About this file

This is a Subcontractor Summary Worksheet instruction document issued by the City of St. Petersburg, Florida, for use with a sheltered bid solicitation. The worksheet serves as a mandatory submittal requirement for all bidders and establishes the framework for identifying and documenting subcontractors on the project. Bidders must complete the form by entering their company name as the prime contractor, providing the name and date of the employee completing the form, and listing all intended subcontractors with their complete legal names, primary business addresses, employee contact information, and project supervision details. The document specifies that subcontractors may not be changed or substituted after award and the pre-construction meeting except under seven defined circumstances: failure or refusal to execute a written contract at the specified price, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure or refusal to perform the subcontract, substantially unsatisfactory work performance, or substantial delays or disruptions to project progress.

The City of St. Petersburg permits up to 80 percent of the contract work to be subcontracted for this shelter bid project, with restrictions limited to first and second tier subcontractors only. The worksheet requires identification of whether each subcontractor is classified as a Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) contractor, the specific trade classifications being subcontracted, and the subcontract dollar amount for each line item. Any requests to change the subcontractor list after award must be submitted in writing to the Procurement & Supply Management department with the Contracts Compliance Manager. If SBE subcontractors are used to satisfy SBE percentage requirements and subsequently replaced, they must be replaced with another SBE subcontractor unless the prime contractor demonstrates that SBE requirements are being met through alternative means. The document notes that subcontracting percentage restrictions and multi-tier subcontractor limitations do not apply to FDOT LAP or federally funded projects.
