About this file

This is a Subcontractor Summary worksheet for the City of St. Petersburg, Florida, designed to accompany bid submissions for the Faith Community Housing Assistance Consultant project. The worksheet must be completed and submitted with all bids and requires prime contractors to identify all intended subcontractors, their business addresses, contact information, trade classifications, and subcontract amounts. Up to 80 percent of the contract work may be subcontracted for this shelter-related project. Subcontractors are classified as either first or second tier, with only these tiers permitted unless otherwise specified in the contract documents. The worksheet provides instructions for identifying whether subcontractors are Small Business Enterprises (SBE) or Minority and Women-Owned Business Enterprises (MWBE), and requires designation of whether each subcontractor entry represents an addition or deletion from the subcontractor list.

Subcontractor substitutions are restricted once a prime contractor is awarded and the pre-construction meeting is held, with changes permitted only under specific circumstances including failure to execute a contract at the specified price, lack of appropriate licensing, bankruptcy or insolvency, ineligibility, failure to perform work, substantial unsatisfactory performance, or substantial project delays. SBE subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substituted. All requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management department, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org. The worksheet includes reason codes for deletions and requires approval signatures from city personnel with dates.
