About this file
This is a Subcontractor Summary Worksheet instruction document issued by the City of St. Petersburg for use in bidding on municipal contracts. The worksheet must be completed and submitted with all bids and serves to document all intended subcontractors and their scope of work for projects where subcontracting is permitted. Bidders must enter their company name as the prime contractor, provide the name and date of the employee completing the form, and list all subcontractors with complete legal names, business addresses, project supervision contact information, trade classifications, and subcontract amounts. The City permits subcontracting up to a maximum percentage specified for each project, with restrictions limited to first and second tier subcontractors only. Once a prime contractor is awarded the project and the pre-construction meeting is held, subcontractors may not be changed or substituted except under seven specific circumstances: failure or refusal to execute a written contract at the specified price, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform the work, failure or refusal to perform the subcontract, substantially unsatisfactory work performance not in accordance with plans and specifications, or substantial delay or disruption to project progress.
Subcontractor changes must be requested in writing to the City's Procurement & Supply Management at Stephanie.Swinson@stpete.org, attention Contracts Compliance Manager. Small Business Enterprise (SBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substituted, unless the prime contractor demonstrates that SBE requirements are being met through alternative means. The worksheet includes fields for the City to track additions and deletions of subcontractors with associated reason codes and approval signatures. Restrictions on subcontracting percentage and multi-tier subcontractor limitations do not apply to Federal-Aid Project (FDOT LAP) or federally funded projects.
