requirements are being met through alternative means. Any requests to modify the subcontractor list after contract award must be submitted in writing to the City's Procurement & Supply Management department, specifically the Contracts Compliance Manager at Stephanie.Swinson@stpete.org. The worksheet was revised in January 2020 and serves as a mandatory component of bid submissions for City of St. Petersburg construction projects.
