Subcontractor Summary Spreadsheet - City of St. Petersburg
This document is a Subcontractor Summary Worksheet template issued by the City of St. Petersburg, Florida, designed to track subcontractor information for city construction and procurement projects. The worksheet requires prime contractors to identify all intended subcontractors, including their legal names, business addresses, contact information, and the specific trade classifications being subcontracted. Contractors must complete and submit this form with their bid if subcontractors will be utilized; alternatively, they must provide a written statement indicating no subcontractors will be used. The worksheet includes fields for the prime contractor name, the name and date of the employee completing the form, subcontractor tier designation (first or second tier only, unless otherwise specified in contract documents), subcontract amounts, and tracking of any additions or deletions to the subcontractor list with associated reason codes.
The spreadsheet includes provisions governing subcontractor substitution, which are permitted only under specific circumstances including failure to execute a contract, lack of appropriate licensing, bankruptcy or insolvency, ineligibility, failure to perform work, substantial unsatisfactory performance as determined by the engineer, or substantial project delays or disruption. Notably, any SBE subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substituted, unless the prime contractor demonstrates SBE requirements are being met through alternative means. Subcontractor changes must be requested in writing to the Procurement & Supply Management office with the Contracts Compliance Manager. The instructions clarify that restrictions on subcontracting percentages and multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects, and all subcontractor information is subject to city review and approval prior to project award.
